Ipsilateral hypohidrosis in brain stem infarction.

BACKGROUND AND PURPOSE: The brain stem is the most important autonomic processing center, but very little attention has been given to clinical manifestations of autonomic failure in brain stem stroke. Our purpose was to evaluate the prevalence, characteristics, and prognostic significance of sweating dysfunction in brain stem infarctions. METHODS: We carried out a prospective study using quantitative evaporimetry to investigate spontaneous and heat-stimulated sweating in 18 healthy control subjects and 18 patients with ischemic brain stem stroke in the acute phase and at 1 and 6 months after …

Auditory brain-stem responses in blepharospasm.

The auditory brain-stem response (ABR) has been reported to detect abnormalities in both the auditory pathways and in adjacent structures. Ten of 35 consecutive patients with blepharospasm were found to have abnormal ABRs involving poor form and delayed peak latency of positive components III or V. Abnormal ABRs in approximately 30% of patients with essential blepharospasm suggest pathology in the brain-stem of a substantial proportion of patients with this form of cranial-cervical dystonia.

[Surgical removal of brain stem hematoma].

Of 5 cases of brain stem hematoma, 4 males and 1 female, 4 had hematoma in the pons, and 1 in the midbrain. The diagnosis was confirmed by CT. Two cases had received craniotomy. CT-guided stereotactic evacuation was performed in other 3 cases. All the cases had satisfactory results after operation. None of them died or had any severe complication. Three cases recovered and resumed work, one had mild disability and recovered well in the early time, but died of pneumonia two months later. We stressed that the method of evacuation for the treatment of hematoma is simple and convenient, and the wo…

Brain stem anesthesia after retrobulbar block.

Presented is a series of eight patients in whom neurologic sequelae developed after retrobulbar anesthesia. All patients demonstrated blockade of one or more cranial nerves and six progressed to apnea, requiring intubation and mechanical ventilation. Neurologic findings included amaurosis in the contralateral eye (5 patients), nonreactive pupil in the contralateral eye (6 patients), ductional defects (2 patients), and dysphagia (4 patients). In all cases, these findings resolved in 2 to 12 hours. In patients who progressed to apnea, spontaneous respiration resumed within 30 to 60 minutes. Thes

Mania resulting from brain stem tumor.

Mania usually represents one extreme of recurrent affective illness in patients with a genetic predisposition. Mania or hypomania is not generally known to signal the presence of an occult neoplasm; however, this possibility should be considered. The authors describe a 55-year-old man without personal or family history of affective disease whose manic behavior precipitated diagnosis of a metastatic mesencephalic tumor.

Secondary brain stem hemorrhage in stroke.

The occurrence of secondary brain stem hemorrhage was studied in 435 autopsies from patients with recent cerebral hemorrhage, infarction or ruptured cerebral aneurysms. The frequency of secondary brain stem hemorrhage was found to be 45% in cerebral hemorrhage, 15% in cerebral infarction, and 36% in ruptured aneurysms. In the majority of cases the secondary brain stem hemorrhage occurred a few days after the onset of cerebral hemorrhage or infarction. Ruptured aneurysms showed a more widespread temporal distribution of secondary brain stem hemorrhage. The median survival time was 2 days in cas…

A functional measure of brain activity: brain stem transmission time.

Surface-recorded auditory nerve and brain stem responses are being used routinely for diagnostic purposes in man. When interest is in auditory diagnosis, the electric response threshold is of primary importance. However, when used in neurological diagnosis, the wave form of the response is important. As a measure of one aspect of response wave form, this paper suggests the use of brain stem transmission time (BTT), defined as the time interval between the first earlobe-negative wave (response of the auditory nerve--the 'input' to the brain stem) and the earlobe-positive wave from the region of…

Dynamic axial brain stem distortion as a mechanism in the production of brain stem hemorrhages: role of the carotid arteries.

Dynamic axial distortion of the brain stem has long been recognized as occurring in the presence of supratentorial mass lesions. Although brain stem hemorrhages occur under these circumstances, the pathophysiology of these hemorrhages is still a matter of controversy. In the present study, the role of the carotid arteries in tethering the brain stem was evaluated during dynamic axial distortion of the neuraxis of the dog. Brain stem hemorrhages occurred in 32 of 48 dogs in which the carotid arteries were left intact (67%) and in only 9 of 46 animals in which the carotid arteries were sectioned…

Effects of blood hyperviscosity on functional integrity in the brain stem: a brain stem evoked auditory potential study.

Clinical hemorheological and brain stem auditory evoked potential (BAEP) investigations were performed in patients with ischaemic brain stem stroke. Lesions were verified by MRI. 55 healthy persons with negative rheological and BAEP findings were used as controls (group A/1). 33 stroke patients with negative rheological parameters (group A/2) and 34 patients with hyperviscosity (group A/3) were also enrolled. In group A/3 bilateral pathological BAEP patterns were found, that could be explained by microcirculatory disturbances. 36 persons with verified blood hyperviscosity, but without neurolog…

Evaluation of the brain-stem function by the auditory brain-stem response and the caloric vestibular reaction in comatose patient.

The auditory brain-stem response (ABR) and the caloric vestibular reaction (CVR) were investigated in 100 patients in deep coma to evaluate the brain-stem function of these patients precisely and to predict their prognosis accurately. In the first ABR examination, 54 patients showed normal and 46 abnormal ABRs. Among the latter, five showed partial wave disappearance and 21 exhibited no ABRs. Twenty-six of the 30 patients who recovered (87%) showed normal ABRs and 21 of the 57 who died (37%) also exhibited normal latency at that time. On the other hand, all the patients who showed wave disappe

Dorsal brain stem syndrome: MR imaging location of brain stem tegmental lesions in neonates with oral motor dysfunction.

The anatomic extent of brain stem damage may provide information about clinical outcome and prognosis in children with hypoxic-ischemic encephalopathy and oral motor dysfunction. The aim of this study was to retrospectively characterize the location and extent of brain stem lesions in children with oral motor dysfunction. From January 2005 to August 2009, 43 infants hospitalized at our institution were included in the study because of a history of hypoxic-ischemic events. Of this group, 14 patients showed oral motor dysfunction and brain stem tegmental lesions detected at MR imaging. MR imagin…
