Occlusion of the axillary artery occurred in a man with erect dislocation (luxatio erecta) of the shoulder. This combination of injuries has not previously been reported. The arterial injury was demonstrated by arteriography and repaired by a vein graft. There was an associated fracture of the greater tuberosity of the humerus, which required open reduction and internal fixation.

Circumflex humeral artery aneurysms are rare vascular injuries that can lead to disastrous consequences if there is a delay or error in diagnosis. Previously only described in young elite athletes, this case series of eight patients, including a symptomatic 59-year-old man, illustrates the need for its inclusion in the differential in isolated arm ischemia regardless of age. Ligation without revascularization is the most commonly reported intervention; however, this does not address the axillary artery inflammatory thickening associated with the development of these aneurysms. We propose that …

We report the unique case of a 14-year-old male with axillary artery pseudo-aneurysm associated with brachial plexus compression after a simple shoulder dislocation. Following shoulder reduction under anaesthetic, the child was discharged on chemical deep vein thrombosis (DVT) prophylaxis. However, progressive shoulder swelling and upper limb neurological symptoms developed. A multi-disciplinary approach was taken to diagnose and treat this complication. After stopping the anti-coagulants, a combination of endovascular stent-graft and open surgical decompression of the brachial plexus was empl…

BACKGROUND: The aim of our anatomic study was to assess whether the commonly used method of perfusion through the right axillary artery is sufficient in providing uniform distribution of blood to both hemispheres of the brain in patients undergoing surgery of the aortic arch. We considered that critical arteries to examine are anterior and left posterior communicating arteries of the circle of Willis because the absence or insufficiency of either one would drastically endanger perfusion to the left hemisphere of the brain. The existence and the diameters of these arteries were studied.

Axillary artery cannulation for cardiopulmonary bypass is becoming increasingly used for surgery of aortic dissections for reoperations and extensively diseased ascending aortas. This can be achieved either directly or with a graft. We describe a case with a repair of chronic type A dissection in which axillary cannulation was achieved by placing the arterial cannula into a saphenous vein graft that had been anastomosed end-to-side to the axillary artery. This provides a natural, inexpensive, readily available, and more hemostatic alternative to the use of prosthetic grafts.

An unusual bilateral variation in the arterial pattern of the axilla was observed in an embalmed cadaver. Each axilla contained two axillary arteries of similar origins but different patterns of branching and fate. The first part of each axillary artery was a single vessel as is the norm. It gave off a supreme thoracic artery and then bifurcated into two medium-sized arteries hence referred to as regular and variant. The variant artery ran in an antero-medial course, partly covered by the one axillary vein. The regular artery gave off the thoracoacromial and two posterior branches from its sec…

BACKGROUND: Femoral arteries are the preferred site of peripheral cannulation for arterial inflow in type A aortic dissection operations. The presence of aortoiliac aneurysms, severe peripheral occlusive disease, atherosclerosis of the femoral vessels, and distal extension of the aortic dissection may preclude their utilization. Axillary artery cannulation may represent a valid alternative in these circumstances.
METHODS: Between January 15, 1989, and August 20, 1998, in our institution, 22 of 152 operations (14.4%) for acute type A aortic dissection were performed with the use of the axillar…

Although axillary artery injury occurs frequently with dislocations of the shoulder and fractures of the clavicle, it is rarely associated with fractures of the proximal humerus. If the axillary artery is damaged, prompt recognition and treatment are necessary to salvage the involved extremity.

By means of a case report the diagnostic and therapeutic procedures and prognosis of a pseudo-aneurysm of the axillary artery after shoulder dislocation are demonstrated. The diagnosis of aneurysms of the axillary artery is difficult in some cases. It is important to mention a possible vascular injury in case of close reduction of the shoulder. Therefore in all patients with a penetrating or blunt trauma of the shoulder or the upper arm a thorough neurovascular examination is required. In patients with neurologic deficits or alteration of the pulse a duplex ultrasound examination should be per…
