Consumption of ultraprocessed foods and breast density in adolescence.

Breast density (BD) is one of the major risk factors for breast cancer (BC) and has the potential to be used as an early marker. Puberty is a critical period for carcinogenesis, and identifying events that modify BD during this stage could aid in understanding future BC risk. The consumption of ultra-processed foods (UPFs) has been associated with BC; however, evidence of their effect on BD, particularly in adolescents, is lacking. To assess the association between the intake of UPFs during the peripubertal period and BD measured at 4 years after menarche onset, absolute fibroglandular volume …

The Relationship of Breast Density and Positive Lumpectomy Margins.

BACKGROUND: A positive lumpectomy margin after breast-conserving surgery (BCS) is a significant predictor for ipsilateral cancer recurrence. The MarginProbe, a Food and Drug Administration (FDA)-approved device for intraoperative assessment of lumpectomy margins, is associated with a reduction in re-excision surgery. This study aimed to evaluate the relationship of mammographic breast density (MBD) and clinicopathologic characteristics with margin status in women undergoing BCS with the MarginProbe. METHODS: The institutional database was queried for patients with breast cancer who had BCS wi…

Breast Density Knowledge and Awareness: A Review of Literature.

OBJECTIVES: We reviewed the literature on breast density knowledge and breast density awareness to explore what challenges are faced by this area of research. METHOD: A review of PubMED, PsycINFO, and CINAHL databases was performed. Studies were published in peer-reviewed journals (in all years available) and written in English. The broad search terms used were ["breast density"] AND ["knowledge" OR "awareness"]. Eligible articles were included in the final analysis after meeting the following inclusion criteria: (1) the records had to quantitatively examine and report breast density knowledg…

The influence of mammographic density on breast tumor characteristics.

Mammographic density (MD) is a well-established risk factor for breast cancer. Whether MD influences the tumor phenotype remains to be clarified. Previous studies are highly inconsistent and most lack important covariate information. This is a case-only study within a population-based case-control study. Cases were all postmenopausal women, aged 50-74 years, with incident breast cancer, diagnosed 1993-1995, and with no history of previous cancer (n = 2,720). 1,747 women with mammograms and information on tumor characteristics were included in analyses. MD was assessed using a computer-assisted

Family History of Breast Cancer, Breast Density, and Breast Cancer Risk in a U.S. Breast Cancer Screening Population.

Background: The utility of incorporating detailed family history into breast cancer risk prediction hinges on its independent contribution to breast cancer risk. We evaluated associations between detailed family history and breast cancer risk while accounting for breast density. Methods: We followed 222,019 participants ages 35 to 74 in the Breast Cancer Surveillance Consortium, of whom 2,456 developed invasive breast cancer. We calculated standardized breast cancer risks within joint strata of breast density and simple (1st-degree female relative) or detailed (first-degree, second-degree, or f…

Impact of breast density on re-excision and recurrence of breast cancer following breast conserving surgery.

BACKGROUND: Mammographic breast density (BD) has garnered clinical interest due to its impact on breast cancer (BC) risk, sensitivity of mammographic screening, and tumour size at presentation. There is conflicting evidence as to BD's effect on re-excisions and recurrence following breast conserving surgery (BCS). We hypothesize that increasing BD may obscure disease, resulting in an underestimation of the extent of disease, and a greater risk of re-excision and local recurrence following BCS for invasive BC. METHODS: A retrospective, population-based review of patients undergoing BCS for inv…

Temporal changes in mammographic breast density and breast cancer risk among women with benign breast disease.

INTRODUCTION: Benign breast disease (BBD) and high mammographic breast density (MBD) are prevalent and independent risk factors for invasive breast cancer. It has been suggested that temporal changes in MBD may impact future invasive breast cancer risk, but this has not been studied among women with BBD. METHODS: We undertook a nested case-control study within a cohort of 15,395 women with BBD in Kaiser Permanente Northwest (KPNW; 1970-2012, followed through mid-2015). Cases (n = 261) developed invasive breast cancer > 1 year after BBD diagnosis, whereas controls (n = 249) did not have breast…

Breast density does not affect breast cancer tumor size assessment: A comparison of radiologic versus pathologic measurement by different imaging modalities across breast densities.

BACKGROUND: Tumor size is an important parameter in breast cancer staging. Definitive tumor size is determined by measurement of the pathologic specimen. However, prior to surgery, size must be assessed by imaging with mammography (MMG), ultrasound (US), or magnetic resonance imaging (MRI). Discrepancies between imaging-assessed and pathologic size are not uncommon. Breast density decreases the sensitivity of MMG, and may affect image-based tumor size assessment. AIM: To compare tumor size assessed by the different imaging modalities to pathologic size across breast densities. MATERIAL & MET…

Mammographic radiomics and breast density for predicting PD-L1 expression in breast cancer.

