Symptomatic Pneumorrhachis from Bronchial-Subarachnoid Fistula.

Bronchial-subarachnoid fistulas are rare occurrences, which are not well defined in the literature. This uncommon clinical phenomenon may result in symptomatic pneumorrhachis and presents unique clinical challenges. This report details a case of a 53-year-old female whose treatment for recurrent chondrosarcoma of the thoracic spine included multiple surgeries and radiotherapy. Two weeks after her most recent debulking surgery, she experienced a rapid onset of unusual symptoms, including headache, back and neck spasms, bladder incontinence, and confusion. The source of her symptoms was found to

[Anesthetic management in bronchial asthma].

In daily practice, acute and chronic pulmonary diseases are common issues presenting to the anesthetist. Respiratory physiology in general is affected by both general and regional anesthesia, which results in an increased number of perioperative complications in pulmonary risk patients. Therefore, anesthetic management of patients with bronchial asthma needs to address different clinical topics: the physical appearance of pulmonary disease, type and extent of surgical intervention as well as effects of therapeutic drugs, anesthetics and mechanical ventilation on respiratory function. The prese

Use of pocket-sized turbine spirometer in monitoring exercise-induced bronchospasm and bronchodilator responses in children.

For field studies of asthma, portable hand-held pulmonary function testing devices are required. Other than for peak flow measurements, little has been done to validate their use in children. Fifty children aged 5-15 years having asthma symptoms were examined using an exercise challenge (8 min free running outdoors) and a bronchodilation test (salbutamol inhalation at a dose of 0.15 mg/kg). Pulmonary function was measured with a turbine spirometer, with a Wright peak flow meter (WPEF) and with a flow-volume spirometer (FVS). A fall of 15% or more in peak expiratory flow associated with wheezin

Airway Hyperresponsiveness in Asthma: Measurement and Clinical Relevance.

Airway hyperresponsiveness is a characteristic feature of asthma, and its measurement is an important tool in its diagnosis. With a few caveats, methacholine bronchial provocation by a 2-minute tidal breathing method is highly sensitive; a negative test result (PC20 > 16 mg/mL, PD20 > 400 μg) rules out current asthma with reasonable certainty. A PC20 value of less than 1 mg/mL/PD20 value of less than 25 μg is highly specific (ie, diagnostic) but quite insensitive for asthma. For accurate interpretation of the test results, it is important to control and standardize technical factors that have

Differential inhibition of bronchoconstriction by the calcium channel blockers, verapamil and nifedipine.

Recent studies have demonstrated that the calcium channel blocking agents can inhibit experimentally induced bronchoconstriction in asthmatics, but their protective action has been variable. To clarify the influence of stimulus intensity and choice of calcium blocker on these reported differences in outcome, we performed noncumulative thermal stimulus-response curves using isocapnic hyperventilation of cold air in 8 asthmatics. Subjects received pretreatment with orally administered nifedipine (20 mg), intravenously administered verapamil (10 mg bolus followed by a continuous infusion), or app

Successful Treatment with Mepolizumab for Coronary Spastic Angina Associated with Eosinophilic Granulomatosis with Polyangiitis.

A 46-year-old man with a history of bronchial asthma and chronic sinusitis presented to our hospital with chest pain. We suspected angina evoked by epicardial coronary spasm and performed an ergonovine provocation test to diagnose coronary spastic angina (CSA). The patient also met the diagnostic criteria for eosinophilic granulomatosis with polyangiitis (EGPA) and was treated with 60 mg prednisolone (PSL) for EGPA-associated CSA. After PSL administration, eosinophils decreased, and angina attacks disappeared. However, when PSL was tapered to 12.5 mg, chest pain recurred. We administered mepol

Hypomagnesemia and smooth muscle contractility: diffuse esophageal spasm in an old female patient.

The aim of this paper is to describe and discuss, on the basis of the available literature, the case of an old female patient, admitted to our university hospital because of a severe dysphagia for solid foods, in whom laboratory data showed a marked hypomagnesemia. She reported a long history (20 years) of allergic bronchial asthma treated with theophylline. Esophagography evidenced a disorder of esophagus motility with diffuse multiple spasm, reminiscent of the 'corkscrew esophagus'. A link with the severe hypomagnesemia (Mg 1.1 mEq/l, normal range 1.6-2.1) was suspected, and a therapy with o

Nebulized colistin as the adjunctive treatment for ventilator-associated pneumonia: A systematic review and meta-analysis.

