To date, there are no population-based epidemiologic studies providing information about the prevalence of food-induced anaphylaxis. However, statistics from the United Kingdom demonstrated an increase of anaphylaxis from 5.6 cases per 100,000 hospital discharges in 1991-92 to 10.2 cases in 1994-95. The increase for the subcategory of food-induced anaphylaxis was above the overall increase in anaphylaxis. In the UK register of fatal anaphylactic reactions, all food-induced fatalities have been accompanied by respiratory problems with respiratory arrest. Atopic individuals with bronchial asthma…

BACKGROUND: Legumes are one of the most common foods causing allergic reactions in children and adults. Cross-reacting antibodies are frequently demonstrated in this family but the real clinical cross-reactivity is uncommon.
OBJECTIVE: To report a case of lupine-induced anaphylaxis and to elucidate in vivo and in vitro cross-reactivity with some legumes.
METHODS: Skin prick test (SPT) with some legumes were performed. Cap-IgE, ELISA-IgE, and immunoblotting were carried out. Open oral challenges with some legumes were performed. Cross-reactivity was studied by ELISA and immunoblotting inhibit…

The clinical features of exercise-induced anaphylaxis in 5 patients are described, with an accurate history, skin tests, IgE determinations and exercise tests. In these 5 patients we were able to distinguish three different forms of exercise-induced disease: exercise-induced anaphylaxis based upon an IgE-mediated allergy to foods, exercise-induced anaphylaxis without food allergy and cholinergic urticaria.

A 13 year old boy suffered two separate episodes of severe anaphylaxis after consuming sandwiches and a piece of bread. Prick skin testings with available food allergens only revealed a positive reaction to a 1:10 w/v of wheat flour extract. A diagnosis of wheat-induced anaphylaxis was made and a double blind food challenge was suggested however was declined by the family. The patient was instructed to avoid all wheat containing foods and to carry a kit containing an epinephrine pre-loaded syringe and an antihistamine tablet to be used in the event of inadvertant consumption with an instructio…

Streptokinase is a thrombolytic agent used most commonly for the dissolution of thrombi obstructing coronary arteries during acute myocardial infarction (MI). Anaphylactic reactions induced by streptokinase occur rarely. We report the case of a patient with acute MI who developed anaphylaxis shortly after the initiation of an intravenous infusion of streptokinase. The patient became profoundly hypotensive and developed an erythematous rash that spread rapidly to cover most of his body. He required mechanical ventilation and the administration of epinephrine for blood pressure support, which su…

Exercise-related anaphylaxis is a novel form of physical allergy that is being recognized with increasing frequency in a society with a growing commitment to health through planned exercise. The clinical manifestations progress from pruritus, erythema, and urticaria to some combination of cutaneous angioedema, gastrointestinal and laryngeal symptoms and signs of angioedema, and vascular collapse. The finding of an elevated serum histamine level during experimentally-induced attenuated attacks indicates mast cell participation, as in a physical allergy, and the signs and symptoms are characteri…

BACKGROUND: Shellfish and fish are increasingly consumed worldwide and are common triggers of food-induced anaphylaxis.
OBJECTIVE: To characterize seafood-induced anaphylaxis using data from the European Anaphylaxis Registry.
METHODS: Seafood-induced cases were selected from the registry (2007-2022) and analyzed regarding age and geographic distribution, elicitors, clinical features, and time trends.
RESULTS: We identified 462 patients, including 316 (68%) with shellfish- and 146 (32%) with fish-induced anaphylaxis. In Europe, shellfish showed geographic differences and was the main elicito…

BACKGROUND: Patients with a history of anaphylaxis are at risk of future anaphylactic reactions. Thus, secondary prevention measures are recommended for these patients to prevent or attenuate the next reaction.
METHODS: Data from the Anaphylaxis Registry were analyzed to identify secondary prevention measures offered to patients who experienced anaphylaxis. Our analysis included 7788 cases from 10 European countries and Brazil.
RESULTS: The secondary prevention measures offered varied across the elicitors. A remarkable discrepancy was observed between prevention measures offered in specializ…

Anaphylaxis is a rapidly fatal complication of anesthesia, and it needs to be appropriately diagnosed and treated by anesthesiologists. A 37-year-old man underwent surgery for pneumothorax under general anesthesia. Postoperatively, the train-of four count was four, and sugammadex was administered. Soon after, the patient's oxygen saturation and blood pressure decreased, and skin flushing was noted. We suspected anaphylaxis and administered adrenaline. Once the blood pressure normalized, we administered hydrocortisone, famotidine, and d-chlorpheniramine maleate. Thereafter, his condition stabil…

BACKGROUND: Prevention and management of allergic reactions at school are recognized public health concerns. We analyzed food-induced anaphylaxis (FIA) cases which occurred in education facilities to better understand their management.
METHODS: FIA cases recorded by the European Anaphylaxis Registry which occurred in (pre-)school settings were descriptively analyzed in a comparative manner to pediatric FIA cases that occurred outside education facilities.
RESULTS: Of the 3450 pediatric FIA cases, 332 (9.6%) occurred in education facilities (median age: 4 years [IQR: 2-9]). 151 (45.5%) childr…

Anaphylaxis, if not promptly recognised and adequately treated, can rapidly lead to death even in previously healthy individuals. At the same time, a number of myths and misconceptions persist, complicating both its diagnosis and management. It is often assumed, for example, that cutaneous manifestations such as exanthema are a defining feature of anaphylaxis and that symptoms always occur immediately following exposure to an allergen. In clinical practice, treatment is not infrequently initiated with corticosteroids and antihistamines, while adrenaline is reserved for cases of clinical deteri…

PURPOSE OF REVIEW: The rising incidence of anaphylaxis and reports of cases unresponsive to standard epinephrine treatment highlight the relevance of refractory anaphylaxis. There is a lack of standardized management protocols, and clinicians across specialties need guidance to handle this rare but life-threatening condition.
RECENT FINDINGS: There is a lack of a universally accepted definition, with most studies describing it as anaphylaxis unresponsive to several doses of intramuscular epinephrine. It emphasizes the pathophysiological complexity, including potential mechanisms like delayed …

Anaphylaxis is a generalized, severe, life-threatening reaction, mostly with an allergic origin. Triggers are usually drugs, insect bites, poisons, contrast material and food. It is caused by various mediators (histamine, prostaglandins, leukotrienes etc.) released from mast cells, basophilic granulocytes. Histamine plays a central role in its creation. Immediate recognition and specific treatment instantaneously are essential for successful treatment. In severe conditions, the clinical features are very similar, regardless of their allergic/non-allergic origin. The incidence can vary over tim…

Idiopathic anaphylaxis (AI) refers to anaphylaxis without a recognizable cause after a comprehensive allergic workup. The diagnostic approach usually includes an accurate clinical history aimed at excluding both the most and the less frequent causes of anaphylaxis and all pathologies that may resemble anaphylaxis. AI is more common in adults than in children. The epidemiology of AI has been reduced in recent years, probably to increase knowledge and discover new clinical entities, such as the α-gal anaphylaxis. Anaphylaxis results from the massive activation of the mast cells (MCs). Thus, it i…

The term anaphylaxis describes a severe, potentially life-threatening allergic reaction. It is caused by an acute, systemic immune response to substances against which in most cases a previous sensitization has taken place. An anaphylactic reaction can affect every organ system of the human body. The first signs of an allergic shock are symptoms such as hypotension, tachycardia, exanthema and dyspnea. The complete expression of anaphylactic shock can occur very quickly. A perioperative anaphylaxis, in particular, is not always easy to recognize. Therefore, it is important to know the possible …
