Acetazolamide for Bipolar Disorders: A Scoping Review.

Acetazolamide, a carbonic anhydrase inhibitor, is used to treat a variety of ailments. It has been highlighted for its potential to benefit people with bipolar disorders, for whom there are clear current unmet treatment needs. This scoping review sought to synthesise all available evidence related to the potential effects of acetazolamide on symptoms related to bipolar disorder, acceptability and tolerability, and intervention characteristics (e.g., dose and duration). Following publication of the review protocol, the Pubmed, Embase, and PsycInfo databases were searched (all dated to 31 August…

Usefulness of acetazolamide in the management of diuretic resistance.

Worsening symptoms and fluid overload are the hallmarks of heart failure (HF) decompensation, and fluid removal is central to improvement. Despite high-dose loop diuretics, patients with decompensated HF may develop suboptimal diuresis/diuretic resistance. Sequential nephron blockade with a combination of loop and thiazide/thiazide-like diuretics may be insufficient, resulting in poor outcomes. We present a case wherein urine output improved significantly with acetazolamide. Although the diuretic capacity of acetazolamide is weak on its own, it might be efficient in aiding the efficacy of loop…

Acetazolamide for the treatment of acute mountain sickness.

INTRODUCTION: Acute mountain sickness is the most prevalent illness related to acute exposure to high altitude, secondary to the hypobaric hypoxia effects in our body. Acetazolamide has been traditionally used for its prevention and treatment, however, there is still controversy regarding the degree of usefulness of this medication as monotherapy. METHODS: We searched in Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the systematic revi…

Acetazolamide for the treatment of idiopathic intracranial hypertension.

Idiopathic intracranial hypertension (IIH) is characterized by an increase of intracranial pressure in the absence of neurologic tumors. The sulfonamide carbonic anhydrase (CA, EC 4.2.1.1) inhibitor (CAI) acetazolamide (AAZ), a compound developed in the 1950s as a diuretic drug and presently used as an antiglaucoma, antiepileptic and diuretic agent, is effective in the treatment of IIH. AAZ is a low nanomolar inhibitor of CA isoforms involved in cerebrospinal fluid (CSF) secretion. Inhibition of brain/choroid plexus CA II, IV, VA and XII leads to a decreased CSF fluid secretion and control of …

Treatment of Kleine-Levin syndrome with acetazolamide.

Kleine-Levin syndrome (KLS) is a rare and disabling disorder characterized by recurrent episodes of hypersomnia as well as cognitive and behavioral abnormalities. Sadly, the treatment options are very limited and often ineffective. To our knowledge, there have been no reports in the literature for the use of acetazolamide as a treatment for this disorder. We report the case of a 26-year-old female patient with KLS in whom acetazolamide was used to treat recurrent hypersomnia and cognitive abnormalities. The patient had a dramatic improvement on acetazolamide in both the frequency and severity …

Acetazolamide and prevention of acute mountain sickness.

When planning a rapid ascent to an altitude above 3000 metres, acetazolamide 250 mg daily reduces the risk of acute mountain sickness, with generally moderate adverse effects.

Oral acetazolamide: A treatment option for blepharochalasis?

This is a report of a middle-aged male with blepharochalasis, who was successfully treated with oral acetazolamide. To our knowledge, this is the first case in the literature reporting the benefits of the use of acetazolamide in the treatment of blepharochalasis.

[Use of acetazolamide in external hydrocephalus in infants].

External hydrocephalus is believed to be benign but published cases do not clearly establish this prognosis. Five children with typical external hydrocephalus were given acetazolamide. Head circumference measurements or other investigations demonstrated therapeutic effectiveness in four of the five patients. Although this series is too small to establish that acetazolamide avoided external hydrocephalus-related adverse events, the drug's outstanding tolerance and anatomic effectiveness justify widespread use in all patients with CSF resorption disorders.

Methazolamide and acetazolamide in acute mountain sickness.

Methazolamide (150 mg/d) was as effective as acetazolamide (500 mg/d) in preventing the symptoms of acute mountain sickness in 20 subjects ascending to 4985 m. PaO2 and oxygen saturation levels were similar on the two drugs but the fall in PaCO2 was greater on acetazolamide. Paraesthesiae, a side-effect of carbonic anhydrase inhibitors, tended to be less at high altitude on methazolamide and was significantly less when taking 100 mg/d at low altitude. It is likely that paraesthesiae is similar on the two drugs when given in doses that affect blood gases equally.

Relationship of pharmacokinetics to pharmacological response for acetazolamide.

Acetazolamide concentration values derived from a nonlinear model system were related to two pharmacological responses in the rabbit. Kidney response was measured by monitoring urine flow and sodium elimination. Ocular response was followed using an applanation tonometer. Maximum urine flow and sodium elimination occurring immediately after injection correlated with log dose. Urine flow dropped below control values along with a rise in osmolality, suggesting the involvement of antidiuretic hormone. Sodium elimination was correlated with plasma levels. Urine pH is thought to be involved in redu…

Acetazolamide Mitigates Opioid-Associated Sleep-Disordered Breathing.

Rationale: Chronic prescription opioid use can exert ventilatory depression and increase loop gain to induce sleep disordered breathing (SDB). Alternative therapies to mitigate opioid-associated SDB are needed. Objective: We sought to examine whether oral acetazolamide (ACZ) will reduce SDB in opioid-associated SDB by reductions in plant and/or by increasing cerebrovascular responsiveness to CO2 (CCR) Methods: Nine adults with opioid-associated SDB were randomized to receive oral ACZ or placebo, in crossover fashion, for 6 days, with multiple measurements of awake CCR to hypercapnia on day 4, …
