The management of childhood cataract begins at the initial contact with the family and typically extends far into the child's future. Decisions affecting long-term care and visual outcomes are often made in these initial preoperative encounters. Treatment will vary depending on whether the cataract is unilateral or bilateral and whether it is infantile onset or later. Thorough discussion of the treatment options is needed, especially with description of the life-long management issues for the child. Visual outcomes will vary, with the best visual acuity results being observed in older children

BACKGROUND: Cataracts pose a serious threat to the visual system in infants and children, requiring prompt surgical intervention and rehabilitation to optimize visual development. Glaucoma is a serious complication following cataract surgery in infants and can lead to loss of vision. Early diagnosis of glaucoma, however, can be quite challenging in these patients.

An Inuit boy was born with microphthalmia and a central corneal opacity of the right globe. Histopathologic examination revealed a central adherent corneal leukoma (Peter's anomaly) as well as absence of the lens. We are not aware of other well documented histopathologic reports of congenital aphakia and Peter's anomaly occurring in the same eye.

Because of their size and mass, weight is a major concern in the fitting of soft gel lenses. Lenticular design is necessary, and there are essentially two lens designs for aphakia: the spin-cast lens of Bausch & Lomb and the lathe-cut lenses of the other manufacturers. A solution to the problem of weight in a single cut soft contact lens is presented.

We performed a variety of tests to assess binocular function on monocular aphakic patients wearing contact lenses, monocular pseudophakic patients, bilateral spectacle-wearing aphakic patients, bilateral aphakic contact lens wearers, and bilateral pseudophakic patients. Stereoscopic acuity levels at distance and near when visual acuity was at least 6/12 (20/40) provided the most useful data. Pseudophakic patients and contact lens wearers performed equally well and outperformed spectacle wearers. Implant patients did not have superior stereoscopic acuity.
