An Ocular Exposure Prediction for Topical Atropine in Human Using Physiologically Based Pharmacokinetic Modeling.

Developing a mathematical model to predict the distribution and bioavailability of atropine in human eyes is an insight approach for clinical practice. This study aims to develop a human ocular physiologically based pharmacokinetic (PBPK) modeling for the ophthalmic drug atropine and explore possible mechanisms by which atropine reduces myopia progression in children. The Ocular Compartment Absorption and Transit (OCAT™) model was employed to describe the ocular distribution of atropine following administration at different dosages in both rabbits and humans. The PBPK model enables the extrapo

[Bradyarrhythmia and topical timolol : highlighting the need for medication reconciliation].

Ophthalmic beta-blockers such as timolol can exert clinically significant systemic effects due to high transnasal and conjunctival absorption. We describe a 94-year-old woman who developed symptomatic bradyarrhythmia, including sinus pauses and atrial fibrillation with slow ventricular response, attributed to topical timolol. Symptoms resolved following discontinuation and substitution with non-beta-blocker therapy. This case underscores the importance of comprehensive medication reconciliation, explicitly addressing non-oral routes of administration. In elderly, polymorbid patients, unrecogni

Design and Development of Ophthalmic Liposomes from the QbD Perspective.

Due to significant lachrymation, drug washing out, and poor adhesion to the lipophilic outer layer of the precorneal and cornea membrane, topical ophthalmic solution drops have poor ocular bioavailability. The rate of transcorneal absorption is impacted in the case of hydrophilic drug molecules as brimonidine tartrate, timolol maleate, cyclosporine, etc. Ophthalmic solution administered in many doses is less patient-compliant. The limitation of multiple-dose and its negative effects can be overcome by the development of delayed- release liposomes. Liposomes are regulatory-approved novel drug d

Ophthalmic nano-bioconjugates: critical challenges and technological advances.

Ophthalmic disease can cause permanent loss of vision and blindness. Easy-to-administer topical and systemic treatments are preferred for treating sight-threatening disorders. Typical ocular anatomy makes topical and systemic ophthalmic drug delivery challenging. Various novel nano-drug delivery approaches are developed to attain the desired bioavailability in the eye by increasing residence time and improved permeability across the cornea. The review focuses on novel methods that are biocompatible, safe and highly therapeutic. Novelty in nanocarrier design and modification can overcome their

Research progress of ophthalmic preparations of immunosuppressants.

Immune ophthalmopathy is a collection of autoimmune eye diseases. Immunosuppressants are drugs that can inhibit the body's immune response. Considering drug side effects such as hepatorenal toxicity and the unique structure of the eye, incorporating immunosuppressants into ophthalmic nanodrug delivery systems, such as microparticles, nanoparticles, liposomes, micelles, implants, and in situ gels, has the advantages of improving solubility, increasing bioavailability, high eye-target specificity, and reducing side effects. This study reviews recent research and applications of this aspect to pr

Drug Delivery for Ocular Allergy: Current Formulation Design Strategies and Future Perspectives.

The incidences of ocular allergy have been growing with the increase in pollution. Because of challenges in new drug development, there have been efforts to maximize the efficacy of existing drugs through drug delivery approaches. The effectiveness of drugs in ophthalmic conditions is primarily determined by permeability across the barrier, corneal retention, and sustained release. Thus, there have been widespread efforts to optimize these parameters to enhance efficacy through novel formulations. This review aims to analyze the approaches to drug delivery systems to encourage further research

Use of Topical Steroids in Conjunctivitis: A Review of the Evidence.

PURPOSE: Conjunctivitis, or inflammation of the conjunctiva, is a common condition that can be caused by infectious (eg, bacterial or viral infections) and noninfectious (eg, allergy) etiologies. Treatment involves diagnosis of the underlying cause and use of appropriate therapies. A broad-spectrum therapy that can address multiple etiologies, and also the accompanying inflammation, would be very useful. In this review, we discuss the usefulness of topical ophthalmic corticosteroids and ophthalmic formulations that combine corticosteroids with anti-infectives/antibiotics for treating acute inf

An eye on metformin: Applications and drug delivery strategies for ocular conditions.

Metformin, a widely recognized anti-hyperglycaemic agent, shows pleiotropic effects across various disease states. Recent research has highlighted its potential therapeutic applications in ocular conditions, including inflammation, oxidative stress, burns, conjunctivitis, and neovascularization. Although the high-water solubility of metformin is considered advantageous, it may pose challenges for conventional eye drops due to the potential for rapid solubilization in aqueous-based lacrimal fluid and removal via tear turnover. To address this limitation, innovative ocular delivery systems, such

Liposomal Drug Delivery in Ocular Therapy: Strategies for Enhancing Corneal Penetration and Bioavailability.

Vision impairment affects approximately 2.2 billion people worldwide, with glaucoma, age-related macular degeneration, fungal keratitis, and diabetic retinopathy among the leading causes of preventable blindness. Effective pharmacotherapy remains severely constrained by the eye's multilayered barrier architecture. Tear film, the corneal epithelium, the blood-aqueous barrier, and the blood-retinal barrier collectively restrict conventional topical drug bioavailability to less than 5% of the administered dose. Liposomal drug delivery systems have emerged as a clinically translatable platform cap
