Limb lengthening and correction of angulation deformity: immediate correction by using a unilateral fixator.

We performed limb lengthening (callus distraction) with immediate correction of the angulation deformity in 11 patients (16 long-bone segments) in Chiba Children's Hospital. The operated segments were 14 tibias and two femurs. The average age at operation was 12.0 years (range, 4.8-15.3). All had varus deformities and limb shortening. We used the Orthofix unilateral fixator in all cases. The following two types of osteotomy were selected, depending on the degree of the varus deformity: opening wedge osteotomy if it was < 20 degrees or dome osteotomy if > 20 degrees. The average amounts of the

[Achondrogenesis. Apropos of a case].

The authors report a new case of achondrogenesis, the study of which is principally directed towards the histopathological examination of the conjugal cartilage. A brief review of the literature recalls the main clinical, radiological and histopathological characteristics of this fatal chondrodysplasia transmitted by an autosomal recessive means. In addition to other features, it reveals the difficulty which may be experienced in differentiating between the two types (I and II). Finally, the differential diagnosis is briefly assessed, the problem being the elimination of other non-transmissibl

Al-Gazali Skeletal Dysplasia Constitutes the Lethal End of ADAMTSL2-Related Disorders.

Lethal short-limb skeletal dysplasia Al-Gazali type (OMIM %601356), also called dysplastic cortical hyperostosis, Al-Gazali type, is an ultra-rare disorder previously reported in only three unrelated individuals. The genetic etiology for Al-Gazali skeletal dysplasia has up until now been unknown. Through international collaborative efforts involving seven clinical centers worldwide, a cohort of nine patients with clinical and radiographic features consistent with short-limb skeletal dysplasia Al-Gazali type was collected. The affected individuals presented with moderate intrauterine growth res

Guided growth using a tension-band plate in Blount's disease.

Our observational study's objective was to determine how effective guided growth with tension-band plates was to correct the deformity in Blount's disease. We reviewed the records of 14 children (18 limbs) with Blount's disease who were treated with tension-band plates as the only surgical intervention at a single institution over eight years. Five children (seven limbs) had infantile Blount's disease with Langenskiöld stage ≤2. Nine children (11 limbs) had late-onset Blount's disease. The mean age at operation was 7.2 years (SD, 3.1, range, 2.9-11.8). The tension-band plate effectively correc

Multicenter Series of Deformity Correction Using Guided Growth in the Setting of Osteogenesis Imperfecta.

BACKGROUND: The mainstay of deformity correction and fracture prevention for patients with osteogenesis imperfecta (OI) includes osteotomies and intramedullary rodding. Guided growth, described in the setting of skeletal dysplasias, offers a less invasive means of deformity correction. We report a multicenter case series of guided growth procedures in the setting of OI. METHODS: We retrospectively reviewed patients with OI at three institutions from April 2012 to April 2019: 18 patients underwent guided growth for angular deformity correction with minimum 1-year follow-up or full deformity co

Dysplasia epiphysealis hemimelica. Clinical, histological and histochemical features.

Dysplasia epiphysealis hemimelica is a condition characterised by asymmetrical and uneven growth of the epiphyses of the long bones of the limbs and of the bones of the tarsus and carpus. The growth disturbance is caused by the development of accessory nuclei that exhibit histological and histochemical features similar to those found in ossifying epiphyseal cartilage and in the ossific centres of developing carpal and tarsal bones. The common histogenesis, as confirmed by the authors, could explain the elective localisations of the disorder.

Short rib-polydactyly syndrome type 3.

Three infants died perinatally of asphyxia due to thoracic narrowing. Their uniform radiographic findings are different from previously described and recognized chondrodysplasias and possibly represent a distinct syndrome. Three identical or very similar cases are found in the literature.

Dysplasia Epiphysealis Hemimelica in the Lower Extremity.

BACKGROUND: Because of the rarity of dysplasia epiphysealis hemimelica (DEH), little is known about the relationship between disease classification and clinical symptoms or patient outcomes. This studies therefore aims to characterize DEH of the lower extremity and correlate radiographic classification to presenting symptomatology and need for surgical intervention. METHODS: A multi-center, retrospective review of all patients with DEH of the lower extremity over a 47-year period was conducted. Demographic data, presenting complaints, treatments, and symptoms at final follow-up were recorded.

Percutaneous hemiepiphysiodesis using transphyseal screws for adolescent tibia vara.

