Pharmacotherapy of childhood anxiety disorders.

Anxiety disorders are among the most common psychiatric disorders of childhood, yet limited data is available regarding the use of psychotropic medications for these conditions. Until recently, much of the data on the pharmacologic treatment of pediatric anxiety disorders has consisted of case reports and small open-label studies, with the exception of pediatric obsessive-compulsive disorder (OCD), which has had a comparatively rich literature consisting of several double blind trials. This void has been lessening, however, with four double blind, placebo-controlled studies published in the pa…

Anxiety Disorders in Neurologic Illness.

Anxiety disorders frequently occur in individuals with neurologic illness. Anxiety may be a symptom of or a reaction to the neurologic disorder, a medication side effect, or a comorbid condition. The most common anxiety disorders seen in neurologic patients are panic disorder, generalized anxiety disorder, social phobia, and obsessive-compulsive disorder (OCD). Very often, these conditions go unrecognized (and therefore untreated) or are attributed to being a normal response to having a neurologic illness. However, if they are not treated, anxiety disorders can significantly increase morbidity…

Anxiety disorders in suicidal youth.

OBJECTIVE: To test the hypothesis that anxiety disorders are associated with suicidal ideation and suicide attempts in a child and adolescent sample referred to a clinic. METHOD: The sample comprised 1979 patients aged 5 to 19 years who were assessed using the Schedule for Affective Disorders and Schizophrenia for School Aged Children--Present Episode (K-SADS-P) at an outpatient mood and anxiety disorders clinic. Subjects were stratified by age and categorized into mutually exclusive groups as being nonsuicidal (n = 817), having suicidal ideation (n = 768), or having attempted suicide (n = 39…

The prevalence and clinical impact of anxiety disorder comorbidity in major depression were studied in 255 depressed adult outpatients conse … ety, hostility, or somatic symptom scores at the time of study presentation, patients with comorbid anxiety disorders tended to be younger during the index episode and to have an earlier onset of the major depressive disorder (MDD) than patients with major depression alone. [Diagnostic aspects of anxiety disorders].

A concise review is presented of epidemiological, family- and genetic studies in anxiety disorders. Special emphasis is given to recent developments in the classification of anxiety disorders. The literature in which a more central role of panic attacks is suggested, is critically reviewed. Based upon the studies reviewed it is questionable whether the diagnosis agoraphobia without panic attacks is justified.

Beta-blockers in anxiety disorders.

Studies evaluating the antianxiety and antipanic properties of beta-blockers do not support their routine use in treating either generalized anxiety disorder or panic disorder. The use of propranolol for anxiety disorders accompanied by physical symptoms, especially cardiovascular complaints, may be effective in some patients when combined with benzodiazepines or perhaps in some non-responders to conventional treatment. Better designed studies are needed to evaluate the exact role of beta-blocking agents in treating anxiety. The efficacy of propranolol in patients with panic disorder has not b…

Pharmacotherapeutic considerations in anxiety disorders.

Anxiety disorders comprise a variety of syndromes that require varied pharmacotherapeutic, psychotherapeutic, and behavioral interventions. Benzodiazepines have now replaced older sedatives and hypnotics as the preferred drug treatment for generalized anxiety. For panic disorder and agoraphobia with panic, the tricyclic antidepressants, the monoamine oxidase inhibitors, and the atypical benzodiazepine alprazolam have proved effective. When a simple phobia is severe enough to require treatment, behavioral approaches, with the possible adjunctive use of benzodiazepines, are recommended. Careful …

Behavioral psychotherapy for anxiety disorders.

Behavioral psychotherapy is probably the treatment of choice for most phobic and obsessive-compulsive disorders and plays a lesser role in the management of panic disorder without agoraphobia, generalized anxiety disorder, and posttraumatic stress disorder. This article discusses treatment goals and strategies for each disorder.

Anxiety disorders: prevalence and treatment.

A method is reported for classifying survey respondents according to syndromes resembling some of the anxiety disorders as defined by DSM III, using responses to a standard clinical symptom checklist administered on a large scale in the U.S.A. by trained survey interviewers in the 1979 National Survey of Psychotherapeutic Drug Use. These syndromes ('agoraphobia/panic', 'other phobia', and 'generalized anxiety') have several characteristics consistent with their DSM III diagnostic counterparts. They show 1-year prevalence rates of 1.2%, 2.3%, and 6.4%, respectively, roughly in accord with the l…

Genetic factors in anxiety disorders.

We investigated genetic factors in the determination of anxiety disorders in a study of 32 monozygotic (MZ) and 53 dizygotic (DZ) adult same-sexed twins. The frequency of anxiety disorders was twice as high in MZ as in DZ co-twins of the total proband group, alike in the MZ and DZ co-twins of the generalized anxiety disorder proband group, and three times as high in MZ as in DZ co-twins of the other proband groups. Anxiety disorders with panic attacks were more than five times as frequent in MZ as in DZ co-twins in a combined group of probands with panic disorders and agoraphobia with panic at…

Differential diagnosis of anxiety disorders.

The literature comparing panic disorder with natural fear, hypoglycemia, hyperthyroidism, pheochromocytoma, the hyperventilation syndrome, the mitral valve prolapse syndrome and partial complex seizures is briefly reviewed. Some features of each of these syndromes may clinically resemble panic disorder. It is concluded that: a) patients with panic disorder should be medically evaluated. b) the diagnosis of panic disorder should be based on a broad system, rather than on symptoms alone. c) diagnostic systems should include a category for "organic anxiety syndromes".

Anxiety sensitivity and its dimensions across the anxiety disorders.

Anxiety sensitivity (AS) refers to the fear of anxiety-related sensations, which is thought to arise from beliefs about their harmful consequences. AS is a multidimensional construct that consists of fears of somatic, social, and cognitive aspects of anxiety. In the present study, we examined the relationship between AS dimensions, assessed by factor-analytically derived subscales of the Anxiety Sensitivity Index-Revised (ASI-R), and anxiety-related psychopathology in a sample of 232 treatment-seeking patients with anxiety disorders. Correlational analyses and comparisons among anxiety disorde…

Use of benzodiazepines in social anxiety disorder, generalized anxiety disorder, and posttraumatic stress disorder.

Benzodiazepines are advantageous treatments for anxiety disorders because they work quickly. However, benzodiazepines can vary in terms of efficacy across anxiety disorders. Benzodiazepines have been found to be a superior treatment in social anxiety disorder. While benzodiazepines are effective in the treatment of generalized anxiety disorder, other treatments such as selective serotonin reuptake inhibitors may be more effective. Also, research indicates that benzodiazepines may not be effective in the treatment of posttraumatic stress disorder. Therefore, physicians need to consider the type…
