Parenchyma-Sparing Bronchial Sleeve Resection for Central Typical Endobronchial Pulmonary Carcinoid in a Young Adult: A Case Report.

INTRODUCTION: Pulmonary carcinoid tumors are uncommon neuroendocrine neoplasms. Central endobronchial lesions may cause obstruction, hemoptysis, recurrent infection, and asthma-like symptoms. Surgery aims for complete resection while preserving functional lung parenchyma when feasible. CASE PRESENTATION: A 26-year-old male smoker presented with an 8-month history of chronic cough, recurrent small-volume hemoptysis, and exertional dyspnea. Chest CT suggested a central right endobronchial lesion. Flexible bronchoscopy showed a smooth polypoid mass obstructing the bronchus intermedius. Bronchial

Bronchial adenocarcinoma presenting as a lingual tonsillar metastasis.

We describe a case of a lingual tonsillar metastasis as the first manifestation of a bronchial adenocarcinoma. Tonsillar metastases infrequently become manifest before the diagnosis of the primary neoplasm. A review of the literature disclosed 89 cases of carcinoma metastasizing to the palatine tonsil, but no one has reported the involvement of the lingual tonsil. Our patient is the first described case of bronchogenic carcinoma with this unusual form of presentation and furthermore is also the first carcinoma metastasizing to the lingual tonsil.

ETV6-NTRK3 translocation-associated low-grade mucinous bronchial adenocarcinoma: A novel bronchial salivary gland-type non-small cell lung cancer subtype.

INTRODUCTION: NTRK fusion genes have been found in several solid tumors, among which NSCLC and sarcoma. Novel NTRK translocation-related tumors are still being discovered. METHODS: We report a 49-year-old patient with a mass in the left lower lung lobe that was resected. This specimen was analyzed and sequenced using targeted DNA next generation sequencing (NGS) and anchored-multiplex-PCR (AMP) targeted RNA NGS. RESULTS: On pathological evaluation, a peribronchial mucinous neoplasm with a unique morphology was found. RNA NGS analysis showed anETV6-NTRK3 translocation in a low-grade mucinous

Management dilemmas of tracheal paraganglioma: a case report and review of literature.

Paragangliomas of the trachea are rare neoplasms and can present to the clinician with acute airway problems. These neoplasms sometimes are misdiagnosed by general practitioners as asthmatic exacerbation. We present the case of a 66-year-old woman who presented to us with a history of dyspnoea at rest in the supine position and on exertion and a productive cough. This was diagnosed as bronchial asthma and she was treated with corticosteroid inhalers for four months by her GP. She was subsequently evaluated by computed tomography of the neck and thorax, which revealed an intratracheal enhancing

Gastroesophageal reflux presented with haemoptysis and carinal erythematous lesion resembling in situ carcinoma.

Gastroesophageal reflux (GER) is often associated with respiratory disorders. We report an unusual case of GER presented with haemoptysis. On fibreoptic bronchoscopy (FFB) a focal erythematous lesion of the mucosa of the main carina was found. Repeated FFB and biopsy excluded in situ neoplasm. Pharmacological treatment of GER with sisapride and ranitidine resulted in complete remission of the bronchial lesions. To the best of our knowledge haemoptysis with bronchoscopic lesions due to the gastroesophageal reflux has not been described previously.

Report of two cases of quintuple primary malignancies and review of the literature.

Multiple primary malignant neoplasms (MPMN) are not uncommon, however, finding more than three primary malignancies in one individual is unusual. Surviving five malignancies is considered exceptional. Two patients surviving five primary malignant neoplasms for 12 and 18 years are reported: a 55-year-old woman with a squamous cell carcinoma of the larynx, two carcinomas of the breast, a carcinoma of the kidney and an adenocarcinoma of the colon, and a 75-year-old woman with a sarcoma of the myometrium, a carcinoma of the thyroid, an adenocarcinoma of the rectum, a leiomyosarcoma of the colon an

Pulmonary Langerhans Cell Histiocytosis Presenting as Multiple Obstructing Bronchial Lesions and Pulmonary Masses.

