Brain changes: aerobic exercise for traumatic brain injury rehabilitation.

INTRODUCTION: Traumatic Brain Injury (TBI) accounts for millions of hospitalizations and deaths worldwide. Aerobic exercise is an easily implementable, non-pharmacological intervention to treat TBI, however, there are no clear guidelines for how to best implement aerobic exercise treatment for TBI survivors across age and injury severity. METHODS: We conducted a PRISMA-ScR to examine research on exercise interventions following TBI in children, youth and adults, spanning mild to severe TBI. Three electronic databases (PubMed, PsycInfo, and Web of Science) were searched systematically by two a…

Brain Shock-Toward Pathophysiologic Phenotyping in Traumatic Brain Injury.

Severe traumatic brain injury (TBI) is a heterogeneous pathophysiologic entity where multiple interacting mechanisms are operating. This viewpoint offers an emerging, clinically actionable understanding of the pathophysiologic heterogeneity and phenotypic diversity that comprise secondary brain injury based on multimodality neuromonitoring data. This pathophysiologic specification has direct implications for diagnostic, monitoring, and therapeutic planning. Cerebral shock can be helpfully subanalyzed into categories via an examination of the different types of brain tissue hypoxia and substrat…

Distinct Brain Regions in Physiological and Pathological Brain Aging.

BACKGROUND: Studying structural brain aging is important to understand age-related pathologies, as well as to identify the early manifestations of the Alzheimer's disease (AD) continuum. In this study, we investigated the long-term trajectory of physiological and pathological brain aging in a large number of participants ranging from the 50s to over 80 years of age. OBJECTIVE: To explore the distinct brain regions that distinguish pathological brain aging from physiological brain aging using sophisticated measurements of cortical thickness. METHODS: A total of 2,823 cognitively normal (CN) i…

Brain-immune interactions in perinatal hypoxic-ischemic brain injury.

Perinatal hypoxia-ischemia remains the primary cause of acute neonatal brain injury, leading to a high mortality rate and long-term neurological deficits, such as behavioral, social, attentional, cognitive and functional motor deficits. An ever-increasing body of evidence shows that the immune response to acute cerebral hypoxia-ischemia is a major contributor to the pathophysiology of neonatal brain injury. Hypoxia-ischemia provokes an intravascular inflammatory cascade that is further augmented by the activation of resident immune cells and the cerebral infiltration of peripheral immune cells…

Deep Brain Stimulation Influences Brain Structure in Alzheimer's Disease.

BACKGROUND: Deep Brain Stimulation (DBS) is thought to improve the symptoms of selected neurological disorders by modulating activity within dysfunctional brain circuits. To date, there is no evidence that DBS counteracts progressive neurodegeneration in any particular disorder. OBJECTIVE/HYPOTHESIS: We hypothesized that DBS applied to the fornix in patients with Alzheimer's Disease (AD) could have an effect on brain structure. METHODS: In six AD patients receiving fornix DBS, we used structural MRI to assess one-year change in hippocampal, fornix, and mammillary body volume. We also used de…

Robust whole-brain segmentation: application to traumatic brain injury.

We propose a framework for the robust and fully-automatic segmentation of magnetic resonance (MR) brain images called "Multi-Atlas Label Propagation with Expectation-Maximisation based refinement" (MALP-EM). The presented approach is based on a robust registration approach (MAPER), highly performant label fusion (joint label fusion) and intensity-based label refinement using EM. We further adapt this framework to be applicable for the segmentation of brain images with gross changes in anatomy. We propose to account for consistent registration errors by relaxing anatomical priors obtained by mu…

Brain neoplasms with development similar to transient brain ischaemia.

Diagnostics of brain tumours still remains a serious problem in neurologist's career. Difficulties in proper diagnosing refer mainly to endocranial tumours with atypical clinical course. Frequency of brain tumours' vascular manifestation is assessed as 1-4%, and in most cases it is the kind of manifestation which corresponds with intracerebral bleedings and more seldom with ischaemic strokes. Reports on brain tumours with developments similar to transient brain ischaemia (1, 2, 3, 4) are published occasionally. In Neurology Clinic of the Medical Academy in Lublin, we conducted observation of t…

Malignant brain stem hyperthermia caused by brain stem hemorrhage.

BACKGROUND: We report two cases of brain stem hemorrhage characterized by severe hyperthermia, rhabdomyolysis, acute renal failure, and a rapidly fatal course. METHODS: A 55-year-old man and a 65-year-old man were admitted with coma-producing brain stem hemorrhage accompanied by hyperthermia. Both underwent ventricular drainage. RESULTS: Within 1 day of onset, both patients developed hyperthermia of over 41 degrees C, increased serum creatine phosphokinase (CPK) level indicating rhabdomyolysis, and acute renal failure. One patient died on the second day and the other on the third day after o…

Aspects of blood-brain barrier in clinical brain imaging.

