Double transection of complete duodenal circumference after blunt abdominal trauma without other intra-abdominal injuries.

We report a case of a 20-year-old male with isolated complete duodenal transection at two duodenal segments after blunt abdominal trauma. On admission, the patient underwent physical examination, laboratory analysis, abdominal ultrasound and plain abdominal x-ray. Physical examination revealed diffuse and rebound tenderness and extreme rigidity of the abdomen, guarding and decreased bowel sounds. Standard surgical techniques were used (median laparotomy, primary anastomosis, nasojejunal three-luminal tube for feeding and gastric decompression). At exploration, we found complete transection of

Hyperacute abdominal compartment syndrome: an unrecognized complication of massive intraoperative resuscitation for extra-abdominal injuries.

Primary and secondary abdominal compartment syndrome (ACS) are well-recognized entities after trauma. The current study describes a "hyperacute" form of secondary ACS (HACS) that develops intraoperatively while repair of extra-abdominal injuries is being carried out simultaneous with massive resuscitation for shock caused by those injuries. The charts of patients requiring abdominal decompression (AD) for HACS at time of extra-abdominal surgery at our level I trauma center were reviewed. The following data was gathered: age, Injury Severity Score (ISS), mechanism, resuscitation details, time t…

The management of abdominal injuries in the presence of head injury.

When head and blunt abdominal injuries are combined, the head injury is often afforded too much attention and the abdominal injury too little, especially when the patient is unconscious. If mismanaged, the abdominal injury is often the more serious threat to life. Except for extradural hemorrhage, neurosurgical intervention, when indicated, can be delayed until the patient has been thoroughly evaluated for the presence of extra cranial injuries with higher therapeutic priority. Abdominal examination of the unconscious or uncooperative patient is difficult. Tenderness as a sign of abdominal inj…

Blunt abdominal aortic injury.

BACKGROUND: Blunt abdominal aortic injury (BAAI) is a rare injury with less than 200 cases in the current reported world literature, mostly in case report format. We sought to describe the experience of a high-volume trauma center and to provide a contemporary review of the literature to better understand the natural history and management of this injury. METHODS: This was a retrospective review of patients with BAAI between 1996 and 2010. Data collected included demographics, mechanism of injury, associated injuries, type of intervention, subsequent imaging, and follow-up. BAAI was classifie

Seatbelt injury to the abdominal aorta.

Seatbelt injury to the abdominal aorta is a rare finding with an unresolved mechanism. We present a case in which a rear-seat passenger wearing a lap belt was involved in a motor vehicle accident. Injuries sustained included a contused abdominal aorta. At the time of aortic repair, the involved segment was found to be atheromatous, had an intimal tear, and a flap that caused complete occlusion. Graft replacement was carried out employing a 12-mm DeBakey woven dacron prosthesis, which restored satisfactory circulation to both lower limbs. The patient developed myoglobinemia and subsequently die

Blunt injury of the abdominal aorta.

This review of 27 cases of blunt injury of the abdominal aorta includes 24 cases reported in the literature and three new cases. Automobile accidents were the cause of this condition in 19 patients (70%). Clinical presentation was acute in 70% of the cases, and consisted of either acute arterial insufficiency or an acute abdomen. Intimal disruption occurred in 15 patients (55%) and was the most common anatomic lesion. Atherosclerotic involvement of the aorta was found in ten patients (37%), and in four it contributed directly to the development of this condition. The infrarenal aorta was the m

Laparoscopy versus laparotomy for pediatric patients with abdominal firearm injuries.

BACKGROUND: Current guidelines recommend exploratory laparotomy for pediatric patients with abdominal firearm injuries. This study compared clinical outcomes of hemodynamically stable pediatric patients with abdominal firearm injuries managed with laparoscopy versus laparotomy. METHODS: A retrospective, intention-to-treat study was conducted using the Pediatric Trauma Quality Improvement Program database. Pediatric patients (age <20) with abdominal firearm injuries from 2010 to 2020 were sorted into laparotomy (n = 1806) and laparoscopy (n = 48) groups. RESULTS: Laparoscopy was associated wi

Abdominal aortic injuries associated with chance fractures in pediatric patients.

Injury to the abdominal aortic artery from blunt trauma is an infrequent event with few cases reported in either the adult or the pediatric literature. Injury to the thoracic aorta after blunt abdominal trauma is more common. It has been estimated that 95% to 99% of all aortic disruptions are in the thoracic region. Injury to the abdominal aorta is rarely seen in association with fractures of the lumbar spine secondary to seat belt use in motor vehicle accidents; there are few cases in the literature of these injuries in pediatric patients. Nevertheless, the overall mortality rate of this inju

Identification of children with intra-abdominal injuries after blunt trauma.

STUDY OBJECTIVE: We sought to determine the utility of laboratory testing after adjusting for physical examination findings in the identification of children with intra-abdominal injuries after blunt trauma. METHODS: The study was a prospective observational series of children younger than 16 years old who sustained blunt trauma and were at risk for intra-abdominal injuries during a 2(1/2)-year period at an urban Level I trauma center. Patients were examined by faculty emergency physicians and underwent standardized laboratory testing. Clinical and laboratory findings were recorded on a stand…

Battlefield advanced trauma life support (BATLS). Chapter 7. Abdominal injuries.

Recognise the possibility of abdominal injury. Two major types of abdominal injury may be present in war, blunt and penetrating. In either case, early surgery is essential. With blunt injuries, a high index of suspicion is required. Diagnosis in the early stages may be very difficult. Serial examination is required; even then, evacuate early. Aids to diagnosis include: DPL, FAST, CT Casualties with penetrating injuries must be given the highest priority for evacuation and surgical evaluation. Penetrating injury to the loin, back, buttocks, lower chest and thighs, should raise suspicion of intr

Mean time loss due to injury was significantly greater for abdominal injuries (10.59 sessions) than for groin injuries (6.59 sessions). A conservative estimate of the impact of groin/abdominal injury on each NHL team is a game loss of 25 player games/year. CONCLUSION: The impact of groin and abdominal strain injury at an eli …

Non-operative management for abdominal solidorgan injuries: A literature review.

The philosophy of abdominal injury management is currently changing from mandatory exploration to selective non-operative management (NOM). The patient with hemodynamic stability and absence of peritonitis should be managed non-operatively. NOM has an overall success rate of 80%-90%. It also can reduce the rate of non-therapeutic abdominal exploration, preserve organ function, and has been defined as the safest choice in experienced centers. However, NOM carries a risk of missed injury such as hollow organ injury, diaphragm injury, and delayed hemorrhage. Adjunct therapies such as angiography

The management and outcome of pediatric blunt chest-abdominal injuries.

BACKGROUND: The aim of this study was to determine the frequency and nature of pediatric blunt chest-abdominal injuries (BCAIs) and to summarize their management, ranging from non-operative management (NOM), with or without angioembolization (AE), to surgical treatment. METHODS: This retrospective study included patients admitted to our hospital for BCAIs from January 1996 to December 2017. The age, injury pattern, organs of injury, outcome, and treatment were summarized. RESULTS: One hundred and thirty-two patients (98 males, 34 females, mean age 7.68 years ± 3.58, range 1-15 years) were in…
