BACKGROUND: The success of anatomic total shoulder arthroplasty depends on glenoid component stability, with loosening responsible for nearly half of revision surgeries. Superior glenoid inclination exacerbates component loosening by promoting unbalanced loading by the humeral head. This study investigates a novel superiorly augmented glenoid component designed to correct excessive superior inclination. We hypothesized that increasing augmentation would limit superior humeral head migration and asymmetric loading, thereby increasing compressive rather than tensile stresses at the implant-bone

BACKGROUND: Satisfactory management of bone defects is important to achieve an adequate reconstruction in revision TKA. Metaphyseal cones to address such defects in the proximal tibia are increasingly being used; however, the biomechanical superiority of cones over traditional techniques like fully cementing the implant into the defect has not yet been demonstrated. Moreover, although long stems are often used to bypass the defects, the biomechanical efficacy of long stems compared with short, cemented stems when combined with metaphyseal cones remains unclear.

An attempt was made to assess the local chronic inflammatory response in patients with failed orthopaedic implant that is clinically associated with osteolysis, bone and bone marrow necrosis. The main objective was to analyse the heterogeneity of the macrophage functional subsets in the bone-implant interface membrane and to evaluate their possible role in the development of an erosive inflammatory lesion within the bone. Immunohistology was performed on 21 specimens of the bone-implant interface obtained from 17 patients during revision arthroplasty, and synovial membranes from rheumatoid (RA…

BACKGROUND: Controversy over the long-term clinical effectiveness of hydroxyapatite (HA)-coated dental implants still persists, despite numerous clinical studies documenting high survival rates. Concerns about the degradation of the coating over the years have been raised; it has been speculated that resorption of the HA could produce a space between the implant and the bone with a resultant mechanical instability.
METHODS: Two HA-coated implants were retrieved due to a fracture of the abutment screws after a loading period of 14 years and were treated to obtain thin ground sections for histo

Implant-associated infection is commonly addressed by surfaces designed for direct bacterial killing, yet early host immune events at the implant interface are critical for infection outcome. Here, we designed Ti-A, a mildly alkaline titanium interface, to examine whether interfacial pH could act as an immune-regulatory cue for host-directed infection control. Ti-A showed acceptable cytocompatibility and promoted neutrophil extracellular traps (NETs) formation, thereby enhancing neutrophil-mediated killing of S. aureus. The attenuation of antibacterial activity after DNase I treatment supporte

Peri-implant diseases are common plaque-related conditions, and implant design may influence peri-implant inflammation. Two-piece systems include an implant-abutment micro-gap, while one-piece implants eliminate this interface. This narrative review aims to integrate microbiological, biomechanical, and clinical evidence to evaluate whether eliminating the implant-abutment micro-gap in one-piece implants is associated with differences in peri-implant inflammation and marginal bone level changes compared with two-piece systems. A focused narrative review was conducted using PubMed/MEDLINE, Scopu

Inadequate osseointegration remains a major clinical challenge in joint replacement surgery, frequently resulting in aseptic loosening and necessitating revision procedures. Herein, we present a novel approach that combines 3D printing, tissue engineering, and biomimetic technologies to address this problem. We developed a novel surface modification strategy to engineer two functional interfaces on microporous titanium implants (PTi): an intrafibrillar/extrafibrillar mineralized collagen interface (I-EMC/PTi) and an extrafibrillar mineralized collagen interface (EMC/PTi). The fabrication proce

The mechanical basis responsible for the development of secondary osteoarthritis in adults and slipped capital femoral epiphysis in adolescents is discussed, with the role of the oblique weight-bearing surface in generating abnormal shearing forces in these two conditions emphasized. The same biomechanical principles have been found to apply to the acetabular component in total hip implants. A vertical cup position will lead to abnormal stress concentration in the outer edge of the bone implant interface with an increase in shearing forces. The form of the subchondral bone sclerosis on the rad

