In 57 aphakic second eyes of 88 patients who had previously suffered an aphakic detachment in the first eye, fifteen patients have so far suffered a retinal detachment in the second eye. However, of these fifteen patients, only two have suffered any reduction of central vision associated with detachment of the macula, reflecting an apparent change in the pattern of aphakic detachment disease in the second eye. In view of this observation, we do not think an energetic policy of prophylaxis involving 360 degrees cryotherapy should be pursued at the present time provided that follow-up facilities

Explanations are given for the physiological behavior of the cornea in the phakic and aphakic eye and for a cornea covered by a contact lens that causes epithelial anoxia. The normal, constant hydration of the cornea is a result of active ion transport by the endothelial layer. The hypoxic epithelium under a contact lens produces lactate ion that migrates to the posterior portion of the cornea where it modifies the normal stromal balance between osmotic and inhibition pressures. The current modality for correcting unilateral aphakia is by the use of daily wear hard and soft contact lenses and by the use of intraocular lens implants. The use of the Cooper Permalens soft contact lens as an extended wear lens for correction of aphakia was investigated in 177 eyes. The relative merits of the extended wear lens are discussed with emphasis on fit, visual function, and contact lens-related problems.

Aphakia may be said to be the best reason for contact lenses. All types are used--(hard) corneal lenses, soft lenses both for daily and extended wear, and even (hard) scleral lenses in exce,tional cases. At present all have their problems so that, although on theoretical grounds an extended wear lens would be ideal, the corneal lens is probably still the lens of choice.

The purpose of this study is to calculate the longitudinal spherical aberration and thickness, and consequently the mass of aphakic corneal contact lenses in air to determine optimum lens designs. The longitudinal spherical aberration has been calculated by the ray tracing method through the aspheric surfaces.
