Hypertensive disease in pregnancy in Botswana: Prevalence and impact on perinatal outcomes.

OBJECTIVES: Perinatal morbidity in sub-Saharan Africa has been attributed to infection, obstetric emergencies, and preterm birth, but less is known about hypertension in pregnancy. Our objective was to characterize the prevalence of hypertension in pregnancy and the impact of hypertension on perinatal outcomes in sub-Saharan Africa. STUDY DESIGN: We performed surveillance of obstetric records at eight of the largest public hospitals in Botswana. Women were included in this analysis if they were HIV-uninfected and had singleton gestations and at least one prenatal blood pressure measurement. …

Risk Factors for Measles in HIV-infected Children and Adolescents in Botswana.

We conducted a matched case-control study of 566 HIV-infected children in Botswana during a 2009-2010 measles outbreak to identify the risk factors for measles. Children in the oldest age quartile (≥13.1 years) were 4-fold more likely to acquire measles than those in the youngest quartile (<7.1 years). HIV-infected older children and adolescents may benefit from additional measles vaccination.

Naturally acquired anthrax antibodies in a cheetah (Acinonyx jubatus) in Botswana.

An outbreak of anthrax in the Jwana Game Reserve in Jwaneng, Botswana, was first observed when three cheetahs (Acinonyx jubatus) died of the disease in November 2004. In the aftermath of this event, banked serum samples collected from 23 wild-caught cheetahs were examined, by the inhibition enzyme-linked immunoassay (ELISA), for antibodies to the protective antigen (PA) of Bacillus anthracis. Of the 23 cheetahs, 16 regularly accessed the reserve. Antibodies to PA were detected in one cheetah collected in May 2004, indicating the disease was occurring well before it was first noticed. This appe…

CK8/18 and Claudin Profile of Female Breast Cancers from Botswana.

BACKGROUND: Globally, breast cancer mortality is rising particularly in transitioning countries where it is exacerbated by late diagnosis and poor diagnostic infrastructure. The dearth of data on the biology of breast tumours from under-studied populations compromise management of breast cancer patients resulting in poor patient outcomes. We conducted a study to determine the prevalence and impact of Claudin-low and CK8/18 in the Botswana female breast cancer cohort. DESIGN: This laboratory experimentally retrospective cross-sectional study was carried out on archived formalin-fixed paraffin …

Complete Genome Sequences of Three Staphylococcus pseudintermedius Strains Isolated from Botswana.

We report here the first whole-genome sequences for 3 strains of Staphylococcus pseudintermedius (112N, 113N, and 114N) isolated in Africa. Samples of this opportunistic pathogen were collected from nasal swabs obtained from healthy carrier dogs in Botswana. The sequence information will facilitate spatial phylogenetic comparisons of staphylococcal species and other bacteria at the genome level.

Neurocognitive impairment among HIV-positive individuals in Botswana: a pilot study.

BACKGROUND: The primary objective of this study was to determine the prevalence of neurocognitive impairment among HIV-positive individuals in Botswana, using the International HIV Dementia Scale (IHDS). We also compared performance on the IHDS with performance on tests of verbal learning/memory and processing speed, and investigated the association between performance on the IHDS and such variables as depression, age, level of education and CD4 count. METHODS: We conducted a cross-sectional study of 120 HIV-positive individuals randomly selected from an outpatient HIV clinic in Gaborone, Bot…

Tuberculosis attributed to transmission within healthcare facilities, Botswana-The Kopanyo Study.

OBJECTIVE: Healthcare facilities are a well-known high-risk environment for transmission of M. tuberculosis, the etiologic agent of tuberculosis (TB) disease. However, the link between M. tuberculosis transmission in healthcare facilities and its role in the general TB epidemic is unknown. We estimated the proportion of overall TB transmission in the general population attributable to healthcare facilities. METHODS: We combined data from a prospective, population-based molecular epidemiologic study with a universal electronic medical record (EMR) covering all healthcare facilities in Botswana…

We evaluated the prevalence of Rift Valley fever virus IgG and IgM in human serum samples (n = 1,276) collected in 2013-2014 in northern Botswana. Our findings provide evidence of active circulation of this virus in humans in the absence of clinical disease in this region.

SETTING: Botswana. OBJECTIVES: To estimate frequencies of tuberculosis (TB) treatment outcomes, assess the validity of reported treatment outcomes, and identify risk factors for death during TB treatment among children aged <15 years during 1998-2002. DESIGN: We examined TB treatment outcome frequencies using the national Electronic TB Registry (ETR) data. Treatment and medical records were reviewed to calculate predictive values (PV) for outcomes recorded in the ETR. We interviewed parents of children treated for TB and assessed risk factors for death during treatment via case-control study…

Baseline evaluation of routine HIV testing among tuberculosis patients in Botswana.

In January 2004, the government of Botswana introduced a policy of routine, non-compulsory human immunodeficiency virus (HIV) testing to increase testing and access to antiretroviral treatment (ART) for individuals presenting for medical treatment. Before a systematic implementation of the policy, we conducted a cross-sectional survey of tuberculosis (TB) record data from 46 clinics in 10 districts to assess baseline HIV testing rates among TB patients. Recorded HIV results from the facility TB register and TB treatment card were reviewed. Of the 1242 TB patients entered in the register, 47% h…

Long-term outcomes of children born with neural-tube defects in Botswana.

BACKGROUND: Major congenital abnormalities (CAs) disproportionately impact low and middle-income countries. Neural tube defects (NTDs) are among the most common major CAs in Botswana, but there has been no research to date on long-term outcomes of children with NTDs in Botswana. METHODS: The Tsepamo study conducted prospective systematic birth outcomes surveillance at 8-18 hospitals in Botswana from August 2014-May 2023, capturing data on ~ 70% of births in the country. Trained midwives performed surface examinations on all infants (liveborn and stillborn) and identified major CAs, including …

Perinatal depression among women in pregnancy and postpartum periods in Gaborone, Botswana.

INTRODUCTION: Perinatal depression is an important cause of maternal and infant morbidity. Rates of perinatal depression amongst pregnant and postpartum women in Botswana are not well-documented. We examined the prevalence of symptoms of depression and potential associated factors among pregnant and postpartum women participating in a sexually transmitted infection screening study. METHODS: We conducted a repeated time-point cross-sectional analysis of the Maduo study, a non-randomized clinic-controlled trial evaluating the benefit of antenatal Chlamydia trachomatis and Neisseria gonorrhoeae s…

Access to and Utilization of Endocrine Therapy Among Breast Cancer Survivors in Botswana.

PURPOSE: Breast cancer (BC) is the most common female cancer worldwide, and the burden is increasing across sub-Saharan Africa. For women with hormone receptor-positive (HR+) cancers, endocrine therapy (ET) taken for 5-10 years can reduce the risk of recurrence by half. We explored experiences with ET and barriers to utilization among survivors in Botswana. METHODS: We recruited women with nonmetastatic disease from a survivorship cohort who had undergone mastectomy within 1-5 years for semi-structured interviews to explore experiences with treatment. This thematic content analysis focused on…
