The Effect of Preoperative Serratus Anterior Muscle Plane Block on Persistent Postsurgical Pain After Video-assisted Thoracic Surgery: A Retrospective Cohort Study.

OBJECTIVES: Acute pain intensity is a strong indicator for the occurrence of persistent postsurgical pain (PPP) after video-assisted thoracic surgery (VATS). The effect of preoperative serratus anterior muscle plane block (SAPB) on PPP was investigated. MATERIALS AND METHODS: In this retrospective cohort study, data of 148 patients who underwent VATS for lung nodules in 2019 were collected. Seventy-four patients received a single-shot of SAPB with 0.33% ropivacaine 30 mL after anesthesia induction and another 74 patients received intercostal blocks using 1% ropivacaine 10 mL at the fifth to s

Is the Glasses-Free 3-Dimensional Display System More Effective for Complex Video-Assisted Thoracic Surgery? A Self-Controlled Study Ex Vivo.

Objective. Considering the demerits of a high-definition 2-dimensional (HD-2D) system, with its lack of stereopsis, and a conventional 3-dimensional (C-3D) system, which results in a dimmed image, we have recently developed a glasses-free 3-dimensional (GF-3D) display system for reconstruction surgeries such as video-assisted thoracic surgery (VATS) for tracheal reconstruction. Methods. Thoracic surgeons were invited to complete thoracoscopic continuous suture of a transected porcine trachea using the HD-2D, C-3D, and GF-3D systems on separate mornings in randomized order. The duration, number

Chest tube drainage versus video-assisted thoracoscopic surgery for a first episode of primary spontaneous pneumothorax: a systematic review and meta-analysis.

Primary spontaneous pneumothorax affects up to 28 patients per 100 000 population yearly and is commonly resolved by chest tube drainage. However, drainage is also known to be associated with ipsilateral recurrence rates ranging from 25% to 43%. Preventive video-assisted thoracoscopic surgery (VATS) may be an effective alternative to diminish these recurrence rates and its associated morbidity. The aim of this study was to compare the efficacy of chest tube drainage and VATS as first line treatments of an initial episode of primary spontaneous pneumothorax. The MEDLINE, EMBASE, CENTRAL and Cli

Effect of lidocaine cream analgesia for chest drain tube removal after video-assisted thoracoscopic surgery for lung cancer: a randomized clinical trial.

BACKGROUND AND OBJECTIVES: Pain management makes an important contribution to good respiratory care and early recovery after thoracic surgery. Although the development of video-assisted thoracoscopic surgery (VATS) has led to improved patient outcomes, chest tube removal could be distressful experience for many patients. The aim of this trial was to test whether the addition of lidocaine cream would have a significant impact on the pain treatment during chest tube removal from patients who had undergone VATS for lung cancer. METHODS: This clinical trial was a double-blind randomized study. For

Uniportal video-assisted thoracoscopic surgery (VATS) sleeve resections for non-small cell lung cancer patients: an observational prospective study and technique analysis.

Bronchus sleeve resection for operative treatment of non-small cell lung cancer (NSCLC) is a gold standard in modern thoracic surgery in cases of centrally located tumors or hilär lymph node metastases. Advanced instruments and growing surgical experience allowed surgeons to reduce the required incisions (from 3-port to uniportal) and to resect larger and more centrally located malignancies minimal invasively. It is a logical and expected advance in thoracic surgery that video-assisted thoracoscopic surgery (VATS) would be ultimately used also for complex bronchial resections. We therefore pre

Body surface area as a novel risk factor for chylothorax complicating video-assisted thoracoscopic surgery lobectomy for non-small cell lung cancer.

BACKGROUND: The study was conducted to demonstrate the predictive value of body surface area (BSA) for chylothorax complicating video-assisted thoracoscopic surgery (VATS) lobectomy for non-small cell lung cancer (NSCLC). METHODS: Large-scale retrospective analysis was conducted on the data of 1379 patients who underwent VATS lobectomy between January 2014 and October 2017 at our institution. Receiver operating characteristic analysis was conducted to determine a threshold BSA value for the prediction of chylothorax. This optimal BSA cutoff, other clinicopathological variables, and P < 0.15 w

Body surface area is a novel predictor for surgical complications following video-assisted thoracoscopic surgery for lung adenocarcinoma: a retrospective cohort study.

BACKGROUND: Body surface area (BSA) is a biometric unit to measure the body size. Its clinical significance in video-assisted thoracoscopic surgery (VATS) was rarely understood. We aimed to estimate the predictive value of BSA for surgical complications following VATS anatomical resections for lung adenocarcinoma (LAC). METHODS: A single-center retrospective analysis was performed on the consecutive patients between July 2014 and January 2016 in our institution. The differences in mean BSA values were evaluated between groups of patients classified by the development of postoperative surgical

Video-assisted breast surgery can sample the second and third sentinel nodes to omit axillary node dissection for sentinel-node-positive patients.

