Treating substance-use disorders among physicians.

Of 40 participants of a two-year program for rehabilitating alcohol- and other-drug-dependent physicians, 15 have completed 24 months of treatment and monitoring, eight have completed 12 to 23 months, ten have completed one to 11 months, and seven discontinued treatment before completion. Of the 33 treated physicians, 31 have returned to full practice and 22 have experienced no relapse. The prognosis is quite favorable for most impaired physicians in appropriate treatment, monitoring, and follow-up are provided.

Alcoholism treatment and evaluation: in search of methods. II. Collation of two-year follow-up studies.

This study collated 23 articles reporting outcome of alcoholism treatment efforts over a 2-year follow-up period. Broad conclusions reached by Costello (1975) in a similar collation of studies reporting on a 1-year follow-up interval found general support. Similar consistencies were noted across studies within groups described as having good outcome profiles as was noted by the previous collation. Good outcome results from matching relatively better prognostic cases wtih programs with broad treatment resources. Multiple baselines were offered as guidelines for program evaluation. Baselines wer

The ultra mentality - Foundations for maximising athletic mental performance.

For over 100 years there has been an academic pursuit of understanding and applying the principals of sport psychology to improve athletic p … bination of all of these characteristics. Without the right mentality everything can fall apart. In Alcoholics Anonymous, it is said, "you don't have a drinking problem - you have a thinking problem." The evidence is quite clear that the mental game can have a substantial impact, both for the better and for the worse, on performance. In this perspectives and viewpoint …

Long-term recovery in alcoholics.

A cross-sectional study of alcoholics who varied widely in duration of abstinence was combined with a four-year follow-up study of the same subjects. Together, the data demonstrated a course of recovery in abstinent alcoholics measured in three ways: severity of symptoms, probability of relapse, and work history. Additionally, the subjects showed high rates of smoking cessation. By every measure, the course of recovery seemed essentially the same in men and women. The recovery process was most rapid in the early years of abstinence but continued for 10 or more years. It was suggested that keys

Sober living houses: potential roles in substance abuse services and suggestions for research.

'Sober living houses' are alcohol- and drug-free residences for individuals attempting to establish or maintain sobriety. They offer no formal treatment services but do provide social support and an abstinent living environment. 'Sober living houses' have been used as aftercare placements for clients completing residential treatment, places for clients to live while attending outpatient treatment, or as stand-alone approaches for substance misuse problems. This article identifies areas of research needing attention and suggests that 'sober living houses' have the potential to play a stronger r

The scale for treatment integration of the dually diagnosed (STIDD): an instrument for assessing intervention strategies in the pharmacotherapy of mentally ill substance abusers.

The authors developed a series of 15 multiple-choice vignettes which pose common dilemmas in the pharmacotherapy of patients suffering from psychoactive substance use disorders coexisting with other mental disorders. Fourteen judges standardized the treatment options along a 'treatment integration continuum'. The authors surveyed 112 psychiatrists with the vignettes and 3 subscales of the Substance Abuse Attitude Survey. Ten of the 15 vignettes were taken to form the normally distributed Scale for Treatment Integration of the Dually Diagnosed (STIDD), which is described in terms of its reliabi

Questionnaire for risk factors of alcoholic and nonalcoholic fatty liver disease.

OBJECTIVES: Non-communicable diseases are estimated to account for 90 % of total deaths and 19 % of premature deaths in Slovakia. Major preventable risk factors of premature mortality are overweight, obesity and alcohol consumption. BACKGROUND: Screening of risk factors related to alcoholic and nonalcoholic fatty liver diseases (AFLD and NAFLD, respectively) in Slovak outpatients with liver disease. METHODS: A total group of 923 patients, aged 19-91 years were included in the study. Self-administered anonymous questionnaires (Q) were filled in by them. Twelve questions were included relating

Current strategies for the treatment of alcohol dependence in the United States.

Substantial progress has been reported in the treatment of patients with alcoholism in the United States. Studies that seek to identify the most appropriate form of therapy for alcohol-dependent patients have been an important part of this effort. Recognition that psychotherapy alone cannot help all patients who have alcoholism has led to interest in the use of pharmacotherapy. Recent research demonstrates that pharmacotherapy with the opioid receptor antagonists naltrexone and nalmefene helps prevent relapse in many alcohol-dependent patients.

Seventy-five alcohol-dependent patients (35 males, 40 females) treated by a Minnesota Model in-patient programme were followed up for 1 year … inent were 53% and 39%, respectively. Good outcome for both sexes was associated with attendance of Alcoholics Anonymous meetings. Poor outcome at 1 year was associated with a lack of GP involvement in aftercare and failure to provide alcohol counselling in the community. The BRENDA model: integrating psychosocial treatment and pharmacotherapy for the treatment of alcohol use disorders.

While the U.S. Food and Drug Administration has approved several medications for the treatment of alcohol-related problems, their use has not gained wide acceptance in the United States. Typically, patients with alcohol use disorders are only referred to psychosocial support (e.g., Alcoholics Anonymous). However, the use of pharmacotherapy may complement psychosocial treatments, as evidence shows that pharmacotherapy can improve treatment outcomes. The effectiveness of pharmacotherapy depends on patient compliance with taking the medication and the context in which the medication is administer…

Native Americans and alcohol: past, present, and future.

Native Americans have higher rates of alcohol use, frequency of use, and increased rates of fetal alcohol syndrome, compared with other ethnic groups (J. Hisnanick, 1992; P. A. May, 1996; J. M. Wallace et al., 2003). High prevalence rates of alcohol misuse among Native Americans must be understood in light of their unique history, which has resulted in trauma and exposure to many risk factors for problem alcohol use. Many risk factors have been identified in the general population; however, only some of these risk factors have been examined among Native American populations. The unique history
