Steroid biochemistry and categorization of breast cyst fluid: relation to breast cancer risk.

Patients bearing macrocysts of the breast are at higher risk of later developing cancer. The fluid filling the cysts (breast cysts fluid, BCF) contains unusual amounts of steroid conjugates, first androgen and estrogen sulfates. Measuring BCF cations (K+,Na+) allows categorization of cysts into two major subsets (type I and type II) that are associated with a different degree and/or turnover of apocrine metaplastic cells in the lining epithelium. Type I cysts (high K+/Na+ ratio) accumulate hugh amounts of dehydroepiandrosterone sulfate, estrone sulfate, androstane-3 alpha,17 beta-diol glucuron…

Apocrine cells in breast cyst fluid and their relationship to cyst type: a morphometric study.

The morphology of apocrine cells exfoliated in breast cyst fluid (BCF) was studied in 78 BCF samples obtained from 39 premenopausal patients with gross cystic disease who were bearing two simultaneously aspirated cysts. 57/78 samples showed cell clusters suitable for computer-assisted cytometry. This was performed on 5820 cells using a Leitz Texture Analysis System (TAS). We measured the surface areas of cytoplasm, nucleus and nucleolus; we also calculated the nuclear/cytoplasmic (N/C), nuclear/nucleolar (N/n) ratios and the nuclear roundness factor (RF). Cysts were divided according to the ca…

Epidermal inclusion cyst of the breast: A case report.

Epidermal inclusion cysts are nonmalignant proliferations of squamous epithelium situated within the dermis. Their presence in the breast is uncommon and presents considerable diagnostic difficulty. This case report aims to elucidate the multimodal imaging findings and differential diagnosis of a rare breast epidermal inclusion cyst, highlighting its capacity to mimic malignant neoplasms. We present a case of a 38-year-old female with a palpable right breast nodule that had progressively increased over a period of 15 years. A clinical examination identified a firm mass in the upper inner quadr

A rare case of isolated hydatid cyst of breast.

INTRODUCTION: Hydatid cyst of the breast is very rare. It is challenging to differentiate it from other tumoral lesions of the breast. Only few reports of breast hydatid cyst are published and majority of the reported cases have been diagnosed postoperatively as it is not possible to reach definitive diagnosis with clinical examination and radiological investigations only. PRESENTATION OF CASE: A 31-year old woman presented with a painless lump in the right breast since one year duration. On clinical examination, a non-mobile, firm lump was detected in the right breast associated with nipple

Alpha 1-antichymotrypsin complexes in human breast cyst fluids.

Alpha 1-antichymotrypsin, a serum protease inhibitor, was found in 72 breast cyst fluids aspirated from women affected by gross cystic breast disease. When fractionated by gel chromatography, the presence of protein complexes or aggregates was demonstrated. A different distribution of the alpha 1-antichymotrypsin appeared to be related to the ionic composition of the breast cyst fluid; when compared with metabolically active apocrine cysts, a statistically significant increase of alpha 1 protease inhibitor values in flattened epithelial cysts was revealed (P < 0.001). Two-dimensional immunoele…

Risk of breast cancer in women with palpable breast cysts: a prospective study. Edinburgh Breast Group.

BACKGROUND: 7% of women in the western world develop palpable breast cysts. Studies of the relation between cysts and breast cancer have conflicting results. There are two clearly defined types of cyst. We investigated whether one cyst type is associated with a higher rate of breast-cancer development than the other. METHODS: We studied 1374 women with palpable breast cysts presenting between 1981 and 1987, who had cysts aspirated between 1981 and 1989. Cysts were classified as type I if the sodium/potassium (Na+/K+) ratio in the cyst fluid was less than 3, or type II if the Na+/K+ ratio was …

Ectopic breast carcinoma presenting as sebaceous cyst left axilla.

A 43-year-old woman with a positive family history of breast cancer presented with a painless lump in her left axilla for 2 years. Clinical diagnosis was a left axillary sebaceous cyst as the lump was inseparable from the skin. The lesion was excised under local anaesthesia and reported as breast tissue widely infiltrated by an invasive ductal carcinoma (grade 2). The malignancy was not involving the epidermis but <1 mm away from deeper margins. Re-excision of the deeper tissue with an axillary sentinel lymph nodes biopsy was performed and deep margins were reported to be tumour-free with no n

Allopregnanolone, pregnenolone sulfate, and epitestosterone in breast cyst fluid.

