Dementia with Lewy Bodies with Pure Agraphia for Kanji (Japanese Morphograms).

A 70-year-old dextral woman was admitted to a hospital with agraphia for kanji (Japanese morphograms). She had a history of severe constipation, nightmares, and visual hallucinations. Neurological examinations revealed no obvious Parkinson's disease symptoms. She showed poor skills in writing the kanji for looking at picture objects, [e.g., writing the Japanese word "inu" (which means dog) when she saw a drawing of a dog] or dictated words. A reduced striatal uptake of [123I]-2β-carbomethoxy-3β-(4-iodophenyl)-N-(3-fluoropropyl) nortropane (FP-CIT) on single-photon-emission computed tomography …

Alexia without Agraphia-report of Five Cases and Review of Literature.

Alexia without agraphia (also called pure alexia or word blindness) was the first of the disconnection syndromes to be described. It results from the loss of visual input to the language area without involvement of the language area. The most common cause is occlusion of the left posterior cerebral artery with involvement of left occipital cortex and the splenium of corpus callosum. However, it can also be caused by any lesion affecting the splenium of corpus callosum disrupting the white matter tracts from the left visual cortex to the angular gyrus. We hereby describe five cases of alexia wi…

[A case of pure agraphia due to left parietal lobe infarction].

We reported a case of a 63-year-old right handed man with pure agraphia due to the left parietal lobe infarction. The characteristics of agraphia in the patient were as follows. 1) The written letters were generally recognizable and well formed. 2) He succeeded in pointing to single Kana letter named by the examiner from the Japanese syllabary, but missed in pointing to Kana words. 3) Further, it took more time for the patient to point to even single Kana letter than for the control. 4) Most errors in Kana writing was substitution. Errors in Kanji writing are partial lacking or no response. Bu…

Frontal allographic agraphia in a patient with behavioral variant frontotemporal dementia.

We report a patient with behavioral variant frontotemporal dementia who developed agraphia, irritability, perseverative and stereotyped behavior, and dietary changes. MRI revealed bilateral frontal convexity atrophy. Neuropsychological examination showed fluent aphasia with perseverative allographic agraphia, mild semantic impairment, and dysexecutive syndrome. Allographic agraphia featured unidirectional conversion from hiragana (cursive form of Japanese phonograms) and kanji (Japanese morphograms) to katakana (square form of Japanese phonograms), as opposed to mutual (bidirectional) conversi…

Kanji (Morphogram) and Kana (Phonogram) Problem in Japanese Alexia and Agraphia.

The kanji and kana (or kanji vs. kana) problem in the Japanese language denotes the dissociation between kanji (morphograms) and kana (phonograms) in reading/comprehension and writing. Since paragraphia of kana in a patient with amyotrophic lateral sclerosis was first reported in 1893, kanji-kana dissociation has been the central topic in Japanese aphasiology. Recent advancements in lesion-to-symptom analyses and functional imaging studies have identified some areas whose damage causes dissociative disturbances of reading or writing between kanji and kana. That is, (1) angular alexia with agra…

We reported a case of an 86-year old woman with pure agraphia due to the left parietal lobe infarction. The characteristics of agraphia were as follows. Most errors in Kana and Kanji writing to dictation and copying were no response. She was able to write only numerals from 1 to 12 precisely. Most errors in numerals were substitution. One unrecognizable numeral was found. She succeeded in pointing to nine among ten single Kana letter named by the examiner with the systematic table of the Japanese syllabary, but missed in pointing to Kana words. It took more time for the patient to point to sin…

[Alexia without agraphia in the Arabic language. Neurolinguistic and and MRI study].

A 33 year-old woman developed an alexia without agraphia, a color anomia, a right hemianopia, an aphasic amnesia and a verbal amnesia. The brain MRI showed the lesions in the left splenium of corpus callosum, forceps major, optic radiations and anterieur temporal lobe. The fact that she measured writing comprehension and had complete recovery of reading impairment despite the persistence of anatomic lesions plead in favour of an active participation of the right hemisphere (RH) on reading; this capacity of the RH may be due to the linguistic particularities of arabic writing.

Brain MRI Lesions in Alexia Without Agraphia: A Case-Control Study.

BACKGROUND: Alexia without agraphia (AWA) is an acquired reading disturbance associated with left posterior cerebral artery (PCA) infarction. Based on autopsy and neuroimaging, there are two explanations for its pathogenesis: a visual cortex-language cortex disconnection and a visual word-form agnosia. Our goal was to discover if more refined brain imaging in a case-control study would provide further imaging support for either of these hypotheses. METHODS: A neuroradiologist masked to the presence of AWA reviewed diffusion-weighted and fluid-attenuated inversion recovery brain MRIs in patien…

[Visuo-constructional disorders and alexia-agraphia associated with posterior cortical atrophy].

A 57 year-old woman developed a slowly progressive environmental agnosia and dressing apraxia without disturbances of language, memory, orientation and social activities. Two years later, alexia, agraphia, visual agnosia, constructional apraxia, simultagnosia and imitation apraxia of nonsymbolic gestures were also noted. Ophthalmic examination demonstrated a left inferior quadranopsia. Oral comprehension was normal. There was no loss of insight, and behavioral response was appropriately concerned. Computed tomography and magnetic resonance imaging revealed bilateral cortical atrophy in parieto…

[Right neglect with hemiasomatognosia, mental confusion, apraxia and agraphia without aphasia].

A 74 year-old woman was admitted with a right hemiplegia resulting from a left infarct that had totally destroyed the territory supplied by the middle and anterior cerebral arteries. The patient presented with anosognosia and hemiasomatognosia and negligence of the right half-field. She was aprosodic, not aphasic, and there was a severe apraxia and total agraphia. These neuropsychological disorders are discussed in relation to the contradictory data relative to the manual lateralization of the patient. Findings in this case of a "crossed apraxia" show that management of language and of the pro…

Alexia without agraphia: lateral and medial infarction of left occipital lobe.

Four cases of alexia without agraphia were studied by 99m-technetium pertechnetate brain scan. Two types of increased uptake were seen. One was of the triangular type near the midline in the posterior view, said to characterize an infarction in the distribution area of the posterior cerebral artery. This was associated with the well-known clinical picture of alexia without agraphia accompanied by hemianopia and color-naming defect, and was confirmed by postmortem examination. The other type of pathologic isotope uptake, observed in three patients, was more laterally placed, in the basal part o…

Frontal pure agraphia for kanji or kana: dissociation between morphology and phonology.

We present two patients with frontal pure agraphia more impaired for either kanji or kana (two separate writing systems for the Japanese language). The lesion of patient 1 (preferentially disturbed for kanji) was restricted to the foot of the middle frontal gyrus and the adjacent anterior precentral gyrus, whereas the lesion of patient 2 (preferentially disturbed for kana) included the posterior two thirds of the middle frontal gyrus. Both patients made agraphic errors (impaired recall) for kanji and agraphic or paragraphic errors (changing into other symbols) for kana. The double dissociation…
