Despite the recent advent of and the successful results from catheter ablation, pharmacological therapy is still used by most clinicians as the first line therapy in patients with regular supraventricular tachycardias. Before prescribing an antiarrhythmic agent, documentation of the arrhythmia using a 12-lead electrocardiogram (ECG) is necessary to identify the type of tachycardia. The ECG diagnosis is based on the presence and polarity of the P wave, the P to QRS relationship, the presence of QRS alternation and the effect of bundle branch block on tachycardia rate. Most regular supraventricu

Forty patients with normal resting ECGs and a history of paroxysmal junctional tachycardia underwent endocavitory electrocardiography. Accessory atrioventricular pathways were demonstrated in 34 patients (82.5%), 14 of whom (35%) had Kent bundles. The ventriculo-atrial conduction time during ventricular stimulation was constantin 85% of the 40 patients but increased after injection of striadyne (ATP). This may suggest a reentry circuit partially bypassing the atrioventricular node.

We report a rare case of broad complex atrioventricular reentrant tachycardia (AVRT) in an 18-year-old female, mediated by two distinct decrementally conducting accessory pathways (APs). The reentrant circuit consisted of antegrade conduction via an atrio-fascicular (AF) fiber and retrograde conduction through a concealed posteroseptal AP. Both APs exhibited decremental properties, and the circuit was independent of the atrioventricular (AV) node and His-Purkinje system, representing a classic example of pathway-to-pathway reentry. Sequential ablation of the retrograde and then the antegrade l

BACKGROUND: Most Mahaim fibers are right free-wall atriofascicular accessory pathways with only antegrade conduction. Concealed Mahaim fiber is not very rare; however, concealed nodoventricular fiber is a very rare kind of retrograde accessory pathway in supraventricular tachycardia with atrioventricular (AV) dissociation. Only a few cases about successful ablation of the nodoventricular accessory pathway have been reported. We describe the case of a 32-year-old woman who underwent an electrophysiology study and radiofrequency (RF) ablation of a rare narrow QRS tachycardia with AV dissociation

BACKGROUND: Accessory pathways (APs) are muscular bundles directly connecting the atria and ventricles, bypassing the atrioventricular (AV) node-His-Purkinje system. Anterograde conduction along the AP results in ventricular preexcitation. A retrograde conducting AP allows ventriculo-atrial (VA) conduction, creating a reentry circuit that mediates orthodromic atrioventricular reentry tachycardia (OAVRT). This condition is well described in humans and small animals, but has not been reported previously in horses.
OBJECTIVES: Describe clinical and electrocardiographic findings of horses with re

We present an update on clinical and electrophysiological criteria used for the differential diagnosis of regular supraventricular tachycardias. Although several electrocardiographic clues may assist in the differential diagnosis, this is usually accomplished at electrophysiology study, and most often, the differential diagnosis is between atrioventricular nodal reentrant tachycardia, atrioventricular reentrant tachycardia due to a concealed accessory pathway, and atrial tachycardia. Atrial and ventricular pacing maneuvers during sinus rhythm or tachycardia have been used with various success

INTRODUCTION: Left-sided accessory pathway (AP) with atrial insertion away from the annulus is an atypical variation. Conventional mapping and ablation performed along mitral annulus (MA) is usually ineffective.
METHODS: A 14-year-old girl without structural heart disease presented with recurrent episodes of sudden onset palpitations and electrocardiogram (ECG) showed a narrow QRS complex tachycardia.
RESULTS: Electrophysiology study (EPS) was done and anterograde atrioventricular reentrant tachycardia (AVRT) with AP was diagnosed. Conventional mapping and ablation performed along TA and MA

Three atrioventricular reciprocating tachycardias (AVRTs) using 3 accessory pathways (APs) occurred with the His bundle eliminated by the previous ablation. Two AVRTs using 2 right APs rotated reversely, and the other AVRT was one with a retrograde conduction through the left AP and anterograde conduction through the 2 right APs.