PURPOSE: Nebulized colistin (NC) is a potential therapy for ventilator-associated pneumonia (VAP); however, the clinical efficacy and safety of NC remain unclear. This study investigated whether NC is an effective therapy for patients with VAP. MATERIALS AND METHODS: We performed a search in Web of Science, PubMed, Embase, and the Cochrane Library to retrieve randomized controlled trials (RCTs) and observational studies published at any time until February 6, 2023. The primary outcome was clinical response. Secondary outcomes included microbiological eradication, overall mortality, length of

[Anesthetic Management of Children with Upper Respiratory Tract Infections].

Respiratory adverse events, such as cough, hypoxia, laryngospasm, bronchospasm and stridor, are major causes of morbidity and mortality during pediatric anesthesia. Since several studies have shown that upper respiratory tract infection is an independent risk factor for perioperative respiratory adverse events, children presenting for elective surgery with upper respiratory tract infection require a careful evaluation to decide whether or not to proceed with anesthesia. However, there are no standard guidelines to proceed with or postpone anesthesia, and thus the decision pro- cess is often ba

The effect of prehospital nebulized naloxone on suspected heroin-induced bronchospasm.

BACKGROUND: Snorting or smoking heroin is a known trigger of acute asthma exacerbation. Heroin abuse may be a risk factor for more severe asthma exacerbations and intubation. Heroin and other opioids provoke pulmonary bronchoconstriction. Naloxone may play a role in decreasing opioid-induced bronchospasm. There are no known clinical cases describing the effect of naloxone on opioid-induced bronchospasm. METHODS: This is an observational study in which nebulized naloxone was administered to patients with suspected heroin-induced bronchospasm. Patients with spontaneous respirations were adminis

Intravenous magnesium sulfate in acute, life-threatening asthma.

Magnesium, a physiologic calcium antagonist, is known to have a direct effect on calcium uptake in smooth muscle, resulting in smooth muscle relaxation. Studies of magnesium sulfate infusions in patients with mild or moderate-to-severe acute asthma have shown that it may have a significant bronchodilatory effect, similar to that of salbutamol (albuterol). We present the case of a patient with acute severe life-threatening asthma (initially in cardiorespiratory arrest) who responded to an IV infusion of magnesium sulfate after aggressive pharmacologic management failed to result in clinical imp

Transdermal scopolamine attenuates methacholine-induced bronchospasm.

Anticholinergic agents have been widely used in the management of asthma. The use of scopolamine has been limited by significant side effects. Transdermal delivery of scopolamine (TS) has, however, been used successfully for the prevention of motion sickness. The purpose of this study was to determine if TS would decrease methacholine-induced bronchospasm in a group of subjects with mild asthma. Bronchoprovocational challenges with inhaled methacholine were performed on three separate occasions in 10 male subjects who each had a past history of asthma. After a baseline challenge, each subject

Anaphylaxis.

The clinical syndrome of anaphylaxis may be produced by a number of mechanisms. The life-threatening clinical features are angioneurotic oedema, bronchospasm, cardiovascular collapse and pulmonary oedema. The initial treatment consists of general resuscitation plus adrenaline. In shock, colloid solutions should be used to restore blood volume. The pulmonary oedema that occurs is a membrane oedema, and should be treated with artificial ventilation, positive end expiratory pressure, and volume replacement. After a reaction, all available tests should be used to determine the cause, and the patie

Anaphylactoid reactions due to haemodialysis, haemofiltration, or membrane plasma separation.

A previously undescribed anaphylactoid reaction to haemodialysis, haemofiltration, or membrane plasma separation occurred in 15 patients receiving regular dialysis. The illness varied in severity from urticaria, sneezing, and watering of the eyes to severe bronchospasm and cardiovascular collapse, and began within a minute of blood being returned from the dialyser or filtration device to the patient. Reactions developed only when a dialyser sterilised with ethylene oxide was used for the first time and never after sterilisation with formalin. Several patients had more than one reaction while t

Pharmacology of disodium cromoglycate.

When given before exposure to an allergen, cromoglycate (DSCG) inhibits immediate and late bronchoconstriction after allergen inhalation. DSCG inhibits exercise- and SO2-induced bronchoconstriction and probably also has a soothing effect on vago-vagal reflexes which cause bronchospasms deriving from irritant receptors of the lung. DSCG given for a long period of time reduces bronchial hyperreactivity, i.e. readiness for asthma in general (the crucial dosage of histamine able to induce asthma increases). Bronchial hyperreactivity is caused by viral infections, late allergic reactions, irritatin