Hemiepiphysiodesis around the knee is becoming the mainstay procedure in adolescents for a wide range of aetiological deformities, when considering adolescent tibia vara (ATV), the published series have variable results. The purpose of this study was to review our experience with the percutaneous transphyseal screw (PETS) in these patients followed until bone maturity. We analysed the charts from 13 patients (20 knees) that underwent lateral tibial hemiepiphysiodesis using PETS. The radiographs were accessed before surgery, at implant removal, when occurred, and at the final follow-up. The cli

The article presents the results of evaluation of clinical pathogenic value of hormones, markers of metabolism and indicators of mineral metabolism information of inherent pathology of locomotive system in children. The sampling included 29 children with dysplasia and deformation of lower extremities and 35 children without pathology of locomotive system. All children aged from 6 to 12 years. The serum levels of parathormone, calcitonin and 25(OH)-D3 were established using analytic module platform "Cobas 6000 SWA" (Roche Diagnostics, Switzerland). The content of somatotropic hormone in blood s

Pediatric aspects of skeletal dysplasia.

Skeletal dysplasia is a disorder of skeletal development characterized by abnormality in shape, length, a number and mineral density of the bone. Skeletal dysplasia is often associated with manifestation of other organs such as lung, brain and sensory systems. Skeletal dysplasias or dysostosis are classified with more than 400 different names. Enchondral bone formation is a coordinated event of chondrocyte proliferation, differentiation and exchange of terminally maturated chondrocyte with bone. Impaired enchondral bone formation will lead to skeletal dysplasia, especially associated with shor

The morbid anatomy of congenital deficiency of the tibia and its relevance to treatment.

Two specimens obtained from a 5-month-old boy with bilateral type 1a congenital tibial deficiency were dissected to describe their morbid anatomy. Examination revealed a rigid equinovarus deformity, absence of a tibial remnant, an abnormal saddle-shaped talus, and several tarsal coalitions. Observation of the arterial pattern during surgery supported the previously reported finding that persistence of an immature arterial structure is inherent in this condition. Knowledge of potential structural anomalies is essential during the planning of an amputation or of a knee or an ankle reconstruction

Radial lengthening for septic growth arrest.

A case is presented of a child who sustained septic growth arrest of the distal radial epiphysis following osteomyelitis at 5 weeks of age. At age 8 years and 8 months radial lengthening using the Wagner apparatus was performed. Two and one-half centimeters of total length (18% of original length) was gained. No significant complications were encountered and normal function has been maintained. Although follow-up is short and additional lengthening is anticipated, this report illustrates the technical considerations for performing this procedure.

[Cranio-vertebral junction anomalies and osseous dysplasias (author's transl)].

The cartilaginously preformed occipital region of the cranium is ontogenetically a part of the backbone. The cranio-vertebral junction is unique among the other junctional areas of the backbone: 1. It has not yet been proved that different combinations of cranial and caudal junction anomalies are not combined with cervical junction anomalies. 2. The assimilations of the atlas which are considered to be progressive and regressive variants and the so-called manifestations of the occipital vertebra, i.e. diametrically opposed members of a developmental line occur together with a frequency that is

Sphenoid dysplasia in neurofibromatosis type 1: a new technique for repair.

PURPOSE: Sphenoid bone dysplasia in neurofibromatosis type 1 is characterized by progressive exophthalmos and facial disfiguration secondary to herniation of meningeal and cerebral structures. We describe a technique for reconstruction of the sphenoid defect apt at preventing or correcting the ocular globe dislocation. METHODS: After placement of spinal cerebrospinal fluid drainage to reduce intracranial pressure, the temporal pole is posteriorly dislocated extradurally. The greater sphenoid wing defect is identified. A titanium mesh covered by lyophilized dura, modeled in a curved fashion, i

Growth disturbance in aseptic osteonecrosis of the humeral trochlea (Hegemann's Disease): a case report with developmental distal radioulnar joint incongruency.

A rare case of aseptic osteonecrosis of the humeral trochlea is described. The patient presented with a late onset of wrist pain caused by a length discrepancy with the short ulna relative to the radius. Close observation is needed for the regeneration of humeral trochlea, as well as for the significant effects of growth disturbance, which might cause the distal radioulnar joint incongruency.

A new method for the correction of hypertelorism with preservation of the olfactory nerve filaments.

A method for the preservation of the olfactory nerve filaments in cases of hypertelorism correction is described. The cribriform plate is completely resected and the nerve filaments gathered in the midline after medial rotation of the orbits. In contrast to Converse's (1970) procedure of paramedian osteotomies the method allows full correction of all degrees of hypertelorism, even in the most extreme cases, without olfactory nerve impairment.