Pulmonary Langerhans cell histiocytosis is a rare lung disease, which was previously classified as interstitial lung disease but is now classified as a myeloid neoplasm. The obstructing tracheobronchial lesion or mass may rarely be the manifestation of the disease. We describe a case of pulmonary Langerhans cell histiocytosis presenting as multiple obstructing bronchial lesions and pulmonary masses in a 36-year-old man. This case represents an extremely rare manifestation of pulmonary Langerhans cell histiocytosis.

Endobronchial lipoma: a case report.

Endobronchial lipoma (EL) is a rare benign neoplasm that may cause irreversible pulmonary damage distally, and may be misdiagnosed clinically as a bronchial carcinoid or malignant tumor. They simulate malignant tumors, because of the age, sex and smoking history of the patients in whom they are found. Proper management is a "must" in order to avoid serious and unnecessary complications of the lung. We present a case of endobronchial lipoma, which is located in the right upper lobe bronchus, and diagnosed and treated by surgery.

Tumour-like presentation of pulmonary sarcoidosis.

We report a case of pulmonary sarcoidosis, which initially presented as a left apical infiltrate. The later course mimicked a pulmonary neoplasm, with left upper lobe atelectasis secondary to bronchial stenosis, resulting from both endobronchial sarcoidosis and extrinsic compression by enlarged lymph nodes. Extrinsic pressure from sarcoid nodes on the left main pulmonary artery and recurrent laryngeal nerve, also caused a reduction in pulmonary parenchymal perfusion and left vocal cord paresis.

Endocrine neoplasms in familial syndromes of hyperparathyroidism.

Familial syndromes of hyperparathyroidism, including multiple endocrine neoplasia type 1 (MEN1), multiple endocrine neoplasia type 2A (MEN2A), and the hyperparathyroidism-jaw tumor (HPT-JT), comprise 2-5% of primary hyperparathyroidism cases. Familial syndromes of hyperparathyroidism are also associated with a range of endocrine and nonendocrine tumors, including potential malignancies. Complications of the associated neoplasms are the major causes of morbidities and mortalities in these familial syndromes, e.g., parathyroid carcinoma in HPT-JT syndrome; thymic, bronchial, and enteropancreatic

Bombesin in human neuroendocrine (NE) neoplasms.

Bombesin is a 14 amino acid peptide isolated from amphibian skin which was found to have stimulatory effects upon gastric and pancreatic secretions, release of gastrointestinal hormones, gallbladder contraction and bronchoconstriction. It is present in amphibian gastric endocrine cells, avian proventriculus endocrine cells and avian brain. In mammals it is present mainly in nerve cells and fibers. The only mammalian endocrine cell shown to date to have bombesin is the P-cell in fetal lung. Bombesin is also found in mammalian brain, with its highest concentration in the hypothalamus. We examine

Morbidity, mortality, and survival after bronchoplastic procedures for lung cancer.

OBJECTIVE: Bronchoplastic procedures represent an effective surgical therapy for benign lesions, tumors of low-grade malignancy and also bronchogenic carcinoma in patients with a limited pulmonary function. We analyzed our experience in order to verify the mortality, morbidity, and long term survival in our patients. METHODS: From 1980 to 1994, 217 patients underwent bronchoplastic procedures. We performed 92 bronchoplasties, 94 bronchial sleeves, and 31 tracheo-bronchial sleeves. Histologic examination revealed 133 epidermoid carcinomas, 28 adenocarcinomas, 11 small cells lung cancers, 5 lar

The role of 68Ga-DOTA derivatives PET-CT in patients with ectopic ACTH syndrome.

INTRODUCTION AND AIM: Ectopic ACTH secretion (EAS) is mostly secondary to thoracic/abdominal neuroendocrine tumours (NETs) or small cell-lung carcinoma (SCLC). We studied the diagnostic accuracy of CT with 68Ga-Dota derivatives (68Ga-SSTR) PET in localizing ACTH-secreting tumor in patients with EAS. MATERIALS AND METHODS: 68Ga-SSTR-PET/CT was performed and compared with the nearest enhanced CT in 18 cases (16 primary and 2 recurrent neoplasms). Unspecific, indeterminate and false-positive uptakes were assessed using conventional imaging, follow-up or histology. RESULTS: We diagnosed 13 thora

Multifocal endobronchial carcinoid tumors: a rare case.