BACKGROUND: The preservation of the axillary node (AN) has become standard therapy for early breast cancer patients with a metastasis-positive sentinel node (SN). However, about half of the patients with metastasis in the SN have no metastasis in the other AN. Late-phase three-dimensional computed tomographic lymphography (3D-CT LG) of the breast can show the axillary lymphatic architecture from the SN into the venous angle. These nodes are classified into five groups. For the sake of aesthetics, video-assisted breast surgery (VABS) was used to sample the second and third nodes shown by 3D-CT

Natural orifice surgery on thyroid gland: totally transoral video-assisted thyroidectomy (TOVAT): report of first experimental results of a new surgical method.

BACKGROUND: Neck surgery is one of the newest fields of application of minimally invasive surgery. The technique of minimally invasive video-assisted thyroidectomy (MIVAT) developed by Miccoli [1] is the method that has so far become most widespread. Limiting factors of this method include the bothersome 20-mm cervical incision and consequently the specimen size to remove. Several papers describing an access outside the front neck region have been published. Such approaches are via the chest, axillary, a combined axillary bilateral breast, or a bilateral axillary breast approach [2-5]. The dev

Uniportal versus Multiportal Video-Assisted Thoracoscopic Surgery for Minimally Invasive Esophagectomy in Esophageal Cancer: A GRADE-Assessed Systematic Review and Meta-Analysis.

BACKGROUND: Minimally invasive esophagectomy has become the preferred surgical approach for esophageal cancer, with uniportal video-assisted thoracoscopic surgery (uVATS) emerging as a potential refinement over conventional multiportal VATS (mVATS). However, comparative evidence remains limited to small single-centre studies. This systematic review and meta-analysis aimed to comprehensively evaluate perioperative and safety outcomes of uVATS versus mVATS in patients undergoing minimally invasive esophagectomy. METHODS: A systematic search was conducted across PubMed/MEDLINE, Embase, Scopus, C

Ipsilateral Recurrent Hemothorax Caused by Spontaneous Intercostal Artery Rupture Treated With Transcatheter Arterial Embolization and Video-Assisted Thoracoscopic Surgery: A Case Report.

Hemothorax caused by intercostal artery rupture is a potentially life-threatening disease. A 66-year-old woman presented to our emergency department with left back pain. She had undergone surgery for left hemothorax 14 years ago. Contrast-enhanced chest computed tomography revealed active bleeding from the left dorsal ninth intercostal artery near the descending aorta. Subsequently, she developed hypovolemic shock. Based on her medical history, the patient was suspected of having adhesions in the left thoracic cavity. Surgery was considered a time-consuming approach to control the bleeding. Ou

Perioperative Outcomes of Complex Versus Simple Segmentectomy Via Uniportal Video-Assisted Thoracoscopic Surgery for Lung Lesions: A Systematic Review and Meta-Analysis.

OBJECTIVES: Uniportal video-assisted thoracoscopic surgery (U-VATS) has been increasingly adopted for anatomical segmentectomy because of its minimally invasive nature and favourable recovery profile. However, whether perioperative outcomes differ between complex and simple segmentectomy when performed via U-VATS remains uncertain. This study compared perioperative outcomes between complex and simple U-VATS segmentectomy. METHODS: A systematic review and meta-analysis were conducted according to PRISMA 2020 and AMSTAR 2 guidelines, and the study was registered in PROSPERO (CRD420251151464). S

The use of remimazolam versus propofol for anesthesia induction in video-assisted thoracoscopic surgery: study protocol for a multicenter randomized controlled trial.

BACKGROUND: Intraoperative hypotension may result in a higher incidence of postoperative myocardial injury, acute kidney injury, and stroke. Notably, more than half of intraoperative hypotension cases occur immediately after induction of general anesthesia. Although intraoperative hypotension has multiple causes, post-induction hypotension is primarily due to the effects of anesthetic drugs. Propofol is the most widely used agent for anesthesia induction. However, propofol can induce hemodynamic instability, potentially leading to adverse postoperative outcomes. Remimazolam, a novel ultra-shor

Association of Erector Spinae Plane Block on Quality of Postoperative Recovery in Patients Undergoing Video-Assisted Thoracoscopic Surgery: An Observational Cohort Study.

INTRODUCTION: Video-assisted thoracoscopic surgery (VATS) reduces surgical trauma compared to thoracotomy, but postoperative pain and recovery remain important concerns. The erector spinae plane block (ESPB) is a promising regional anesthesia technique, yet its impact on patient-centered outcomes, such as quality of recovery, remains underexplored. METHODS: In this prospective observational study, 54 patients undergoing elective VATS at a tertiary center were included. Patients received either an ultrasound-guided ESPB plus the institutional protocol of general anesthesia with intercostal ner

Bolus intertransverse process block and continuous erector spinae plane block for perioperative analgesic management of video-assisted thoracoscopic surgery - Three cases report.

BACKGROUND: Common regional anesthesia approaches for video-assisted thoracoscopic surgery (VATS) include paravertebral block (PVB) and erector spinae plane block (ESPB). PVB is considered a deep nerve block which is contraindicated in antithrombotic therapy. ESPB is effective when administered as a bolus, as well as continuously. However, the recently proposed intertransverse process block (ITPB) ensures more effective diffusion of the local anesthetic into the paravertebral space. CASE: We report cases of three patients who received bolus ITPB (costotransverse foramen block and mid-point tr
