Modified Masquelet Technique and Primary Tibial Metaphyseal Shortening for the Management of Proximal Tibial-infected Non-union in a Patient with Alcohol-induced Neuropathy: A Case Study.

AIM: To highlight the role of the Masquelet technique as a limb salvage procedure for a neuropathic patient presenting with infected non-union of proximal tibia. BACKGROUND: The management of an infected non-union in neuropathic patients is most challenging; with various treatment options available, the prognosis is often guarded. CASE DESCRIPTION: A 37-year-old male with chronic polyneuropathy, also possessing a contralateral midfoot Charcot arthropathy secondary to a history of alcohol abuse, developed infection after a proximal tibial osteotomy for a preceding mal-non-union of a proximal

Characteristics of alcoholic neuropathy have been obscured by difficulty in isolating them from features of thiamine-deficiency neuropathy. We assessed 64 patients with alcoholic neuropathy including subgroups without and with coexisting thiamine deficiency. Thirty-two patients with nonal …

Not Always as It Seems: A Case of Ascending Paralysis.

Although generally presenting as a chronic, progressive peripheral neuropathy, we present a case of acute alcoholic neuropathy initially mistaken for acute Guillain-Barré syndrome. Electrodiagnostic evaluation of alcoholic neuropathy reveals an axonal neuropathy and may be complicated by demyelination if comorbid nutritional deficiencies are present. This is in contrast to the significant demyelination classically associated with Guillain-Barré syndrome. F waves, which are absent or prolonged in Guillain-Barré syndrome, are unaffected in alcoholic neuropathy. Further evaluation with lumbar pun…

A clinicopathologic study of acrodystrophic neuropathies.

Sixteen patients presenting with trophic changes associated with a peripheral neuropathy were investigated. Muscle power was normal in all p … ensory fibers is the most plausible explanation for neurological and pathological abnormalities. In alcoholic neuropathy with trophic changes, loss of sensory fibers is more important than in alcoholic neuropathy without trophic changes. In familial and sporadic cases, axonal loss is more severe and unmyelinated fibers are more severely affected than in alcoholic acro …

[Alcoholic polyneuropathy and myopathy--a contribution to clinico-neurologic diagnosis of alcoholic patients].

Neurological lesions due to alcohol most frequently affect the peripheral nervous system. Alcoholic myopathy or the combination of alcoholic neuropathy and myopathy are less frequent. The features of these disorders are discussed in detail. To support the clinical diagnosis of alcoholic neuropathy and/or myopathy various criterions corresponding to their clinical relevance are set up. It is suggested that a clear diagnosis ordinarily points to alcohol endangering or even to alcohol addiction and it therefore is incompatible with administering a driving license.

Reappraisal of alcoholic myopathy. Clinical and biopsy study on chronic alcoholics without muscle weakness or wasting.

To determine if alcoholic neuropathy which causes denervation of the distal muscles of chronic alcoholics also produces a subclinical myopathy of their proximal muscles, we studied 11 chronic alcoholics who had no muscular weakness or wasting. Six patients demonstrated distal hyporeflexic (ankle jerks) sensory neuropathy on clinical examination. Four patients, one of whom was asymptomatic, had slow peroneal motor nerve conduction velocities. Patterns of neuropathy were present in the electromyograms of the proximal muscles of two patients. Muscle biopsy studies with enzyme histochemistry indic…

[Alcoholic polyneuropathy. Electrophysiological and clinical findings in 85 patients (author's transl)].

Maximum motor and sensory nerve conduction velocities, amplitudes of muscle action potentials (surface electrodes) and of sensory nerve action potentials, and needle electromyograms were studied in 80 patients with alcoholic neuropathy and in 5 chronic alcoholics without clinical signs of neuropathy. The electrophysiological results were compared to the clinical findings. Neurographic criteria compatible with the diagnosis "alcoholic neuropathy" were defined. Neurographic findings contrary to a diagnosis of alcoholic etiology were demonstrated in 3 patients. Only in a smaller group of patients…

[Increased serum IgA levels in alcoholic polyneuropathy. Pathogenetic discussion].

Increased serum IgA in patients with direct toxic alcoholic peripheral neuropathy was studied prospectively in 33 patients with an obvious clinical polyneuropathy confirmed by electrophysiological investigations. The average concentration of IgA in this group (450 mg/100 ml) was significantly higher than in a group of 24 matched alcoholic control patients without peripheral neuropathy (306 mg/100 ml), p less than 0.0001. The authors discuss possible direct and indirect modifications of the humoral immune response due to the peripheral neuropathy.

Chronic alcoholism and esophageal motor activity: a 24-h ambulatory manometry study.

BACKGROUND: Stationary esophageal manometry has shown esophageal motor abnormalities in patients with chronic alcoholism. The abnormalities identified in different studies are not consistent. Twenty-four hour ambulatory esophageal manometry enables monitoring of esophageal motor activity under a variety of physiological conditions and gives a more complete assessment. METHOD: Twenty-four hour ambulatory esophageal manometry and pH-metry were performed using a combined pH and pressure catheter. Subjects with chronic alcoholism with no other illness and not in withdrawal were studied with age-

Alcohol-induced neuroarthropathy in the foot: a case series and review of literature.

