Naming ability in patients with mild to moderate Alzheimer's disease: what changes occur with the evolution of the disease?

OBJECTIVES: Naming deficit is a linguistic symptom that appears in the initial phase of Alzheimer's disease, but the types of naming errors … Errors were classified as verbal semantic paraphasia, verbal phonemic paraphasia, no response (pure anomia), circumlocution, unrelated verbal paraphasia, visual errors or intrusion errors. RESULTS: The patients with moderate Alzheimer's disease had significantly fewer correct answers than did both the control group and the group with mild Alzheimer's di …

Speech and lexico-semantic errors during direct cortical stimulation mapping of the language-dominant hemisphere: effects of object and action naming.

OBJECTIVE: In this retrospective study, the authors aimed to establish the stereotactically defined probability distribution for speech (i.e., anarthria, speech arrest) and lexico-semantic errors (i.e., anomia) through direct cortical stimulation (DCS) by using two tasks: action naming and object naming. They also analyzed the patterns of interindividual variability in the localization of the language sites involved, and investigated whether any patient or lesion location factors were associated with greater variability. METHODS: Eighty-one Italian-speaking patients who underwent awake surger…

[Central vision].

The clinical assessment of vision by means of optotypes does by no means test just two-point resolution, since a correct naming of the letters or digits requires a preceding visual object recognition. Cortical lesions can massively deteriorate vision up to a "Seelenblindheit" in spite of intact optics and retina. There are different processing levels involved in the analysis which can be individually defective, leading to disorders from visual indiscrimination to agnosia or anomia.

[Aphasia arising from left thalamic haemorrhage].

A patient, aged 58, with sudden loss of speech and right-sided haemiparesis is described. CT brain showed intracerebral haemorrhage in the left thalamus. The examination of speech functions established aphasia characterized by anomia, comprehension disorders, paraphasia, perseveration, reduction of spontaneous speech, dyslexia and agraphia. Relation between thalamic aphasia and cortical aphasic syndromes is discussed. The obtained results are in agreement with the theory that emphasizes a specific role of thalamus in verbal communication.

Psychological consequences of infestation of the dwelling unit.

Interview data from a three-wave panel study of the impact of the residential environment upon the psychological well-being of 337 minority women were used to examine the effects of household infestation upon self-reported depression, phobic anxiety, somatization, hostility, and anomia. The findings of cross-sectional and change analyses over the three waves of data point to a stable relationship between rat infestation and a single dimension of psychological well-being, somatization.

[Language rehabilitative training for postoperative aphasia in neurosurgery].

Rehabilitation nursing focus on assisting patients to reconstruct their health. We trained 21 cases of aphasia after operation with different types of early comprehensive speech reconstruction exercises, including hearing, speaking, reading and writing. We achieved expected results. Anomia aphasia has the best outcome, next is motor and sensory aphasia, and combined aphasia is less effective. In addition, patients with aphasia before surgery got slightly better rehabilitation than those whose aphasia was resulted from surgery.

Slowly progressive aphasia, a left temporal variant of probable Pick's disease: 15 years of follow-up.

It is known that word-finding difficulties or anomia are the first signs observed in Pick's disease. We report a 15 year follow-up of a woman who at the age of 52 suddenly developed a speech impairment and only 6 years later showed other cognitive disturbances compatible with Pick's disease. This case demonstrates that nonfluent aphasia and agrammatic aphasia may constitute the first signs of this disease, stressing the importance of prolonged follow-up in such cases.

"Semantic variant primary progressive aphasia" due to comorbidity of Lewy body disease and a previous cerebral venous infarction in the left anterior temporal lobe: A case report.

Primary progressive aphasia (PPA) is a neurological syndrome characterized by progressive language impairment. Various neurodegenerative disorders cause PPA. Dementia with Lewy bodies (DLB) is one known cause of PPA, and little is known about this association. Almost all published cases of PPA associated with DLB are the logopenic variant of PPA. Here, we describe the novel case of a patient with DLB presenting clinical features of the semantic variant PPA (svPPA). A 75-year-old woman was referred to our hospital with a 2-year history of progressive anomia and amnesia. Two months before admiss…

Behavioral disorders in association with posterior callosal and frontal cerebral infarction.

