We present a symptomatic bradycardia-dependent paroxysmal atrio-ventricular (A-V) block occurred within the bundle of His. Paroxysmal A-V block was caused by atrial or ventricular premature beats and also by sinus slowing. This case also showed tachycardia-dependent A-V block within the bundle of His. We also discussed the controversial mechanisms of paroxysmal A-V block.

Painful left bundle branch block syndrome is a rare disorder in which patients develop typical angina-like pain without identifiable ischemia. To date, there have been few published cases of effective treatment. In this case report, we describe successful implementation of His bundle pacing for durable symptom resolution in this disorder.

BACKGROUND: It is generally accepted that bundle branch block (BBB) may be corrected simply by selective His bundle pacing (S-HBP) distal to site of block. However, clinical HBP is often non-selective (NS), the site of block unknown and moving the HB lead is not always an option. Thus, while correction of right (R) BBB in NS-HBP has been reported, its mechanism is unknown.
METHODS: Threshold testing was done during His bundle pacing in 39 patients with RBBB. We compared NS-HBP and S-HBP and analyzed the role of pacing voltage and capture threshold of the parallel pathway, in partially or comp

Cocaine abuse can cause degenerative and inflammatory alterations of the myocardium. Cocaine-induced myocarditis has been previously described, but mostly in chronic cocaine addicts. Among several pathogenetic mechanisms of cocaine-related myocardial damage, the myocardial adrenergic stress and cardiomyocytes apoptosis have been recently proposed. We report an unusual case of myocarditis involving the bundle of His and left bundle branch and cardiac myocytes apoptosis in otherwise healthy occasional cocaine abuser.

Although His bundle pacing (HBP) can provide a physiologic ventricular activation pattern, it has disadvantages such as the difficulty of lead implantation, reduced R wave amplitudes, and high and unstable pacing thresholds. Recent studies have demonstrated that left bundle branch area pacing (LBBaP) might overcome these deficiencies. A total of 7 nonrandomized controlled studies including 786 patients (n = 442 receiving LBBaP and n = 344 receiving HBP) with bradyarrhythmia were evaluated. Compared with HBP, LBBaP appeared to result in increased R wave amplitudes (at implant: MD 9.84 mV, 95% c

Ablation of the bundle of His by an electric discharge was made in 10 patients (average age 64 years, range 48-80) unsuccessfully treated with 3-14 antiarrhythmic drugs or their combinations (average 8.7) on account of supraventricular tachycardia occurring repeatedly for 3-44 years (average 12.5 years). By means of a bipolar electrode inserted into the area of the bundle of His a nonsynchronized defibrillation discharge with a mean energy of 323 J (40-380) was administered. On average 2.9 discharges were used (1-9) per patient, in 1-3 sessions. The patients were followed up for a period of 15…

A histological study by the method of seriated sections in one case of "double-outlet left ventricle" made it possible to confirm and to describe the embryology of the nodal-His junction: not only the atrio-ventricular node (AV N) but also the His bundle (H) have a double origin, atrial and ventricular. Fusion of these two buds, inexistant in the "lone ventricle" is certainly imperfect. Two macroscopically normal hearts provided examples of nodal buds in one case, of a persisting cleft between the two His buds in the other.

The authors described seven cases of supraventricular tachycardia with atrio-ventricular dissociation, associated with the activity of an automatic focus in the bundle of His. These tachycardias, which appear at birth or are discovered at a very young age, appear to be congenital and sometimes familial, and are always isolated, having no associated cardiac abnormality. They give rise to cardiac failure, which is more marked when the rate is high (180-260/mn) and particularly resistant to treatment. The most effective form of treatment is amiodarone, almost always used in combination with digit…

BACKGROUND: Chronic left bundle branch block (LBBB) can lead to LBBB-mediated cardiomyopathy from left ventricular dysynchrony. His-bundle pacing (HBP) results in direct electrical synchrony using the native His-Purkinje system, providing a novel treatment for this cardiomyopathy.
OBJECTIVE: To assess the feasibility of HBP for cardiac resynchronization therapy (CRT) in LBBB-mediated cardiomyopathy patients.
METHODS: Retrospective database review was conducted on patients who underwent CRT by the HBP capable provider at Indiana University Health and Eskenazi Hospital from August 2015 to Augu

Painful left bundle branch block (LBBB) syndrome is a rare cause of episodic chest pain associated with transient LBBB in the absence of flow-limiting coronary artery disease and myocardial ischaemia on functional testing. The aetiology of this phenomenon is not clear, but in many reported cases, these transient episodes of LBBB are rate related. The mechanism of chest pain is not well understood. Still, it is postulated that sudden loss of the ventricular contraction synchrony, which happens in LBBB, will induce a different perception of heartbeat in the brain with possible translation to the

INTRODUCTION: Pacing of the His bundle and conduction system seems an attractive site for pacing. Lead placement in His-pacing might be technically challenging due to surrounding structures and particular anatomic location.
PATIENT CONCERNS: A 62-years old male patient was admitted for recurrent syncope. Electrocardiographic monitoring revealed periods of complete atrioventricular block with left branch block morphology and a QRS duration of 160 ms.
DIAGNOSIS: A diagnosis of intermittent complete atrioventricular block was made with a Class I indication of permanent dual-chamber cardiac paci

BACKGROUND: Biventricular pacing (BiV) to effect cardiac resynchronization therapy can be technically difficult and fails to elicit a clinical response in 30% to 40% of patients. Direct His-bundle pacing (DHBP) theoretically could obviate some of these problems. Although DHBP is capable of narrowing the QRS in some patients, the consistency with which this can be achieved has not been characterized.
OBJECTIVE: The purpose of this study was to restore His-Purkinje functionality in consecutive patients undergoing de novo clinically mandated cardiac resynchronization therapy.
METHODS: DHBP was

Atrioventricular nodal (AVN) ablation with concomitant pacemaker implantation is one of the strategies that reduce symptoms in patients with atrial fibrillation (AF). However, the long-term adverse effects of right ventricular (RV) apical pacing have led to the search for alternating sites of pacing. Biventricular pacing produces a significant improvement in functional capacity over RV pacing in patients undergoing AVN ablation. Another alternative site for pacing is direct His bundle to reduce the adverse outcome of RV pacing. Here, we present a case of direct His bundle pacing using steerabl

Eleven patients aged 33-66 years were referred for treatment of refractory supraventricular arrhythmias. Each patient had failed to respond to medical treatment. One patient had a paroxysmal atrial flutter with a rapid ventricular response. Six patients had a chronic atrial fibrillation. Two others had a supraventricular tachycardia due to reentry in the atrioventricular (AV) node, and two patients suffered from circus movement tachycardia using a concealed left free wall accessory pathway.