BACKGROUND: Programmed death-ligand 1 (PD-L1) expression is a critical biomarker for guiding immunotherapy in breast cancer, particularly in triple-negative subtypes. However, conventional assessments rely on invasive biopsies and are limited by tumor heterogeneity. This study aims to develop a non-invasive approach for predicting PD-L1 expression using mammography-based radiomics features integrated with clinicopathological variables and breast density, and to evaluate its performance in both an internal development cohort and an independent external validation cohort. METHODS: A total of 121…

Canadian Society Breast Imaging Position Statement on Mammographic Breast Density and Supplemental Screening.

Screening aims to detect breast cancer before it becomes clinically apparent, enabling identification of tumors when they are smaller and have not yet spread and when treatment options are more effective, less invasive, and more affordable. However, screening mammography has known limitations, with breast density being a primary challenge. Denser breast tissue not only increases the likelihood of cancer but also makes tumors harder to detect due to overlapping tissue. Strong evidence now exists to support updating our previous guidelines to recommend supplemental screening beyond mammography f…

Impact of adding breast density to breast cancer risk models: A systematic review.

PURPOSE: Assessment of a woman's risk of breast cancer is essential when moving towards personalized screening. Breast density is a well-known risk factor and has the potential to improve accuracy of risk prediction models. In this study we reviewed the impact on model performance of adding breast density to clinical breast cancer risk prediction models. METHODS: We conducted a systematic review using a pre-specified search strategy for PubMed, EMBASE, Web of Science, and Cochrane Library from January 2007 until November 2019. Studies were screened using the Covidence software. Eligible studi…

Independent association of lobular involution and mammographic breast density with breast cancer risk.

BACKGROUND: Lobular involution, or age-related atrophy of breast lobules, is inversely associated with breast cancer risk, and mammographic breast density (MBD) is positively associated with breast cancer risk. METHODS: To evaluate whether lobular involution and MBD are independently associated with breast cancer risk in women with benign breast disease, we performed a nested cohort study among women (n = 2666) with benign breast disease diagnosed at Mayo Clinic between January 1, 1985, and December 31, 1991 and a mammogram available within 6 months of the diagnosis. Women were followed up fo…

Automated Breast Density Assessment for Full-Field Digital Mammography and Digital Breast Tomosynthesis.

Mammographic density is a strong risk factor for breast cancer and is reported clinically as part of Breast Imaging Reporting and Data System (BI-RADS) results issued by radiologists. Automated assessment of density is needed that can be used for both full-field digital mammography (FFDM) and digital breast tomosynthesis (DBT) as both types of exams are acquired in standard clinical practice. We trained a deep learning model to automate the estimation of BI-RADS density from a prospective Washington University clinic-based cohort of 9,714 women, entering into the cohort in 2013 with follow-up

Association of mammographic breast density measurements and hormone receptor status of breast cancer.

PURPOSE: Breast cancer is the most frequent cancer in women, with significant mortality. Mammography is a routine investigation for breast disease. A known risk factor for breast cancer is increased breast density. Here, we tried to observe if mammographic density also affects the hormone receptor status of breast cancer, which will help in the understanding of the biological mechanisms of breast cancer development. MATERIAL AND METHODS: Suspected breast cancer patients at Lok Nayak Hospital, Delhi, underwent mammography in the Department of Radiodiagnosis. The density of breast contralateral…

Long-term excess risk of breast cancer after a single breast density measurement.

AIM: Breast density is a risk factor for breast cancer. As density changes across a woman's life span, we studied for how long a single density measurement taken in (post-)menopausal women remains informative. METHODS: We used data from Singaporean women who underwent a single mammography screen at age 50-64 years. For each case with breast cancer diagnosed at screening or in the subsequent 10 years, whether screen detected or diagnosed following symptoms, two age-matched controls were selected. We studied the excess risk of breast cancer, calculated as an odds ratio (OR) with conditional log…

Prognostic Influence of Preoperative Mammographic Breast Density in Operable Invasive Female Breast Cancer.

We aimed to investigate the potential of preoperative mammographic breast density (MBD) as a prognostic factor in breast cancer. Data of 969 patients with primary breast cancer were analyzed. We defined low MBD as fatty or fibroglandular breast, and high MBD as heterogeneously dense or extremely dense breast, respectively. The high MBD group demonstrated a superior overall survival rate compared to the low MBD group (p < 0.001). Favorable prognostic effects of high MBD were observed in subgroups aged >50 years (p < 0.001) and with positive hormone receptor (HRc) and negative human epidermal gr…

A prospective study of breast cancer risk using routine mammographic breast density measurements.

Mammographic breast density is a major risk factor for breast cancer but estimates of the relative risk associated with differing density patterns have varied widely. It is also unclear how menopausal status influences this association and to what extent the effects of density are due to its correlation with other risk factors. Most recent investigations of breast density have been case-control studies, which provide indirect estimates of relative risk. We have prospectively followed 61,844 women for an average of 3.1 years to directly estimate risk among women in the four mammographic breast …