Behavioral disorders were a prominent clinical feature after the surgical treatment of an anterior communicating artery aneurysm rupture in a 44-year-old man. Callosal apraxia was associated with an alien hand. The latter remained 1 year after surgery while diagonistic apraxia disappeared after 3 months. Other callosal signs included left agraphia, tactile anomia and auditory suppression. MRI revealed posterior callosal infarction and a right frontal infarct. The association of diagonistic apraxia and alien hand is rarely reported.

Progressive aphasia without dementia--a clinical and cognitive neuropsychological analysis.

This article provides a clinical and cognitive neuropsychological analysis of a patient who presented a marked and progressive linguistic disturbance in the absence of any other significant cognitive impairment. A PET scan indicated that this disorder was associated with focal left temporofrontal hypometabolism. The essential characteristics of this disorder were anomia, surface dyslexia, and surface dysgraphia, along with a mild grammatical disturbance and deficits in both visual and auditory word comprehension. The patient's disorder is considered in relation to other similar cases and to mo…

Subcortical crossed aphasia: a case report.

A 74-year-old, right-handed woman suffered acute aphasia and left hemiplegia secondary to a cerebral infarction in the right cerebral hemisphere. The lesion was located deep in the parietal lobe and extended to the posterior limb of the internal capsule and the head of the caudate nucleus. The patient's aphasia was characterized by severe impairment in auditory and visual comprehension and auditory retention span, as well as by anomia, agraphia, and dyscalculia. She showed rapid recovery from her aphasia, with residual deficits in writing, naming, calculation, and memory.

[Alexia without agraphia in the Arabic language. Neurolinguistic and and MRI study].

A 33 year-old woman developed an alexia without agraphia, a color anomia, a right hemianopia, an aphasic amnesia and a verbal amnesia. The brain MRI showed the lesions in the left splenium of corpus callosum, forceps major, optic radiations and anterieur temporal lobe. The fact that she measured writing comprehension and had complete recovery of reading impairment despite the persistence of anatomic lesions plead in favour of an active participation of the right hemisphere (RH) on reading; this capacity of the RH may be due to the linguistic particularities of arabic writing.

Disconnection syndrome in a right-handed patient with right hemispheric speech dominance.

The callosal disconnection syndrome is characterized by unilateral (left) tactile anomia, unilateral (left) agraphia and unilateral (left) ideomotor apraxia. We report on a right-handed patient, who developed a callosal disconnection syndrome due to the rupture of an anterior communicant aneurysm. As he had right hemisphere dominance for language and praxis, the signs described above were crossed. He also featured a left alien hand sign and motor aprosody. The authors believe this is the first case of callosal disconnection in a right-hander with crossed language and praxis functions.

Picture naming: Stereoelectroencephalography and connectivity insights.

OBJECTIVE: This study investigates the neural substrates of picture naming (PN) using stereoelectroencephalographic (SEEG) stimulations and evaluates the contribution of white matter (WM) fascicles related to the basal temporal language area (BTLA) to the broader functional PN network. METHODS: In patients undergoing SEEG for drug-resistant epilepsy, stimulation sites were classified as either eloquent (inducing anomia) or noneloquent (NE). Spatial distribution was assessed along the ventral temporal cortex (VTC). Structural connectivity was analyzed using voxelwise disconnectome mapping, con…

Critical role of the ventral temporal lobe in naming.

OBJECTIVE: Lexical retrieval deficits are characteristic of a variety of different neurological disorders. However, the exact substrates responsible for this are not known. We studied a large cohort of patients undergoing surgery in the dominant temporal lobe for medically intractable epilepsy (n = 95) to localize brain regions that were associated with anomia. METHODS: We performed a multivariate voxel-based lesion-symptom mapping analysis to correlate surgical lesions within the temporal lobe with changes in naming ability. Additionally, we used a surface-based mixed-effects multilevel anal…

Apraxia of speech involves lesions of dorsal arcuate fasciculus and insula in patients with aphasia.

OBJECTIVE: To determine the contributions of apraxia of speech (AOS) and anomia to conversational dysfluency. METHODS: In this observational study of 52 patients with chronic aphasia, 47 with concomitant AOS, fluency was quantified using correct information units per minute (CIUs/min) from propositional speech tasks. Videos of patients performing conversational, how-to and picture-description tasks, word and sentence repetition, and diadochokinetic tasks were used to diagnose AOS using the Apraxia of Speech Rating Scale (ASRS). Anomia was quantified by patients' scores on the 30 even-numbered…