The risk of breast cancer is 2 to 5 times higher in patients suffering from gross cystic disease. Breast cysts are categorized into two groups (type I and type II) according to the concentration of electrolytes in the cyst fluid. The two types also differ with respect to accumulation of steroids and steroidogenic enzyme activity. In type I cysts a higher risk of breast carcinoma could be expected. Here, we studied a possible relationship between the type of cyst and levels of epitestosterone (an endogenous antiandrogen), allopregnanolone (a product of 5alpha-reductase activity), and pregnenolo…

The biochemistry of breast cyst fluids and duct secretions.

The ratio of potassium to sodium concentrations in breast fluids has led other investigators to the subclassification of cysts into two types: 1) apocrine (secretory) cysts with high potassium and low sodium, and 2) attenuated (flattened) cell cysts with low potassium and high sodium content. Apocrine cells are thought by some to actively secrete potassium. Cell typing is considered important as apocrine cysts are more likely to be bilateral, multiple, recurrent, and serve as markers for epithelial cell atypia. A retrospective study of the biochemical analyses of 58 cyst fluids and 28 duct sec

Levels of human chorionic gonadotropin in breast cyst fluid.

The results of dosage of human chorionic gonadotropin (HCG) in 75 breast cyst fluids taken from 61 patients suffering from breast gross cystic disease (BGCD), 13 of which had multiple mono and/or bilateral cysts are discussed. The corresponding sera were also examined. Assays were carried out using the ELISA method. For comparison, HCG levels were also determined in 21 breast cyst fluids using RIA method. In 66.7% of the breast cyst fluids examined the hormone levels were higher than normal serum values. Corresponding sera showed HCG to be within the range of normal levels. In 5 patients the m…

Epidermal growth factor levels in human breast cyst fluid.

Epidermal growth factor (EGF) seems to modulate the in vitro and in vivo growth of normal and neoplastic breast cells. We determined, by a radio-receptor assay, EGF levels in cyst fluid and in plasma of patients with gross cystic disease of the breast. The mean levels of EGF were lower in plasma than in breast cyst fluid (BCF) (p less than 0.001). In BCF of apocrine cysts we found higher EGF levels than in flattened cysts (p less than 0.001). The EGF content of apocrine BCF seems to be under sex steroid hormone control, being higher in reproductive age than in post menopause (p less than 0.05)…

Oestradiol and epidermal growth factor in breast cyst fluid.

Gross cystic breast disease may be categorised into 2 groups according to the intracystic cation ratios (Na+/K+ less than 3 and Na+/K+ greater than 3). It has been suggested that women in the low electrolyte ratio group may be at higher risk of developing breast cancer. Epidermal growth factor concentrations in breast cyst fluid were significantly higher in the low than in the high electrolyte ratio group but there was no significant difference in oestradiol concentrations between the two groups of breast cysts. A negative correlation was obtained between intracystic concentrations of epiderma…

Progesterone-binding cyst protein in human breast tumor cytosol.

Breast tumor cytosol has been analyzed for the presence of a progesterone-binding protein (PBCP), commonly present in benign breast cysts in huge concentrations. In 377 primary carcinoma investigated PBCP was present in measurable quantities in 60.7% using "rocket" immunoelectrophoresis. Concentrations of PBCP ranged from 0 to 12.4% of total cytosol protein with an average of 4.0 micrograms/mg cytosol protein. The distribution of PBCP values seems to suggest two tumor populations, one of which is lacking in PBCP and the other showing a lognormal distribution. In malignant tumors PBCP levels we…

[Directed ultrasonography in the diagnosis of abscesses and cysts of the breast].

Based on 1,301 examinations the author refers to the contribution of aimed ultrasonography in the diagnosis of abscesses and cysts of the breast. The aimed ultrasonography is a simple, fast and exceptionally sensitive and reliable method of examination in their detection and differentiation from solid homomorphcus processes depicted on mammograms. Its importance grows in the examination of fibroglandular tissue which cannot be differentiated on the mammogram. It reduces or completely suppresses invasive methods of examination, enables aimed diagnostic and therapeutic puncture and enables a lon
