Isolated capitate fractures that result in the development of aseptic necrosis of the proximal pole are uncommon. Five patients were treated who developed aseptic necrosis of the proximal pole. Four had a history of trauma with radiographic evidence of nonunion. In vitro arterial injection studies were done to correlate clinical problem with intraosseous capitate vascularity. Palmar vessels were found to contribute the majority of the blood supply to the capitate. Three patterns of intraosseous blood supply were seen. In each pattern, the proximal pole received its blood supply exclusively in

Proximal row carpectomy (PRC) is a long-time, well-accepted, easy-to-reproduce procedure for the treatment of several painful degenerative c … y of strength. The authors propose an resurfacing capitate pyrocarbon implant technique without any capitate bone resection, to preserve as much as possible the carpus length and so to improve the functional recovery. The surgical technique, is described in detail and preliminary results are discussed.

INTRODUCTION AND IMPORTANCE: Capitate fractures are rare and usually occur due to high-energy trauma. They are often associated with other wrist injuries or dislocations. An isolated occurrence of capitate and triquetral fractures due to high-energy trauma is rarely reported in the literature. This rare injury pattern has a significant effect on wrist function, as presented in this case.
CASE PRESENTATION: A 19-year-old male manual worker, presented with persistent right wrist pain and progressive loss of function after sustaining motor vehicle collision 5 months prior. The patient had major

Diagnosing ulnar-sided carpometacarpal joint dislocation is difficult, and more than half of injuries are missed on initial examination. The authors hypothesized that measuring the angle between the capitate and the metacarpals (capitate-metacarpal angle) on a plain radiograph would provide a simple, reliable tool to aid in the diagnosis of ulnar-sided carpometacarpal dislocation. This study retrospectively reviewed patients who underwent surgery for ulnar-sided carpometacarpal dislocation (study group). Two authors identified the contour of the capitate and the second, fourth, and fifth metac

Background: The failure of scaphoid reconstruction by restoring both length and shape may lead to carpal mal-alignment and progressive degenerative arthritis. The aim of our study is to find a reliable method to find out the scaphoid length without measuring the contralateral scaphoid. Methods: Three X-ray wrist views were collected for 51 patients without any signs suggesting any hand and wrist fractures. The scaphoid, capitate and 3rd metacarpal bone axes lengths and carpal height were measured by 4 hand surgeons separately. Results: The scapho-capitate ratio was 1.1 ± 0.084, 1.01 ± 0.084 an

Background: Factors such as age and joint structure affect normal wrist motion. The midcarpal joint participates in flexion/extension, radial-ulnar/deviation (RUD), and since lunate/wrist type-2 is likely more stable than a lunate/wrist type-1, we suggest that midcarpal joint structure will affect wrist motion. Our purpose was to compare wrist motion in Caucasian and Japanese populations and to associate motion with midcarpal joint morphology. We hypothesised that radiographic structure would correlate with wrist motion. Methods: A total of 164 patients with normal wrist radiographs were inclu

PURPOSE: Rates of evaluation and treatment for osteoporosis following distal radius fragility fractures remain low. As a subset of patients with these fractures undergo diagnostic computed tomography (CT) scan of the wrist, utilizing bone mineral density (BMD) measurements available with this imaging can be used to detect osteopenia or osteoporosis. This information may consequently prompt intervention to prevent a subsequent fracture. The purpose of this study was to determine if Hounsfield unit (HU) measurements at the wrist correlate with BMD measurements of the hip, femoral neck, and lumba

BACKGROUND: Scaphoid excision and 4-bone fusion (4BF) is a surgical procedure to treat scapholunate advanced collapse. Some surgeons align the lunate over the capitates, whereas others leave the capitate in its uncovered native position. The capitolunate angle may affect long-term outcomes. This study examined whether postsurgical outcomes differed based on these differences in positioning.
METHODS: A retrospective analysis was performed for patients that underwent a 4BF between 2006 and 2020. Wrist range of motion; pain (0-10); and Disabilities of the Arm, Shoulder, and Hand (DASH) scores we

There are various biomechanical procedures for the treatment of lunate necrosis in the literature, but only a few deal with the possible revascularisation of the bone. We used the second dorsal metacarpal vascular bundle implantation technique in different stages of Kienböck's disease. Since 1990, twelve patients were treated by this method. In stage I-II according to Lichtman, we implanted the vessels in the lunate after excochleation and cancellous bone grafting, in stage III we used a modified form of the Graner operation and revascularised the proximalised part of the capitate. In the firs

PURPOSE: To investigate the normal enhancement patterns of the scaphoid, lunate, and capitate bones with dynamic contrast-enhanced magnetic resonance imaging (MRI).
MATERIALS AND METHODS: The study was approved by the hospital's Ethics Committee. Nineteen volunteers (13 female, 6 male; mean age 38 years) were examined and all gave written consent. Perfusion was assessed at 3 Tesla using dynamic contrast-enhanced MRI. After two-dimensional (2D) motion correction of the data set, regions of interest were placed in the capitate, lunate, and distal and proximal pole of scaphoid bone and from the …

