Antiplatelet Therapy Bridging With Cangrelor in Patients With Coronary Stents: A Case Series.

BACKGROUND: Cangrelor is an intravenous P2Y12 receptor antagonist approved for use during percutaneous coronary intervention (PCI) to reduce ischemic events associated with new stent placement and has been used off-label at reduced doses guided by platelet function testing as a "bridge" from discontinuation of oral P2Y12 receptor antagonists to surgical procedures when the long-term effects of oral agents are undesirable. OBJECTIVE: To describe the dosing, laboratory monitoring, and clinical outcomes of a series of patients who received cangrelor as a "bridging" antiplatelet agent. METHODS:

Postcardiotomy veno-arterial membrane oxygenation as a bridge to heart replacement therapies.

BACKGROUND: Data on the outcome of veno-arterial extracorporeal membrane oxygenation (V-A-ECMO) support as a bridge to replacement therapies are scarce. OBJECTIVES: We investigated the outcome of V-A-ECMO support after adult cardiac surgery as a bridge to ventricular assist device (VAD) implantation and/or heart transplantation. METHODS: This is a retrospective, multicenter study recruiting patients who underwent heart transplantation or VAD implantation immediately after postcardiotomy V-A-ECMO for persistent heart failure at 14 European centers of cardiac transplantation from 2010 to 2024.

Pharmacological management of childhood constipation: Bridging today's gaps with tomorrow's therapies.

Childhood constipation is a prevalent yet frequently underestimated global health concern, affecting approximately 10% of children across diverse populations. Its impact extends well beyond bowel-related symptoms, with affected children often experiencing extraintestinal manifestations such as pain in multiple sites, behavioral disturbances, and a markedly diminished quality of life. The burden is further compounded in low- and middle-income countries, where financial and logistical constraints limit access to effective treatment. The impact is intensified in developing countries due to limite

Complement 3 Glomerulopathy Revisited: Bridging Pathogenesis and Therapy in the New Era.

Complement 3 glomerulopathy is a rare and highly heterogeneous complement-mediated kidney disease characterized by the aberrant deposition of complement components in the glomeruli, triggering inflammation and contributing to progressive kidney function impairment. Despite recent advancements in understanding complement 3 glomerulopathy pathophysiology, therapeutic options remain limited. Current treatments often fail to prevent poor outcomes, with 30-50% of adult patients progressing to kidney failure and high rates of disease recurrence following kidney transplantation. Genetic studies have

Microcurrent Therapy in Cardiac Repair: Bridging Mechanistic Biology and Clinical Evidence.

Cardiovascular disease remains a major cause of morbidity and mortality, with heart failure representing the common and often irreversible end stage, for which current therapies are largely palliative rather than restorative. Cardiac microcurrent therapy delivers a low-intensity, nondepolarizing direct current across the myocardium and has been proposed to modulate fibrosis, inflammation, and remodeling in heart failure with reduced ejection fraction. In Cardiac Microcurrent I (New York Heart Association III nonischemic heart failure with reduced ejection fraction), left ventricular ejection f

Bridging the resolution gap in alpha therapy dosimetry: a space for quantitative MRI?

While external beam radiotherapy relies heavily on pre-treatment imaging for advanced treatment planning and radiation dosimetry, tools for predicting local dose delivery in systemic radiopharmaceutical therapies have generally lagged behind. Furthermore, targeted alpha particle-emitting radiopharmaceuticals, with their uniquely short range and high-energy dose deposition, require specialized dosimetry methods at the micro- and mesoscale. Magnetic resonance imaging methods may represent the missing link between standard diagnostic tumor imaging and personalized radionuclide treatment planning

Hypertension and Atrial Fibrillation: Bridging the Gap Between Mechanisms, Risk, and Therapy.

Background and objectives: Atrial fibrillation (AF), the most prevalent sustained arrhythmia, poses a significant public health challenge due to its links with stroke, heart failure, and mortality. Hypertension, a primary modifiable cardiovascular risk factor, is a well-established risk factor for AF that facilitates structural and electrical changes in the atria, including dilation, fibrosis, and pressure overload. Material and Methods: we conducted a literature search regarding the shared mechanisms, risks and treatments of hypertension and atrial fibrillation. Results: The renin-angiotensin

Bridging Inflammation and Repair: The Promise of MFG-E8 in Ischemic Stroke Therapy.

