Extensive research into the physiology of red blood cells and platelets has shown that it is more advantageous to separate blood into its components than to store it as whole blood. After the red blood cells and platelets have reached the restricted period of storage in the liquid state, they can be freeze-preserved and stored for prolonged periods. Plasma can be frozen for future use or can be fractionated into the various stable components. The use of a combination of liquid-preservation and freeze-preservation methods affords a more efficient use of the limited supply of available blood.

The physician's responsibility regarding the indication and induction of blood transfusion, the request for fresh whole blood and the decision for the degree of urgency are outlined. The risk of hemolytic transfusion reaction, one of many transfusion hazards, is calculated on the basis of the results obtained from a 7-year antibody screening program in blood recipients. Measures for the prevention of hemolytic transfusion-reactions are discussed.

It is widely accepted that blood transfusions can cause allosensitization, but it is often reported that new human leukocyte antigen (HLA) antibodies are nonspecific and transient. This study explores the effect of blood transfusion on allosensitization in waitlisted transplant patients including the development of transfusion-specific antibodies (TSAs), both while they remain on the waiting list, and following subsequent transplantation. A total of 105 blood donors of transfusions received by 50 patients on the transplant waiting list were HLA typed. De novo HLA antibodies developed in 62% of…

Transfusions are more common in premature infants with approximately 40% of low birth weight infants and up to 90% of extremely low birth weight infants requiring red blood cell transfusion. Although red blood cell transfusion can be life-saving in these preterm infants, it has been associated with higher rates of complications including necrotizing enterocolitis, bronchopulmonary dysplasia, retinopathy of prematurity and possibly abnormal neurodevelopment. The main objective of this review is to assess current red blood cell transfusion practices in the neonatal intensive care unit, to summar

PURPOSE OF REVIEW: Blood transfusion is ubiquitous in major burn injury. The present article describes recent research findings directly impacting blood transfusion strategies in major burn injury both in the operating room and the ICU.
RECENT FINDINGS: Transfusion strategies have been the focus of recent burn investigations. First, a randomized prospective trial encompassing both the ICU and operating room reported that a restrictive red blood cell transfusion threshold (7 g/dl) had equivalent outcomes to a traditional threshold (10 g/dl) for burns more than 20% in terms of mortality, infect…

Anaemia is the most frequent haematological disease in older patients with prevalence up to 60% among patients hospitalized in acute geriatric ward. Furthermore, people aged 65 years and over receive more than half of the blood transfusion. The tolerance and the symptom of anaemia in older patients are very variable from one patient to another depending on ageing and comorbidities. The transfusion decision should not be based on haemoglobin levels but should be based on the benefit/risk balance taking into account patients' symptoms due to anaemia, benefit and expected efficacy as well as risk…