Placing a dental implant elicits a time-dependent bone response controlled by wound-healing factors (cytokines, bioelectrical signals), biomechanics (gravitational, functional, and therapeutic loads), and mineral metabolism (hormones, diet, excretion). The osseous response to an implant involves four physiological stages: (1) endosteal and periosteal callus formation; (2) compaction and remodeling of the callus; (3) remodeling (turnover) of the nonvital interface and adjacent bone; and (4) maturation (secondary mineralization) of new bone. Long-term maintenance of a rigid implant interface is

Artificial disc replacement (ADR) is a clinical procedure used to diagnose cervical degenerative disc disease, preserving range of motion (ROM) at the fixation level and preventing adjacent segment degeneration (ASD). This study analyzed the biomechanics of ADR by examining range of motion (ROM), stress levels in bone and implants, and strain in the bone-implant interface using multi-scale loadings. The study focused on single- and double-level patients across various loading scales during physiological motions within the cervical spine. Results showed increased ROM in single-level and double-…

Large cyclic movements between the femoral stem and bone during the first weeks after total hip arthroplasty may hamper bone ingrowth and adversely affect the eventual success of the arthroplasty. Little is known, however, about the magnitude of the motions and its relationship to design and surgical factors. A two-dimensional finite element model of a cementless prosthesis inserted into the proximal femur was constructed to study the effects of two mechanical variables--the stiffness of the implant and the coefficient of friction between bone and implant--on the magnitude of the motions. We i

Three types of shoulder prostheses are commonly used to cover the different indications of shoulder joint replacements: classical modular, inverse and cup. The modular prosthesis can replace the glenoid as well as the humeral head components. Exact visualization of the interface between the glenoid component and bone (or cement-bone interface) is necessary to detect osteolysis, which is relatively common. Therefore, it is mandatory to make true AP and axial projections. When describing the humeral component, the interface between bone and implant must again be considered. Additionally, the pos

Thirteen patients with titanium fibermetal segmental bone/joint prostheses at a minimum follow-up period of six months were reviewed. Detailed radiographic data were analyzed quantitatively in terms of stem/bone interface radiolucency and bone bridging over the segmental portion of the prosthesis at predefined zones. Although stem radiolucent lines were common in this series, only one case had clinical loosening and required reoperation. There were two cases of stem fracture; one achieved extracortical bone union without revision, and the other case required implant removal and revision. Analy

This computational study investigates the effect of the Von Misses stresses and deformations distribution generated by coupling a customized … d implant and the finite element analysis demonstrated that the fixation and coupling system of the bone-implant interface guarantees adequate protection for the internal structures of the restored area. In addition, the custom-designed and placed implant will not cause non-physiological harm to the patient. Nor will failures occur in the anchoring system.

OBJECTIVE: The study employed three-dimensional (3D) finite element analysis (FEA) and examined how implant diameters affect stress distribution across the implant-bone contact and how stress transmission through this interface changes during axial and non-axial loading.
MATERIALS AND METHODS: A 3D mandibular model was created using cone beam CT of a patient with implants inserted into the first mandible molar. Nobel Biocare implants (Nobel Biocare, Switzerland) with specific dimensions of 3.5 mm, 4.3 mm, 5.0 mm, and 6.0 mm were chosen. Models were created in CATIAV5R19 (Dassault Systemes, Fr

PURPOSE: To gain insight into a new technology, a novel facet arthroplasty device (TFAS) was compared to a rigid posterior fixation system (UCR). The axial and bending loads through the implants and at the bone-implant interfaces were evaluated using an ex vivo biomechanical study and matched finite element analysis. Kinematic behaviour has been reported for TFAS, but implant loads have not. Implant loads are important indicators of an implant's performance and safety. The rigid posterior fixation system is used for comparison due to the extensive information available about these systems.

Magnesium (Mg)-based implants are promising for lamina defect reconstruction because of their biodegradability, mechanical compatibility, and bioactivity. However, in vivo degradation is usually assessed at the whole-implant level, and it remains unclear whether distinct regions of the same implant experience different degradation-repair couplings. In this study, a ZE21C Mg alloy artificial lamina was implanted in a New Zealand rabbit lumbar lamina defect reconstruction model to evaluate region-dependent degradation and local repair responses. Finite element analysis showed higher equivalent s
