Data from a prospective study of 50,826 neonates were used to determine the frequency and significance of disorders related to apnea in newborns. Such apnea proved to be a significant prognosticator for neonatal death. Fifty-eight percent of preterm neonates with multiple apneic episodes and 18% with a single episode died, whereas only 6% of those without recorded apnea died. The rates were 44%, 5%, and 1% for full-term infants. Amniotic fluid infection was the underlying disorder in 35% of the preterm and 25% of the term neonates who had multiple episodes of apnea. A third of the preterm and …

We have tested the hypothesis that severe lypoxia causes apnea, regardless of the arterial CO2 and pH, and that extreme hypoxia causes gasping. Acute experiments with airway occlusion and with low inspired oxygen (FIo2) were performed on anesthetized adult dogs and monkeys. Arterial oxygen saturation was recorded continuously with fiberoptic oximetry, and Pco2 by an electrode catheter. In addition, blood samples were obtained for Po2, Pco2, and pH. Apnea was induced regularly when the Pao2 fell below 10 torr, whether the Paco2 was high with asphyxia (63 torr) or low (26 torr) with low FIo2. Si…

Idiopathic central sleep apnoea (ICSA) is a form of periodic hyperventilation followed by central apnea that occurs in the absence of a clear etiologic factor. ICSA generally develops beyond the 5th decade of life and may occur in the presence of cardiovascular risk factors. The natural history and prognostic implications of ICSA are not well understood. In this context, the role of long-term suppressive treatment has not been established. For patients presenting with suspected ICSA, we suggest a careful evaluation for underlying metabolic or physiologic disturbances, monitoring of symptoms an…

Sleep apnea is a sleep disorder and a common comorbidity among patients with diabetes worldwide. Previous studies have demonstrated a high prevalence of prediabetes and diabetes in individuals with obstructive sleep apnea (OSA). The physiopathology of OSA is characterized by recurrent collapse of the upper airway during sleep, resulting in intermittent hypoxia and fragmented sleep. These cardinal features can induce sympathetic activation, systemic inflammation, and oxidative stress, thereby increasing the risk and severity of diabetes. OSA has been reported to be associated with the worsening…

BACKGROUND: Sleep apnea is a widespread and yet still underdiagnosed condition. Various studies from the past have provided evidence that there is a link between sleep apnea and various cardiovascular diseases, including arrhythmias.
OBJECTIVE: The aim of this article is to provide an overview of the current study situation and to point out possible consequences relevant to everyday life.
MATERIAL AND METHODS: A systematic search was carried out in various databases using the keywords sleep apnea (OSAS/SA) and arrhythmias/dysrhythmias.
RESULTS: There are several pathophysiological links bet…

Understanding the clinical neurophysiology of apnea generation encompasses discussion of the neuroanatomic aspects of central respiratory rhythm and pattern generation, including the central respiratory control networks, central and peripheral chemoreceptors, mechanisms of respiratory muscles, and sleep state dependent differences. Anatomical and functional links to apnea also involve central respiratory motor output recruited from the hypoglossal nerve, which has led to novel treatments for obstructive sleep apnea. Autonomic fluctuations occur in relation to sleep-wake and sleep states (i.e.,…

Obstructive sleep apnea (OSA) is a chronic and common adult disorder characterized by recurrent episodes of upper-airway obstruction and reopening during sleep. OSA is associated with intermittent hypoxia, sympathetic overactivity, oxidative stress and high cardiovascular mortality and morbidity. It is known to be more common in men than women, partly due to differences in anatomy and functional respiratory components. There are also gender differences in reported symptoms, leading to potential under-diagnosis in females. This gender difference tends to decrease after menopause, demonstrating …

Obstructive Sleep Apnea Syndrome Abstract. The obstructive sleep apnea syndrome (OSAS) is a disorder of the control of breathing in which repetitive collapse of the upper airway during sleep leads to frequent apneas/hypopneas with cyclic oxygen desaturation and arousals. Sleep is fragmented and unrefreshing. The affected patients suffer from an increased tendency to fall asleep, impaired concentration and a reduced quality of life. The consequences of OSAS also include an increased risk of accidents caused by falling asleep as well as cardiovascular diseases. The diagnosis of OSAS is based on …

Snoring, chronic rhinitis and fragmentation of sleep are common symptoms in pregnancy, especially in the third trimester. The prevalence of obstructive sleep apnea is also increased in pregnant women and can be associated with pregnancy induced hypertension, preeclampsia and diabetes mellitus. The fetus is as well exposed to a higher risk of prematurity and intra-uterine growth restriction. Given the lack of adequate screening tool, the diagnosis of obstructive sleep apnea is a challenge for the physician and a high degree of clinical suspicion is necessary in order to identify the patients at…

Apnea of prematurity (AOP) remains a major clinical problem in present day neonatology that warrants frequent evaluations and imposes challenges in therapeutic strategies. Although the pathogenesis of AOP is poorly understood, it is probably a manifestation of physiologic immaturity of breathing control rather than a pathologic disorder. Immature breathing responses to hypoxia, hypercapnia and exaggerated inhibitory pulmonary reflexes in preterm infants might also contribute to the occurrence or severity of AOP. Recent data suggest a role for genetic predisposition. Although typically resolve …

Central sleep apnea (CSA) is characterized by a lack of drive to inspire for at least 10 sec. In the CSA-syndrome accompanying arousals and desaturations of the arterial blood cause sleep disturbances and sympathetic nerve activations which lead to excessive daytime sleepiness and increase the risk for cardiovascular morbidity. There are six manifestations of CSA: a rare primary or idiopathic form, often in hypocapnic patients with an increased hypercapnic ventilatory drive; Cheyne-Stokes respiration, characterised by periodic CSA and a crescendo/decrescendo breathing pattern, often in patient…

Recurrent total or partial upper airway collapse occurring during sleep characterizes obstructive sleep apnea syndrome (OSAS). Although the disease is affecting more than 5% of the general population, it remains largely undiagnosed. The disease is associated with augmented cardiovascular risk, reduction in vigilance and attentional abilities, being responsible for traffic or occupational accidents. There is also glucose metabolism impairment likely to occur during OSA i.e. resistance to insulin and diabetes. Simplified tools are available in order to achieve the diagnosis either in the sleep l…

Some patients with obstructive sleep apnea (OSA) develop persistent central sleep apnea activity when started on continuous positive airway pressure (CPAP). This recently recognized syndrome, complex sleep apnea syndrome (CompSAS), is thought to be due to dysfunction of the respiratory center in a setting of an obstructive sleep apnea. We present the current knowledge on epidemiology, pathophysiology, and treatment options in CompSAS. We review the current literature on CompSAS, and present our own data on this interesting condition.

Obstructive sleep apnea syndrome is characterized by recurrent total or partial upper airway collapse during sleep. Although this disease affects more than 5% of general population, it remains largely undiagnosed. It is associated with an increase in cardiovascular risk and with a decrease in sustained attention that may cause automobile accidents or occupational injuries. Several simplified diagnostic tools (polysomnography) now allow easier diagnosis in a sleep laboratory or on an outpatient basis. Nasal continuous positive airway pressure remains the reference treatment, acting as a pneumat…
