Metabolic acidosis and alkalosis.

The factors controlling renal bicarbonate reabsorption and acid excretion under normal conditions and in the presence of metabolic acidosis and alkalosis are reviewed. The methods used to assess distal acidification and its limitations are also discussed. Measurement of urinary pCO2 in maximally alkaline urine (pH greater than 7.8) is a very useful qualitative method to assess distal acidification. The finding of a low urinary pCO2 in maximally alkaline urine indicates a distal acidification defect. We propose that both the secretory and gradient defect types of distal renal tubular acidosis a…

Linezolid-induced lactic acidosis.

Linezolid is a commonly prescribed antibiotic in clinical practice. Although thrombocytopenia and peripheral neuropathy are frequently encountered following prolonged administration of linezolid, lactic acidosis is a rare adverse drug reaction. We present the case of a patient on linezolid for disseminated multidrug-resistant tuberculosis who presented with vomiting, dyspnoea, hypotension and high anion gap metabolic acidosis. The initial presentation mimicked sepsis syndrome. Ketoacidosis and renal dysfunction were ruled out. There was no history of ingestion of toxins/toxic alcohols. Sepsis …

Understanding renal tubular acidosis.

Renal tubular acidosis is a group of disorders characterised by metabolic acidosis, hyperchloraemia, normal anion gap, and potassium imbalance. Genetic mutations, drugs or acquired disorders disrupt the function of various transport proteins and enzymes in the renal tubules, causing diminished bicarbonate reabsorption or inability to excrete hydrogen ions, leading to proximal (type 2) and distal (type 1) renal tubular acidosis, respectively. These conditions are typically associated with hypokalaemia, which, if severe, can cause muscle paralysis and dangerous cardiac arrhythmias. A rare mixed …

[Metformin-associated lactic acidosis].

BACKGROUND: Metformin accumulation is associated with lactic acidosis and haemodynamic instability. CASE PRESENTATION: A woman in her seventies with diabetes, renal failure and hypertension presented unresponsive with severe acidosis, lactataemia, bradycardia and hypotension. After the initial survey, hypotension and bradycardia were noted before she went into cardiac arrest. After resuscitation and intubation, she was moved to the intensive care unit for dialysis and supportive care. After seven hours of dialysis, her hypotension persisted despite treatment with high levels of aminopressors.…

[Metformin -associated lactic acidosis].

Lactic acidosis is a feared complication of metformin therapy. In our article we describe 2 case reports of patients treated with metformin, who developed this complication. In the first case, which was fatal, cummulation of lactate was a result of acute kidney failure caused by diarrhea. In the second patient, lactic acidosis developed in the terrain of preexisting chronic kidney disease, when dyspepsia and decreased fluid intake caused progression into acute kidney failure. In this case, treatment of lactic acidosis was successful. Death of the first patient was probably caused by the presen…

Diet-induced metabolic acidosis.

The modern Western-type diet is deficient in fruits and vegetables and contains excessive animal products, generating the accumulation of non-metabolizable anions and a lifespan state of overlooked metabolic acidosis, whose magnitude increases progressively with aging due to the physiological decline in kidney function. In response to this state of diet-derived metabolic acidosis, the kidney implements compensating mechanisms aimed to restore the acid-base balance, such as the removal of the non-metabolizable anions, the conservation of citrate, and the enhancement of kidney ammoniagenesis and…

Familial renal tubular acidosis.

The kidney maintains systemic acid-base homeostasis through proximal tubular reclamation of filtered bicarbonate, and excretion of the daily mineral acid load by collecting duct type A intercalated cells. Impairment of either process produces renal tubular acidosis (RTA). This article will provide an overview of familial forms of proximal and distal renal tubular acidosis (pRTA and dRTA). Recessive pRTA with ocular and central nervous system abnormalities is caused by loss-of-function mutations in basolateral membrane Na-HCO3- cotransporter NBCe1/ SLC4A4. Recessive dRTA with deafness is caused…

Hyperchloraemic acidosis: another misnomer?

OBJECTIVE: To review the term hyperchloraemic acidosis. DATA SOURCES: Articles and reviews from peer reviewed journals on acid-base physiology. SUMMARY OF REVIEW: The concept of hyperchloraemic acidosis is well established in medicine and regularly taught to medical students. Unfortunately, it is yet another medical misnomer. Hyperchloraemic acidosis is only likely to exist with normal plasma sodium concentrations. This is because the acidosis is due to a decreased strong-ion-difference rather than the hyperchloraemia alone. If hyponatraemia is present, an identical acidosis can exist withou…