We present a case of multifocal endobronchial carcinoid tumor and review the literature on multifocal endobronchial carcinoid tumors. Our patient was admitted with complaints of paroxysmal cough and recurrent lower respiratory tract infections. Computed tomography of the chest showed tubular densities in the bilateral lower lobes and a 15-mm soft-tissue mass in the right lower lobe without any enlargement in the mediastinal lymph nodes. On positron emission tomography scan, there was no fluorodeoxyglucose uptake in any of these lesions. Bronchoscopy showed multiple endobronchial tumors with hy

[Bronchiolitis obliterans associated with paraneoplastic pemphigus: a paraneoplastic autoimmune multiorgan syndrome].

INTRODUCTION: Paraneoplastic pemphigus is a mucocutaneous disease characterized by well defined clinical and immunopathological features associated with neoplasia. Recent evidence of bronchial epithelium involvement has led to the suggestion that this process is a paraneoplastic autoimmune multiorgan syndrome. CLINICAL OBSERVATION: We report the case of a patient with lichenoid eruptions on the skin and mucous membranes who later developed progressive dyspnea. With a suspected diagnosis of paraneoplastic autoimmune multiorgan syndrome, the following diagnostic tests were performed: histology

Bronchial carcinoid tumors: clinical and radiological correlation.

PURPOSE: To evaluate the incidence of bronchial carcinoid tumor (BCT) in population affected by various lung tumors, retrospectively reviewed between 1986-2001, and to emphasize the radiographic patterns if they are characteristic. MATERIALS AND METHODS: In the above mentioned period, chest x-ray and CT were performed in 1110 patients, both male and female, affected by lung neoplasms. RESULTS: 20 patients were affected by BCT (16 males, and 13 females), aged between 26 and 75 years (mean age 57.5), with histopathologic diagnosis of typical (9 cases, 31%) and atypical (20 cases, 69%) bronchia

Benign tumours of the bronchopulmonary system.

The vast majority of tumours arising in the bronchopulmonary system are malignant in nature. Benign tumours of the lung are relatively rare and are often incidental findings during clinical investigations for unrelated conditions. These lesions can arise in the bronchial tree or the pulmonary parenchyma and may be of epithelial, mesenchymal, salivary gland-type or unknown differentiation. Although the spectrum of these lesions is wide, the clinical, pathological and immunohistochemical characteristics of the most relevant will be the subject of this review. In addition, the most important feat

Endobronchial lipoma a rare cause of pleural empyema: a case report.

Benign neoplasm of the endobronchial tree is quite rare, while endobronchial lipoma is extremely rare. The irreversible pulmonary damage is due to progressive bronchial obstruction; even so, pleural empyema is exceptionally encountered in a case of endobronchial lipoma. We report a case of a 47-year-old man who had left lung pneumonia with hemoptysis. The chest computed tomography showed cystic bronchiectasis with pleural effusion, Flexible bronchoscopy revealed a round tumor on the left main bronchus.

Case Report: Uniportal Video-Assisted Thoracoscopic Parenchymal Sparing Secondary Carinal Resection and Reconstruction for the Treatment of Tracheobronchial Mucoepidermoid Carcinoma.

Surgical resection is currently the mainstay of treatment for tracheobronchial mucoepidermoid carcinoma (TMEC). The parenchymal sparing secondary carinal resection and reconstruction for TMEC under the uniportal thoracoscopic approach has seldomly been reported in the literature. Here, we report a case of a 42-year-old male patient complaining of the incidental finding of a 1.5 × 1.2 cm neoplasm at the opening of the right bronchus intermedius and a 5.1 × 3.1 cm patchy lesion located at the left upper lobe by chest CT scans in February 2021. This patient successively underwent fiberoptic bronc