Regarding the upper extremity, osteonecrosis can relate to the humeral head and to any carpal bone, most commonly the lunate (Kienböck's disease), scaphoid (Preiser's disease and nonunion), and capitate bone (osteonecrosis of the capitate head). In children and adolescents, osteochondrosis is an important differential diagnosis at the epiphyses. Appropriate imaging of osteonecrosis depends on knowledge about blood supply, biomechanical load, and bone repair mechanisms. Contrast-enhanced MRI (ceMRI) enables the differentiation of up to three mostly band-shaped zones: necrotic tissue (proximal),…

A nine-year-old boy fell on his outstretched hand and was diagnosed with simultaneous fractures of the scaphoid and the capitate. The patient was treated conservatively with a cast for two months. Fracture of the capitate healed, however, fracture of the scaphoid resulted in non-union. We treated scaphoid non-union with an iliac bone graft and internal fixation. Three years after surgery, the patient had neither complaints nor complications and union of the scaphoid was confirmed on the radiographs.

UNLABELLED: The aim of this study was to assess the results of capitate osteotomy and transposition for stage III Kienböck's disease. Capitate osteotomy and transposition combined with an autologous iliac bone graft was carried out in 17 patients. At the final follow-up for a mean of 68 months (range 16-127 months) after surgery, the mean visual analogue scale score was 0.6 (range 0-5). The mean Wrightington wrist function score was 8. The mean grip strength was 79% of the unaffected side. There were 16 satisfactory results. The one unsatisfactory result occurred in a woman who developed a non

UNLABELLED: Pre-operative assessment of the deformity in scaphoid non-unions influences surgical decision-making. To characterize deformity, we used three-dimensional computed tomographic modelling in 28 scaphoid non-unions, and quantified bone loss, dorsal osteophyte volume and flexion deformity. We further related these three-dimensional parameters to the intrascaphoid and capitate-lunate angles, and stage of scaphoid non-union advanced collapse assessed on conventional two-dimensional images and to the chosen surgical procedure. Three-dimensional flexion deformity (mean 26°) did not correla

Three-dimensional (3D) structured light scanning is a beneficial documentation technique in forensic anthropology because such models facilitate continued analysis and data sharing; they can also be 3D printed for demonstrative purposes in legal proceedings and training, without risk of damage to the original skeletal material. As its application in forensic anthropology is relatively novel, the aim of the present study is to statistically evaluate the dimensional accuracy of 3D structured light scans and 3D prints for ten bone types, including the cranium, mandible, 2nd cervical vertebra (C2)

CASE: Capitate avascular necrosis should be entertained in a differential diagnosis of young, active adults with midcarpal wrist pain. We present a case study of a 30-year-old laborer who developed avascular necrosis (AVN) of his right proximal capitate. Grip strength and wrist motion were limited on examination, with advanced imaging confirming AVN. A diagnostic arthroscopy confirmed the pathology. Treatment was completed with a medial femoral trochlea vascularized flap for cartilaginous resurfacing. At 10-month follow-up, the patient's capitate was healed with stable fixation, and he is work

BACKGROUND: This review aims to synthesize current knowledge on capitate fractures, with an emphasis on diagnostic challenges, therapeutic approaches, and long-term complications, including nonunion and avascular necrosis.
METHODS: A narrative literature review was conducted using databases including PubMed, Scopus, and Google Scholar for studies published up to April 2025. Keywords included "capitate fracture," "carpal injuries," "avascular necrosis of the capitate," and "capitate nonunion." Because of the rarity of these fractures, both clinical studies and case reports were included. Artic

Carpal bone fracture-dislocation is an uncommon wrist injury. Regarding limited prevalence of such injury, most of orthopedic surgeons have low experience in treatment and handling of these fractures and dislocations. A 25-year-old male worker with an uncommon carpal bone fracture-dislocation, namely bilateral scaphocapitate fracture syndrome, was described. Volar and dorsal approaches were used for reduction and fixation; complete stabilization was achieved after open reduction and fixation using Herbert screws into scaphoid and capitate. A 5-year follow-up showed satisfactory functional and

A rare injury of the wrist, scapho-capitate fracture syndrome, in a young patient is reported. Despite early recognition of the injury and surgical intervention, the scaphoid fracture did not unite and another attempt to achieve union with bone grafting and internal fixation also failed. The wrist continued to be painful and stiff. Radiographs of the wrist, 18 months after the injury, showed nonunion of the scaphoid, avascular necrosis of the scaphoid and the lunate and carpal collapse with midcarpal joint arthritis. Due to persistent and disabling symptoms arthrodesis of the wrist had to be c

Midcarpal instability syndrome is often misdiagnosed, leading to delayed recognition, treatment and possibly poor clinical outcome. Four-dimensional computed tomography (4D CT) has previously proved useful in assessment of the acromioclavicular joint and wrist motion, allowing clinicians and radiologists to gain an understanding of abnormalities in function as well as morphology, which often contribute to patient symptoms. We present the case of a 25-year-old male with no history of trauma who presents with several months of a right wrist clunk on both passive and active (load bearing) motion.
