[Neurologic complications after bone marrow transplantation].

The retrospective study presented here reports clinical, electrophysiological and CT-data from 21 patients who underwent bone marrow transplantation for leukemia or aplastic anemia between 1980 and 1985. The aim of the investigation, which was performed at the end of 1986, was to elucidate whether antineoplastic drugs and radiation can result in long-term damage of the central nervous system. In 9 patients we observed neurological signs (1 leukencephalopathy, 5 cerebellar symptoms). Seven patients revealed dysrhythmia in the EEG and five had delayed latencies of visual evoked potentials. Eight

[Hepatic complications of bone marrow transplantation].

Bone marrow transplantation has become an accepted procedure for the treatment of severe aplastic anemia, hematologic malignancies (particularly lymphoma and leukemia) and certain inborn errors of metabolism and immunity. However, numerous complications follow bone marrow transplantation. Liver disease is a very common and complex complication after human transplantation. The major complications are: veno-occlusive disease, graft-versus-host disease acute and/or chronic. In this decade, the results of bone marrow transplantation will enhance.

[Transplantation using allogeneic bone in orthopedics].

The reconstruction of large skeletal defects presents a challenging problem to the orthopaedic community. Currently bone graft surgery enjoys renewed interest because of the increased demand for bone grafts in limb salvaging bone tumour surgery and reconstructive surgery on failed arthroplasties. Two patients treated with an allograft are presented.

[Artificial nutrition in bone marrow transplantation].

The issue of nutritional support in oncologic patients is reviewed. Special reference is made in relation to patients undergoing bone marrow transplant and their possible digestive tract and nutritional changes induced by the different therapies available in these situations. Nutritional evaluation and caloric and nitrogen requirement calculations have been proposed. Likewise, several possibilities have been considered regarding the route of administration, timing and quality of the nutriments to be administered, both to adult and pediatric patients.

Autoimmune thyroiditis after bone marrow transplantation.

We describe the transfer of autoimmune thyroiditis by bone marrow transplantation. The male recipient developed compensated hypothyroidism 3 years after transplantation, and progressed to thyroid failure 1 year later. The female sibling had compensated hypothyroidism when screened 4 years after marrow donation. The donor, the recipient, and an older male sibling were HLA DR4, 5; the father was presumed homozygous for DR4 and the mother for DR5. Only the donor and recipient had abnormal thyroid function and positive antithyroid antimicrosomal antibodies. The most probable mechanism for the orga

Bone marrow transplantation for genetic diseases.

Bone marrow transplantation is the treatment of choice for a number of genetic diseases. Recently, bone marrow transplantation has been increasingly used for erythroid disorders, such as thalassemia and sickle cell anemia. A number of inherited metabolic disorders (i.e., storage diseases, leukodystrophies, and the like) may be corrected with a marrow transplant. Successful correction of genetic diseases with allogeneic bone marrow transplantation lays the groundwork for the use of specific gene therapy.

Bone marrow transplantation. Part I--Allogeneic.

Major progress in experimental and clinical research has made allogeneic bone marrow transplantation a highly effective therapy for a variety of malignant and nonmalignant diseases. Allogeneic bone marrow transplantation from histocompatible donors is now the therapy of choice for some of these disorders. We review in part I the history, technical approach, complications, and the results achievable with this therapeutic approach. Further experimentation and future goals are also discussed.

Recent advances in bone marrow transplantation.

An increasing number of diseases may be treated successfully by allogeneic bone marrow transplantation (BMT). Initially used for the treatment of immunodeficiency where a cell series or product is replaced, it has now become routine treatment for many forms of leukemia where the transplant provides the rescue after lethal marrow ablation. Recently, diseases such as thalassemia and other inherited metabolic diseases have also been treated by BMT. Formerly the problems of BMT were mainly concerned with graft versus host disease (GVHD) in HLA-matched transplants with HLA-mismatched ones not being

Ocular complications of bone marrow transplantation.

Thirty-four patients who had undergone bone marrow transplantation (BMT) were examined; 83.3% of those who received single shot and none of those who received fractionated total body irradiation (TBI) developed cataracts. The use of steroids to treat chronic Graft Versus Host Disease (GVHD) produced more severe cataracts in those who had allogeneic transplants after single shot TBI, but follow-up has not been long enough to assess their effect following fractionated TBI. Keratoconjunctivitis sicca (KCS) was seen in 81.8% of patients with chronic GVHD and in 33.3% of patients after autologous B

Autologous bone marrow transplantation in children.

Autologous bone marrow transplantation is a procedure that allows for the delivery of high doses of chemotherapy and radiation to treat pediatric malignancies. There have been many studies showing a dose-response curve for many of the drugs that have been used for cytoreductive therapy in autologous bone marrow transplantation. These dosage ranges are achievable in many of the preparative regimens used in autologous bone marrow transplantation. The results in neuroblastoma, Hodgkin's disease, non-Hodgkin's lymphoma, Ewing's sarcoma, rhabdomyosarcoma, osseous sarcoma, other soft tissue sarcomas

Allogeneic and autologous bone-marrow transplantation.

The author of this paper presents an overview of the current status of bone marrow transplantation, including indications, pre-transplant considerations, the transplant procedure, acute and delayed transplant-related problems, results currently attainable, and a short discussion of possible future developments.

Q fever following bone marrow transplantation.

We describe a patient who developed an acute febrile illness 85 days after receiving an allogeneic bone marrow transplant for acute myeloid leukaemia. Serological investigation indicated the cause to be Q fever. The patient was receiving concurrent immunosuppressive therapy for graft-versus-host disease (GVHD). Coxiella burnetii should be added to the list of organisms which may complicate bone marrow transplantation. This case also provides further evidence to support an association between immunocompromised conditions and the development of Q fever.

Bone marrow transplantation in DiGeorge syndrome.

A Hispanic infant girl with DiGeorge syndrome underwent successful bone marrow transplantation (BMT) at age 28 1/2 weeks. She had typical facies, a cardiac defect, hypoparathyroidism, severe T and B cell immunodeficiency, and low levels of facteur thymique serique (FTS). In vitro incubation of the peripheral blood lymphocytes with thymosin alpha 1 showed no increase in the number of T cells on two occasions. A fetal thymus for transplantation was not available, and further review of past experience with thymic cells or factors revealed inconsistent and incomplete responses. Because of the pati