The blood-brain barrier (BBB) is selectively permeable and shows a wide range of permeability to various substances in health and becomes generally permeable to all small molecules in almost all disease states. The capillary bed in brain (the BBB) is uniquely impermeable to many polar substances. The mechanism by which the capillary bed is maintained in this state is unknown, but may be an astrocytic function. In most major neuropathology, the BBB is lost because these unique structural features creating the BBB come to resemble the permeable capillaries in other tissues. Early isotopic brain …

Brain Protective Ventilation Strategies in Severe Acute Brain Injury.

PURPOSE OF THE REVIEW: This narrative review synthesizes ten key evidence-based principles for optimizing ventilatory management in patients with acute brain injury, including traumatic brain injury, stroke, and hypoxic-ischemic encephalopathy. RECENT FINDINGS: We emphasize the need to individualize ventilator settings to preserve intracranial pressure (ICP) and cerebral perfusion pressure (CPP), while maintaining lung-protective strategies. Key recommendations include prioritizing physiological targets over ventilator modes, judicious use of positive end-expiratory pressure (PEEP) with concu…

Skull-meninges-brain connectivity and extra-axial brain tumours.

The cortex of the brain is covered by three meningeal layers: the dura, the arachnoid, and the pia mater. Substantial discoveries have been made demonstrating the structural and functional relationships between these layers, and with other neighbouring structures such as the skull. Importantly, improved understanding of the meningeal lymphatic network places the meninges at the nexus of a cross talk between the brain, peripheral immune system, and the skull bone marrow. The meningeal lymphatic network has been shown to regulate immune responses in models of health and disease states, such as i…

The airway-brain axis: Connecting breath, brain, and behavior.

The neural control of breathing is both dynamic and essential, ensuring life-sustaining gas exchange while protecting the respiratory system from harm. Peripheral neurons innervating the respiratory tract exhibit remarkable diversity, continuously relaying sensory feedback to the brain to regulate breathing, trigger protective reflexes such as coughing and sickness behaviors, and even influence emotional states. Understanding this airway-brain axis is especially critical given the increasing global burden of respiratory diseases, as it holds implications for both human health and broader brain…

Brain endothelial CD200 signaling protects brain against ischemic damage.

Источник пометил публикацию как отозванную

Ischemic stroke induced inflammatory responses contribute significantly to neuronal damage and stroke outcomes. CD200 ligand and its receptor, CD200R, constitute an endogenous inhibitory signaling that is being increasingly recognized in studies of neuroinflammation in various central nervous system disorders. CD200 is a type 1 membrane glycoprotein that is broadly expressed by endothelia and neurons in the brain. In the present study, we have examined the role of endothelial CD200 signaling in acute ischemic stroke. Endothelial CD200 conditional knock out (CKO) mice were generated by breeding…

Pediatric Traumatic Brain Injury: Impact on the Developing Brain.

Traumatic brain injury (TBI) is a serious public health concern impacting millions of children and adolescents each year. Experiencing a brain injury during key critical periods of brain development can affect the normal formation of brain networks that are responsible for a range of complex neurocognitive outcomes. In addition, there are multiple pre- and postinjury factors that influence the trajectory of recovery and outcomes. In this review, we will focus on the current state of the literature within pediatric TBI; systematically review the available research on developmental aspects of TB…

The Molecular Gut-Brain Axis in Early Brain Development.

Millions of nerves, immune factors, and hormones in the circulatory system connect the gut and the brain. In bidirectional communication, the gut microbiota play a crucial role in the gut-brain axis (GBA), wherein microbial metabolites of the gut microbiota regulate intestinal homeostasis, thereby influencing brain activity. Dynamic changes are observed in gut microbiota as well as during brain development. Altering the gut microbiota could serve as a therapeutic target for treating abnormalities associated with brain development. Neurophysiological development and immune regulatory disorders …

Brain Activity after Intermittent Hypoxic Brain Condition in Rats.

Hypoxic brain injury is accompanied by a decrease in various functions. It is also known that obstructive sleep apnea (OSA) can cause hypoxic brain injury. This study aimed to produce a model of an intermittent hypoxic brain condition in rats and determine the activity of the brain according to the duration of hypoxic exposure. Forty male Sprague-Dawley rats were divided into four groups: the control group (n = 10), the 2 h per day hypoxia exposure group (n = 10), the 4 h per day hypoxia exposure group (n = 10), and the 8 h per day hypoxia exposure group (n = 10). All rats were exposed to a hy…

Functional role of brain-engrafted macrophages against brain injuries.

BACKGROUND: Brain-resident microglia have a distinct origin compared to macrophages in other organs. Under physiological conditions, microglia are maintained by self-renewal from the local pool, independent of hematopoietic progenitors. Pharmacological depletion of microglia during whole-brain radiotherapy prevents synaptic loss and long-term recognition memory deficits. However, the origin or repopulated cells and the mechanisms behind these protective effects are unknown. METHODS: CD45low/int/CD11b+ cells from naïve brains, irradiated brains, PLX5622-treated brains and PLX5622 + whole-brain…
