Sagittal plane biomechanical and EMG analyses from eight below knee (B/K) amputee trials demonstrate considerably modified motor patterns from the residual muscles at the hip and knee. Five SACH fittings, two Uniaxial and one Gressinger prostheses were analysed. Moments of force and mechanical power were analysed on all eight trials and EMG profiles are reported for three of the amputees fitted with SACH prostheses. The findings can be summarized as follows: 1. All eight trials had similar internal moment of force patterns at the ankle. A dorsiflexor moment commenced at heel contact and contin…

The running ability of ten unilateral below-knee amputees was examined by analyzing the temporal and length characteristics of their running strides and the angular displacement patterns of their intact and prosthetic limbs. Six of the individuals tested were able to run and in several cases their locomotor patterns were similar to those of non-amputees. The running gait of three of the amputee runners, however, did contain one particular undesirable element namely an excessively straight residual knee during at least a portion of support on the prosthesis. Although several explanations could …

Osseointegrated transradial prostheses have the potential to preserve the natural range of wrist rotation, which improves the performance of … to 71, MMDT from 258s to 231s. Natural forearm rotation can be successfully restored in transradial amputees by using osseointegration and our novel mechanical attachment to the hand prosthesis.

Mutilated hands at the distal level may pose a challenge for reconstruction. Biological treatment options may require multiple surgical interventions and a long rehabilitation course with little hope of good functional outcome. Standard hand prostheses are also not an ideal solution, as they are too long and cumbersome for partial hand injuries. This paper outlines the functional outcomes of prosthetic reconstruction with devices customized for the transcarpal amputation levels. The functional outcome was evaluated with the Action Research Arm Test (ARAT), Southampton Hand Assessment Procedure

The human motor system undergoes reorganization after amputation, but the site of motor reorganization and the mechanisms involved are unknown. We studied the site and mechanisms of motor reorganization in 16 subjects with traumatic lower-limb amputation. Stimulation at different levels in the CNS was used to determine the site of reorganization. The mechanisms involved were evaluated by measuring the thresholds for transcranial magnetic stimulation (TMS) and by testing intracortical inhibition and facilitation. With TMS, the threshold for muscle activation on the amputated side was lower than

The clinical characteristics of 234 amputees admitted to the National Taiwan University Hospital during a 10-year period were analyzed. Diabetes mellitus accounted for 37.2% (87 cases) of the amputations and was the most commonly associated disease. Malignancy was the second cause of amputation (16.2%). Traffic accidents and atherosclerosis (without diabetes mellitus) each accounted for about 13% of the amputations. Below-knee amputation was the most commonly performed procedure among the diabetics, while above-knee, below-knee and ray amputations were roughly equally performed among non-diabe…

The functional outcome of rehabilitated bilateral lower limb amputees was studied. The study included 31 amputees who were admitted during 1980-1990 to a rehabilitation centre in the north of the Netherlands. The clinical notes made during the patients' admission were studied to obtain information about their characteristics, while mobility and prosthetic use were studied at discharge. The patients who were alive and willing to participate in the study were interviewed by a physician at their residence in November 1992, using among other things, the Sickness Impact Profile (SIP) and the Life S…

In running, large gait asymmetry is expected due to the inability of the foot prosthesis to comply with the kinematic demands and produce a powerful plantarflexion moment. In this work, interlimb asymmetry in below-knee (BK) amputee running gait was assessed for one rigid and three flexible keel prostheses, using vertical and anteroposterior ground reaction forces and respective impulses. Nine BK amputees and 6 controls participated in this study. The running speed was monitored by two light sensitive detectors while the ground reaction forces were measured with a Kistler force plate. Between …

Above-knee amputees were studied while wearing a prosthesis containing either a standard or a modified single axis foot. The modified foot allowed 12-15 degrees of dorsiflexion and reduced stiffness in the area of the metatarsal-phalangeal (MTP) joint. The amputees were allowed to vary the stiffness at the two joints to obtain what "felt best." The amputees agreed that in comparison to the standard single-axis foot, the modified foot: a) offered a more comfortable gait while walking on level ground; b) made walking up inclines easier; c) affected knee stability while walking on declines; and d…

Reamputation above the knee after failure of primary below-knee amputation was related to the distal blood pressure. With pressures of 20, 30 and 40 mmHg, two-thirds, one-third and one-seventh, respectively, of the below-knee amputations required reamputation above the knee.

Amputees face many challenges associated with residual limbs. Overall, amputees have an increased risk for skin disease occurring at residual limb sites. Although prosthetists and primary care physicians often accomplish routine care, the dermatologist plays a very important role within the multidisciplinary team. Many military dermatologists have unique clinical experience treating amputees, as a portion of their practice consists of providing care to soldiers with traumatic amputations from complex and dramatic blast injuries. Although current therapies and preventative treatments are for th…

Bone overgrowth of the residual limb after an amputation is a well documented complication in the pediatric amputee population. Bone overgrowth can cause pain, problems with skin breakdown, and poor prosthetic fit. There have been few reports of bone overgrowth in the adult amputee. Two cases of traumatic transfemoral amputations after extensive tissue damage are presented. Both patients successfully completed an in-patient amputee rehabilitation program and achieved functional ambulation with their prostheses. However, each developed distal residual limb pain within a year after their amputat