Ischemic stroke is a neurological disorder resulting from localized brain injury due to focal cerebral ischemia, typically caused by the blockage of one or, in some cases, a few cerebral arteries. This arterial obstruction leads to hypoxia and energy failure, culminating in primary brain damage. Although reperfusion is critical to salvage viable tissue, it often intensifies injury through oxidative stress, inflammation, and cell death-a phenomenon called ischemia-reperfusion (I/R) injury. Milk fat globule-EGF factor 8 (MFG-E8), a multifunctional glycoprotein secreted by stem and immune cells,

Metformin as a disease-modifying therapy in osteoarthritis: bridging metabolism and joint health.

BACKGROUND: Osteoarthritis (OA) and impaired glucose tolerance (IGT) frequently coexist, leading to compounded clinical and metabolic challenges. This study investigates the effects of metformin in improving both clinical outcomes (pain, stiffness, physical function) and metabolic parameters (inflammatory markers, lipid profile, BMI) in patients with knee OA and IGT. METHODS: The study included 60 patients diagnosed with knee OA and IGT. Participants were divided into two groups: 26 patients received standard OA treatment without metformin (Without Metf), while 34 received metformin (500 mg t

Advances in cell therapy for orthopedic diseases: bridging immune modulation and regeneration.

Orthopedic diseases pose significant challenges to public health due to their high prevalence, debilitating effects, and limited treatment options. Additionally, orthopedic tumors, such as osteosarcoma, chondrosarcoma, and Ewing sarcoma, further complicate the treatment landscape. Current therapies, including pharmacological treatments and joint replacement, address symptoms but fail to promote true tissue regeneration. Cell-based therapies, which have shown successful clinical results in cancers and other diseases, have emerged as a promising solution to repair damaged tissues and restore fun

Engineering Inflammation-Resistant Cartilage: Bridging Gene Therapy and Tissue Engineering.

Articular cartilage defects caused by traumatic injury rarely heal spontaneously and predispose into post-traumatic osteoarthritis. In the current autologous cell-based treatments the regenerative process is often hampered by the poor regenerative capacity of adult cells and the inflammatory state of the injured joint. The lack of ideal treatment options for cartilage injuries motivated the authors to tissue engineer a cartilage tissue which would be more resistant to inflammation. A clustered regularly interspaced short palindromic repeats (CRISPR)-Cas9 knockout of TGF-β-activated kinase 1 (T

Bridging Endocrine Therapy for HR+/HER2- Resectable Breast Cancer: Is it Safe?

BACKGROUND: The COVID-19 pandemic has required new treatment paradigms to limit exposures and optimize hospital resources, including the use of neoadjuvant endocrine therapy (NAET) as bridging therapy for HR+/HER2-invasive tumors and DCIS. While this approach has been used in locally advanced disease, it is unclear how it may affect outcomes in resectable HR+/HER2- tumors. METHODS: Women ≥18 years diagnosed with in situ (Tis) or non-metastatic HR+/HER2- breast cancer from March-May 2019 and 2020 were included. Fisher's exact test and two-sample t test were used to compare baseline characteris

Early neovascular bridging after photodynamic therapy of myopic choroidal neovascularization.

BACKGROUND: Photodynamic therapy is a relatively new modality for the treatment of subfoveal choroidal neovascularization (CNV). An unusual response to verteporfin therapy of myopic CNVs is reported. METHODS: Out of 187 CNVs secondary to pathologic myopia treated with photodynamic therapy, three eyes manifested two separate foci of CNV before treatment. The three eyes were treated with verteporfin therapy according to the standard protocol, using a single spot of light that covered both neovascular fronds. RESULTS: All the three eyes showed neovascular bridging of the two separate neovascula

Scimitar syndrome and single ventricle physiology: evolving strategies and the role of bridge-to-transplant therapy.

Scimitar syndrome is a rare CHD involving anomalous pulmonary venous drainage and lung hypoplasia. Severe cases complicate single ventricle palliation, prompting a shift to transplantation. Our case series highlights suboptimal outcomes despite this shift, emphasising the need for further research to optimise treatment in this challenging population.

Bipolar Androgen Therapy as a Potential Mechanistic Bridge to Enhance PARP Inhibitor Efficacy in Prostate Cancer.

Prostate cancer remains a leading cause of cancer-related mortality, largely driven by progression to metastatic castration-resistant prostate cancer (mCRPC). Although poly(ADP-ribose) polymerase inhibitors (PARPis) have improved outcomes in patients with homologous recombination repair (HRR) alterations, particularly in BRCA2-mutated disease, their clinical benefit is limited by restricted patient selection, modest efficacy in non-BRCA HRR alterations, and the frequent emergence of resistance. These limitations highlight an unmet need for strategies that can both expand the therapeutic popula
