[Surgical management of bone metastases of the lower extremity with AO interlocking nail].

Between January 1991 and June 1995 we have operated on 19 patients (9 male, 10 female) with 22 skeletal metastases of the lower limb (19 femora, 3 tibiae) using a static interlocking nail. Closed intramedullary nailing without resection of the metastasis has been established as our standard procedure. We have stabilized 15 patients with advanced osteolysis and seven pathological fractures. Sixteen patients underwent postoperative local radiation therapy with 40 Gy. As intraoperative complications we have observed one fracture of an osteolysis and one death due to fat embolism. Postoperatively

Tibial fractures with bone loss treated by primary reamed intramedullary nailing.

We reviewed the results of the treatment of 30 tibial fractures with minor to severe bone loss in 29 patients by early soft-tissue and bony debridement followed by primary locked intramedullary nailing. Subsequent definitive closure was obtained within the first 48 hours usually with a soft-tissue flap, and followed by bone-grafting procedures which were delayed for six to eight weeks after the primary surgery. The time to fracture union and the eventual functional outcome were related to the severity and extent of bone loss. Twenty-nine fractures were soundly united at a mean of 53.4 weeks, w

Treatment of excessive bone loss from both crus bones with internal bone transport using an intramedullary nail. Case report.

We present a case report of a 16-year-old patient who suffered multisite and multiorgan injuries following a road accident. The most prominent musculoskeletal problem was a bilateral crush injury of the crus in combination with open fractures. The aim of this study is to present the possibilities and available methods of treatment of patients with extensive posttraumatic and postinflammatory bone loss of the lower limbs that make it possible to avoid amputation. We used intramedullary nailing and internal bone transport to reconstruct continuity and leg length of both crus bones. The treatment

Slow Recovery of Weight Bearing After Stabilization of Long-Bone Fractures Using Elastic Stable Intramedullary Nails in Children.

Stabilization of diaphyseal long-bone fractures using elastic stable intramedullary nails (ESIN) in children promises early mobilization and rapid resumption of full weight bearing. We evaluated the duration of postoperative functional rehabilitation after ESIN, measured by the time from stabilization until first partial weight bearing, full weight bearing, and resumption of school sports. Fifty children with unstable, displaced fractures of the femur or lower leg treated with ESIN between 2002 and 2012 were included in this retrospective analysis. We classified fractures according to the pedi

Threaded Antibiotic-Coated Locking Nails in Osteomyelitis-Associated Long-Bone Non-Union: Short-Term Outcomes of a Prospective Cohort.

Background: Long-bone non-unions complicated by osteomyelitis remain a major reconstructive and healthcare challenge, particularly in resource-limited settings with a high prevalence of multidrug-resistant (MDR) pathogens. Conventional staged management is associated with a prolonged treatment burden, repeated procedures, and delayed functional recovery. This study evaluated the clinical, radiological, functional, and short-term safety outcomes of a single-stage approach using custom-threaded antibiotic-coated locking nails (TACLNs) in a high-resistance cohort. Methods: This prospective single

Biodegradable Zn-Mg-Ca-Sr alloy intramedullary nails promote vascularized bone regeneration and suppress osteoclastogenesis for osteoporotic fracture repair.

Osteoporotic fractures are commonly associated with impaired bone regeneration and insufficient mechanical stability, posing major challenges for load-bearing internal fixation. Biodegradable metallic implants that combine mechanical support with biological regulation are therefore highly desirable for osteoporotic fracture repair. Here, we designed and evaluated a biodegradable Zn-Mg-Ca-Sr quaternary alloy as small-diameter intramedullary nails (IMNs) for osteoporotic load-bearing fracture fixation. Benefiting from multi-element alloying and hot extrusion, ZnMgCaSr(0.2) exhibited the most fav

Results of Lower Limb Bone Lengthening by Using Motorized and Magnet-Driven Intramedullary Nails to Treat Limb Length Discrepancy.

INTRODUCTION: Limb length discrepancies (LLDs) can be treated with limb lengthening using external fixation or intramedullary nails, such as PRECICE or FITBONE. PRECICE is a magnetically driven titanium nail, while FITBONE is a motorized nail; both use an external remote control. This study aimed to determine the complications, compensation of length difference, time needed for compensation, and healing outcome of bone lengthening using motorized or magnet-driven intramedullary nails among non-cosmetic limb lengthening cases. METHODS: This retrospective study was derived from a single surgeon

Painful pathologic fracture of the humerus: percutaneous osteoplasty with bone marrow nails under hybrid computed tomography and fluoroscopic guidance.

A case of a 75-year-old patient with a painful pathologic humeral shaft fracture, with unacceptably high surgical risk and unsatisfactory analgesia is reported. In this case, impaired arm function and persistent pain with conservative management resulted in a poor quality of life. Palliation with image-guided percutaneous osteoplasty was considered. Because of potential cement leakage, inadequate fracture reduction, the site of the fracture, and the mobility of the joints in that area, image-guided percutaneous delivery of metallic bone marrow nails implanted together with polymethyl methacryl

Bone Healing Index and Complications of a Magnetic Internal Lengthening Nail: A Retrospective Series of 286 Bone Lengthening Events.

BACKGROUND: The time required to form new bone is a critical aspect of bone lengthening but is not constant depending on the osteotomy location. The objective of this study was to establish the bone healing index (BHI) after femur and tibia bone lengthening procedures. Additional objectives included identifying procedure-related complications and implant reliability. METHODS: A retrospective analysis was conducted on all consecutive internal lengthening nails (Precice; Nuvasive) implanted in the femur or tibia at a single institution from September 2012 to November 2019. Groups were delineate

Endoscopic resection of scar tissues and bone graft with an intramedullary nail left in situ for distal femoral nonunion after nailing: A case report.

Femoral shaft nonunion after the intramedullary nailing has been successfully treated with advances in surgical technique. Nonetheless, the techniques cause morbidity to periosteum at the fracture site. We report the case of a 67-year-old man who underwent endoscopic surgery for refractory nonunion following the fracture of the distal femoral shaft, despite two fixations using an interlocking nail. In addition, the patient had uncontrolled diabetic mellitus. Endoscopy allowed us to acquire a clear view of the nonunion site, in which the resection of scar tissues and the packing of the cancello

Reunion of post nail aseptic non-union of diaphyseal femoral fractures by augmentation plating, decortication and bone grafting - Replacement for exchange nailing.

INTRODUCTION: The aim is to re-unite post nail aseptic non-union of diaphyseal femoral fractures in an economical and safe way with almost certainty, reducing dependence on others with early rehabilitation. The cascade of fracture healing is dependent on mechanical stability and uninterrupted cellular and bio-chemical responses (biology). Intra-medullary nailing in fresh diaphyseal femoral fractures is gold standard treatment with high success rate, still non-union of these fractures is reported, though the incidence has declined considerably. METHOD: This prospective study conducted at level

Titanium vs. carbon fiber-reinforced intramedullary nailing for humeral bone tumors.

BACKGROUND: Multiple techniques have been described to treat humeral diaphyseal bone tumors requiring curettage or excision. Recent studies have suggested that carbon fiber-reinforced polyetheretherketone (CFR-PEEK) intramedullary nails (IMNs) may be preferable to titanium IMNs for patients with musculoskeletal tumors due to CFR-PEEK's high tensile strength, radiolucency, a modulus of elasticity closer to native bone, and improved postoperative surveillance/radiation dosing. In this study, we describe the rate of fixation failure for both CFR-PEEK and titanium humeral IMNs when used for humera