Charcot arthropathy, also known as neuroarthropathy, is most commonly associated with diabetes mellitus, despite a variety of other etiologies. A limited number of case reports have been published on neuroarthropathies caused by other forms of neuropathy, including alcoholic peripheral neuropathy. We report 4 cases of neuroarthropathy associated with chronic alcoholism in nondiabetic individuals. Conservative management similar to that afforded diabetic patients was successfully employed in these cases. A review of the clinical presentation and the pathology of alcoholic neuropathy is included…

[Acute and reversible myoclonic encephalopathy, extrapyramidal syndrome, polyneuropathy caused by chronic disulfiram poisoning].

This 44-year woman was admitted for weight loss and global intellectual slowing. She had mild chronic alcoholic neuropathy. She was discontinued alcoholic consumption for 6 months and was given disulfiram (1.5 g/day) since then. She developed over a 5-day period acute neuropathy, confusion and extrapyramidal symptoms with oculo-cephalogyric and dystonic movements and myoclonus. Electromyography revealed a severe polyneuropathy. After disulfiram withdrawal, confusion and extrapyramidal symptoms disappeared within a few days, but sensitivo-motor deficit improved more slowly. Nerve biopsy was sug…

Disulfiram neuropathy: two case reports.

BACKGROUND: Neuropathy is a rare adverse side effect of disulfiram therapy and is under-recognized. There have been few case reports documen … rs in persons with a history of chronic alcohol use. It is under-recognized and often attributed to alcoholic neuropathy given its comorbidity with alcoholic neuropathy. A greater understanding of this side effect may reduce neurologic complications related to disulfiram neuropathy and aid in early withdrawal of this offending agent.

Peripheral systems: neuropathy.

Long-term, excessive consumption of alcoholic beverages produces a peripheral neuropathy with symptoms of decreased superficial sensation, hyperalgesia, and weakness. Alcoholic neuropathy is characterized by axonal degeneration with reduced density of both small and large fibers and axonal sprouting. Electrophysiologic studies reveal a marked reduction in the amplitude of sensory potentials and moderate slowing of nerve conduction, mainly in the lower extremities. Dietary deficiency of vitamins, which are often associated with chronic alcoholism, can contribute to the pathogenesis. Recent stud…

Alcohol amplifies cingulate cortex signaling and facilitates immobilization-induced hyperalgesia in female rats.

Complex Regional Pain Syndrome (CRPS) is a musculoskeletal pain condition that often develops after limb injury and/or immobilization. Although the exact mechanisms underlying CRPS are unknown, the syndrome is associated with central and autonomic nervous system dysregulation and peripheral hyperalgesia symptoms. These symptoms also manifest in alcoholic neuropathy, suggesting that the two conditions may be pathophysiologically accretive. Interestingly, people assigned female at birth (AFAB) appear to be more sensitive to both CRPS and alcoholic neuropathy. To better understand the biobehavior…

F-wave conduction velocity in alcoholic polyneuropathy.

We used the F-wave conduction velocities to estimate the motor nerve conduction along the proximal segment (spinal cord to knee) of the axon of tibial and common peroneal nerves in ten patients with alcoholic polyneuropathy and compared these to the motor nerve conduction velocities in ten normal subjects. All the studies so obtained showed a normal conduction in the proximal segments even though in three patients the distal nerve conduction values (knee to ankle) revealed moderate to severe neuropathy. These data support the hypothesis that in alcoholic polyneuropathy, motor conduction in pro

[Autonomic disorders in polyneuropathies].

BACKGROUND: Many polyneuropathies manifest autonomic disturbances. Diabetic neuropathy, the most frequent neuropathy in the western world, serves as model of the symptomatology of autonomic disturbances. DIABETIC NEUROPATHY: Clinical symptoms comprise pupillary and cardiovascular dysfunction such as orthostatic hypotonia and syncopes, thermoregulatory, gastrointestinal symptoms, disturbances in urogenital and respiratory function and unawareness of hypoglycemia. OTHER NEUROPATHIES: This article also describes autonomic symptoms in alcoholic neuropathy, in Guillain-Barré syndrome, in paraneop…

Autonomic dysfunction in patients with non-alcoholic chronic liver disease.

AIMS: To assess the presence of autonomic neuropathy in patients with non-alcoholic chronic liver disease and its relationships with the severity of liver damage. METHODS: Thirty non-alcoholic patients with chronic liver disease and 26 healthy control subjects were studied. The silicone imprint technique was used to quantify the number of functioning sweat glands in order to assess peripheral sympathetic dysfunction. Heart rate variations in response to deep breathing at 6 per minute (deltaR6), to a Valsalva maneuver, and with orthostatism (RRmax/RRmin) were determined to assess parasympathet

[Cardiovascular parasympathetic and orthosympathetic lesions in alcoholism and their association with peripheral polyneuropathy].

Our aim was to identify the damaged baroreceptor reflex in alcoholic patients, and its association to peripheric neuropathy, and the electrophysiological data of primary axon damage and secondary myelinic damage. We measured the R-R interval and the blood pressure resting, after valsalva maneuver, six deep breathing/min and decubitus-biped, changing position, in 22 alcoholic patients and 22 healthy persons. Electromyography was performed, measuring the nerve conduction velocity and peripheral latency time of sciatic-popliteal external and internal nerves. We observed important damage of the ca
