Bereavement after homicide: a synergism of trauma and loss.

OBJECTIVE: The purpose of this study was to examine the intensity and relationship of trauma and responses to bereavement in family members after homicide. METHOD: The authors established an outpatient clinic that offered evaluation and supportive psychotherapy after the homicide of a family member. A standardized evaluation protocol was followed with 18 adults in order to detail variables of previous trauma, bereavement, and psychiatric disorder. Standardized measures of bereavement (Texas Revised Inventory of Grief) and trauma (Impact of Event Scale and Dissociative Experiences Scale) were …

Working with hope: Learning from bereaved parents.

OBJECTIVES: The complexity of hope in serious childhood illness can be underestimated in clinical practice. We sought to explore the lived experience of hope among bereaved parents to help clinicians interpret and engage with parents' varied and changing expressions of hope over the course of their child's illness. METHODS: Semi-structured interviews were conducted with bereaved parents of children known to a statewide pediatric palliative care service who died aged 16 years or younger from any medical condition. A phenomenological framework underpinned the study, and reflexive thematic analy

Guilt in bereavement: The role of experiential avoidance and rumination.

Guilt is a common emotional response to bereavement and has been associated with prolonged grief symptoms. Experiential avoidance and rumination are associated with the persistence of grief related symptoms, yet their role in explaining how guilt contributes to grief severity remains unclear. This study examined whether experiential avoidance and rumination mediate the association between guilt and prolonged grief symptoms in bereaved adults. A community sample of 690 Portuguese adults who experienced an expected or unexpected loss completed measures of guilt, experiential avoidance, ruminatio…

Alcohol and substance abuse in parentally bereaved youth.

OBJECTIVE: Little is known about the role of parental bereavement regarding alcohol and substance abuse. Our aim was to examine whether the incidence of alcohol and substance abuse is higher in parentally bereaved youth and, if so, what might explain this increased incidence. METHOD: In a longitudinal population-based study conducted between November 2002 and December 2012, the incidence of alcohol and substance abuse or dependence (ASAD) during a period of 5 years was examined (using DSM-IV criteria) in 235 youth whose parents died of suicide, accident, or sudden natural death and 178 demogr…

Bereavement, complicated grief, and the rationale for diagnosis in psychiatry.

Recent controversies over the DSM-5 raise a foundational question for all diagnostic classifications: what is their underlying purpose? The author raises this question in the context of the proposed elimination of the "bereavement exclusion" from the DSM-5; and the possible addition of the category called "Complicated Grief." The author argues that our psychiatric diagnostic scheme should not be aimed primarily at establishing boundaries among putative "natural types." Rather, it ought to be guided by the principle of "instrumental validity, " which focuses on reducing the suffering and incapa…

Rumination and psychological distress among bereaved partners.

Recently bereaved men who evidenced more negative ruminative thoughts in free-response interviews showed greater psychological distress on several outcome measures both 1 month and 12 months after their loss and less increase in positive morale over this 12-month period. Men who engaged in more analysis of themselves and the meaning of their loss reported greater positive morale 1 month after their loss but showed more persistent depression and absence of positive states of mind over the 12 months following their loss. Finally, men who reported more social friction also evidenced more enduring

Midwives' competence in perinatal bereavement care: A socio-ecological analysis.

BACKGROUND: Perinatal bereavement, including miscarriage, stillbirth, and neonatal death, poses profound psychological challenges for parents. Midwives are often the primary professionals supporting families through this process. However, many report feeling underprepared, lacking confidence, and experiencing compassion fatigue, which may affect the quality of care. AIM: This study aimed to examine midwives' competence in perinatal bereavement care within the framework of the Socio-Ecological Model, focusing on the influence of individual, interpersonal, and organizational factors, as well as…

Spouse bereavement and brain pathologies: A propensity score matching study.

AIM: Spouse bereavement is one of life's greatest stresses and has been suggested to trigger or accelerate cognitive decline and dementia. However, little information is available about the potential brain pathologies underlying the association between spouse bereavement and cognitive decline. We aimed to investigate that lifetime spouse bereavement is associated with in vivo human brain pathologies underlying cognitive decline. METHODS: A total of 319 ever-married older adults between the ages of 61 and 90 years underwent comprehensive clinical assessments and multimodal brain imaging includ…

Mania Following Bereavement: State of the Art and Clinical Evidence.

Bereavement is the state of loss, determined in most of the cases by the death of a close person. It is probably the greatest sorrow that can occur in an individual life. Grief is a normal, healthy response to loss, evolving through stages in the process of mourning. In some cases, bereavement may lead to the outburst of manic episode: despite literature data being scarce, reports have explored this important clinical entity, variously called as "funeral mania" or "bereavement mania". We systematically reviewed the literature exploring the possible relationships between bereavement and the ons…

Childhood bereavement due to parental death from acquired immunodeficiency syndrome.

Numerous articles stress the unique problems of mourning an acquired immunodeficiency syndrome (AIDS)-related death and suggest psychotherapeutic interventions. However, no studies have been conducted that identify what differs in the grief process when the death was AIDS-associated and whether the extensive clinical and research literature on bereavement can be generalized to AIDS. This paper focuses on a bereaved population that has suffered AIDS-related loss--the uninfected children of mothers who die of AIDS, a group that has been referred to as "AIDS orphans." We begin with a brief discus…