A 59 year old patient with a left postero-septal Kent bundle had daily attacks of reciprocating tachycardia resistant to anti-arrhythmic therapy and was referred for catheter ablation. Kent bundle activity was recorded in the proximal part of the coronary sinus. An endocavitary electric shock delivered at this site suppressed conduction through the Kent bundle for several minutes. Catheter ablation of the His bundle (2 X 200 joules) was then attempted suppressing anterograde conduction for 2 days, after which conduction was reestablished and the reciprocating tachycardia recurred. A second ses

AIMS: Seeking to account for accessory atrioventricular conduction potentially leading to ventricular pre-excitation, Mahaim in the mid-20th century had described pathways between the atrioventricular conduction axis and the muscular ventricular septum. We aimed to look for such 'paraspecific' connections in adult human hearts.
METHODS AND RESULTS: We serially sectioned 21 hearts, covering the triangle of Koch and the aortic root, and assessing the atrioventricular node, the penetration of the conduction axis, and the bundle branches in our search for fasciculo-ventricular connections. We als

A wide complex tachycardia may result from supraventricular tachycardia (SVT) with bundle branch block (pre-existing or tachycardia-related), SVT with atrioventricular conduction over an accessory pathway (AP), or ventricular tachycardia (VT). The timing of PACs and their effect on tachycardia can help differentiate between atrioventricular nodal re-entry tachycardia variants and AP-mediated tachycardias. A proximal coronary sinus recording is critical for evaluating septal refractoriness and excluding nodal tachycardia and other forms of nodal bystander pre-excitation (such as nodoventricular

BACKGROUND: Wolff-Parkinson-White (WPW) syndrome is characterized by ventricular pre-excitation, which can lead to severe arrhythmic events such as supraventricular tachycardia and pre-excited atrial fibrillation. The diagnostic value of non-invasive exercise tests in detecting high-risk accessory pathways remains inconsistent in the literature.
OBJECTIVES: To evaluate the diagnostic accuracy of non-invasive exercise tests compared to invasive electrophysiological studies (EPS) for identifying high-risk accessory pathways in WPW syndrome.
METHODS: Following PRISMA-DTA guidelines, a comprehen

Abnormal development of the atrioventricular ring can lead to the formation of a bypass pathway and the occurrence of Wolff-Parkinson-White (WPW) syndrome. The genetic mechanism underlying the sporadic form of WPW syndrome remains unclear. Existing evidence suggests that both T-box transcription factor 3 (TBX3) and T-box transcription factor 2 (TBX2) genes participate in regulating annulus fibrosus formation and atrioventricular canal development. Thus, we aimed to examine whether single-nucleotide polymorphisms (SNPs) in the TBX3 and TBX2 genes confer susceptibility to WPW syndrome in a Han C

Electrocardiographic tracings of an English Bulldog referred for cardiogenic shock due to an orthodromic atrioventricular reciprocating tachycardia conducted with intraventricular conduction disturbance and mimicking ventricular tachycardia (VT) are presented. At admission the surface ECG showed a wide QRS complex tachycardia (WCT) that was converted to sinus rhythm using manual cardioversion (chest thump). This change revealed pre-existing right bundle branch block, and a final diagnosis of supraventricular tachycardia (SVT) with intraventricular conduction disturbance was made. Electrophysio

Para-Hisian accessory pathways (PHAPs) remain among the most challenging substrates for catheter ablation because of their close anatomical relationship to the atrioventricular (AV) conduction system and the consequent risk of iatrogenic AV block. The non-coronary cusp (NCC) has emerged as a valuable alternative when right-sided ablation is ineffective or associated with an unacceptable risk of injury to the His bundle. We report the case of a 23-year-old man who presented with pre-excited atrial fibrillation. Electrophysiological study localized an anteroseptal/para-Hisian accessory pathway.

This paper reviews the anatomical substrates responsible for the induction and maintenance of supraventricular tachycardia and discusses the ECG findings associated with these tachycardias. The normal anatomy of the supraventricular conducting system, particularly within the atria, is complex with conduction proceeding along preferential pathway, which are in turn determined in part by the anisotropic properties of the atrial myocardium. There appear to be at least dual inputs to the atrioventricular node, a posteriorly situated slow pathway and an anterior fast pathway. It is sometimes possib
