The authors conducted a revisional study of intraepithelial papillary lesions of the bile ducts, characterized by being a kind of rare, intraductal growing cholangiocarcinoma. Articles published in the last 10 years were reviewed. The authors considered that the adenoma-carcinoma development is an important feature to warrant prophylactic measures through excisions. The histological type and biomolecular behavior may have relevance in the postoperative course of such lesions, which have a better prognosis when compared with other histological types.

Congenital dilations of the biliary tract are a group of rare conditions, commonly associated with the presence of an abnormality at the junction of the common bile duct and pancreatic duct, which may lead to the reflux of pancreatic juice within the biliary tree. The main clinical symptoms are, abdominal pain, cholangitis and acute pancreatitis, and the most serious complication is malignant degeneration. The treatment of choice is cholecystectomy and complete excision of the bile duct from its bifurcation to its intra-pancreatic segment.

OBJECTIVE: Parasites residing in the biliary tree include Clonorchis sinensis, Opisthorchis viverrini, Opisthorchis felineus, and Fasciola hepatica. They are willowy, leaf-like, flat flukes dwelling in the bile ducts and gallbladder. Human ascarides, Ascaris lumbricoides, dwelling in the small intestine, inadvertently migrate into the bile ducts and cause biliary obstruction. The purpose of this article is to illustrate typical imaging findings of liver fluke infection and biliary ascariasis.

Parasitic infestations of the biliary tract are a common cause of biliary obstruction in tropical countries and can lead to such serious complications as cholangitis and cholangiocarcinoma. In the years 1987--1989 endoscopic biliary tree stenting was performed in 64 patients. The straight prostheses type Amsterdam 10 or 12 F and double pigtail prostheses were applied. In 36 cases the indication for stenting was malignant stricture of the biliary tree (group I) caused by carcinoma of the ampulla of Vater (n = 12), carcinoma of the head of the pancreas (n = 15), common bile duct and bifurcation tumor (n = 5) and gallbladder cancer (n = 4). Gallstone disease occurs in 20% to 30% of the elderly, is usually silent, and is rarely fatal. Silent GSD requires no treatment. Symptomatic GSD can be treated surgically, nonsurgically, or, if there are minimal symptoms, expectantly. The decision is based largely on physician experience and informed patient preference. Multiple synchronous tumors of the extrahepatic biliary tree are not frequently reported. Over a 2-year period, 54 operative procedures were performed for tumors of the extrahepatic biliary tract or periampullary region. In five of these cases, unsuspected tumors were observed. Cystic disease of the biliary tract is rare in Western countries with an incidence of 1/2,000,000 live births. Five anatomical types are recognized. The majority (80%) become symptomatic before the age of 30 years. The cysts rarely have an intact epithelial lining and this often exhibits metaplasia. Although the clinical significance of a high antibiotic level of the biliary tract has not been proved, few antibiotics are both well excreted and active against the common pathogens of bile. The biliary excretion of these agents is a complex phenomenon which may be altered by a number of host factors. The results of studies on the levels of antimicrobial agents vary with respect to the timing of antibiotic measurement, type of bile sampled, presence or absence of biliary tract obstruction and assessment of data.

We report the clinical and pathologic features of five children wih a botyroid rhabdomyosarcoma of the biliary tract. There were three boys and two girls ranging in age from 11/2 to 51/2 years at diagnosis. Obstructive jaundice was the usual presenting sign, often with fever and hepatomegaly. Cholescintigraphy with technetium-labeled biliary agents has great value in evaluation of the patient with suspected acute cholecystitis. Visualization of the gall bladder virtually excludes acute cholecystitis and obstruction of the cystic duct. Damage of the biliary tract at the end of a period of hepatic preservation prior to liver transplantation has been observed in man. This damage may be important in the development of early biliary complications. A model of hepatic biliary ischaemia in the pig has been developed which permits comparison between the influences of warm and cold ischaemic damage on the biliary tract. The major damage to the biliary tract in the pig in this model appears to occur after prolonged cold preservation.
