The benefit of coronary artery bypass grafting (CABG) in patients with coronary artery disease (CAD) who require extensive surgical procedures not involving the heart has been established. During the past decade, percutaneous transluminal coronary angioplasty (PTCA) has been developed as an alternative therapy to CABG for patients with CAD. In an attempt to determine the safety of other surgical procedures after PTCA, we retrospectively reviewed 148 patients who underwent 193 surgical procedures from four to 1,867 days after PTCA for CAD. Seventy-two patients had surgical treatment within 90 d

The seismic impact of transcatheter interventions is rocking the spectrum of structural heart disease (SHD) treatment, with the compelling and attractive appeal of minimally invasive procedures and fast-track discharge. The trend is relentless and continual innovation comes to our doors nearly on a daily basis. Litwinowicz and colleagues describe their trailblazing experience in 223 consecutive patients in whom they performed left atrial appendage occlusion via the percutaneous route. All interventions were performed by surgeons, who had undergone pretraining in a simulation model. Soon therea

OBJECTIVE: Many robotic mitral surgeons utilize right thoracotomy with transthoracic clamping of the aorta, while a smaller number employ a port-only endoscopic approach with endoaortic balloon occlusion of the aorta. We present our technique for a port-only endoscopic robotic approach with transthoracic clamping.
METHODS: From July 2019 through December 2022, 133 patients underwent port-only endoscopic robotic mitral surgery with transthoracic clamp aortic occlusion and antegrade cardioplegia. Perfusion was through the femoral artery in 101 patients (76%) and axillary in 32 patients (24%). C

BACKGROUND: Cardiac surgical capacity in sub-Saharan Africa remains severely limited, and sustainable locally led programs are scarce. This report describes the first 1000 consecutive cardiac operations performed by a fully local cardiac surgical team in Ethiopia, with a focus on implementation experience, service development, and outcomes.
METHODS: Study investigators conducted a retrospective descriptive cohort review of 1000 consecutive cardiac surgical procedures performed between June 2017 and January 2025 in Addis Ababa, Ethiopia. The same local team undertook all procedures using stand

A statewide assessment of the compliance of surgical antibiotic prophylaxis (SAP) with guidelines was undertaken for large public hospitals in Victoria, Australia. This was carried out using data collected as part of a surveillance system for hospital-acquired infections. The study population comprised patients in Victorian public hospitals with >100 beds (N=27) undergoing cardiac surgical procedures, hip or knee arthroplasty, cholecystectomy, appendectomy, colon surgery or hysterectomy over a 21-month period. Australian guidelines recommend SAP for all 10 643 surgical procedures included in t

BACKGROUND: Mitral valve repair is the standard therapy for patients with severe mitral regurgitation. Currently, robotic mitral valve repair is the least invasive surgical approach and an alternative to the traditional sternotomy. Recent studies and newer guidelines on mitral valve repair advocate for earlier referral to surgery, resulting in better long-term outcomes of valve function and decreased left ventricular remodeling.
PURPOSE: Robotic mitral valve repair outcomes were compared with nonrobotic mitral repair outcomes through analysis of 4 recent studies, one of which was the Food and

INTRODUCTION: Severe pectus excavatum (PE) is considered a relative contraindication to robotic cardiac surgery and information is lacking on surgical solutions to allow for a robotic approach in this setting.
OBJECTIVE: We present a case of concomitant minimally invasive treatment of severe PE with initial pectus correction with Nuss bar insertion followed by robotically assisted mitral valve repair.
METHODS: A multidisciplinary team planned and executed the operation. Thoracoscopic assessment at the onset of the case demonstrated mediastinal exposure was inadequate for robotic repar withou

Since 2014, we have used the da Vinci surgical system to perform internal thoracic artery harvest in minimally invasive direct coronary artery bypass (MIDCAB), and since 2016, we have also performed robot-assisted cardiac surgery( RACS) for mitral and tricuspid valve surgery, left atrial thrombosis, and myxoma, all of which we have had performed as minimally invasive cardiac surgery( MICS) previously. Even after the introduction, different ideas (Chordalizer, COR-KNOT, special long CP cannula, de-air technique, etc.) were developed. As a result of the learning curve, more stable surgery could

BackgroundWith improved cardiac surgical outcomes, attention has shifted to patient-centred outcomes, including cosmetic appearances of surgical scars. Minimally invasive cardiac surgery (MICS) is often promoted for its cosmetic benefits, yet empirical evidence remains limited. This study aimed to systematically evaluate scar-specific outcomes comparing MICS to conventional full median sternotomy (FMS) in adult cardiac surgery.MethodsA systematic review and meta-analysis was conducted following PRISMA guidelines. PubMed, Embase, Central, and Scopus were searched from inception to August 2025.

AIM: Atrial septal defects (ASD) are the most common congenital cardiac malformations. Over the preceding decades, a host of minimally invasive and interventional techniques for ASD closure have emerged. Minimally invasive ASD (MIASD) repair utilises thoracotomies, endoscopic, robotic, and even beating heart approaches to facilitate MIASD repair. We performed a systematic review to summarise the literature describing the outcomes of patients following MIASD repair.
METHOD: PubMed, Medline, and Scopus were searched by two authors for manuscripts describing outcomes following MIASD repair. Over

Background and Objectives: Minimally invasive cardiac surgery (MICS) has been developing since 1996. Peripheral cannulation is required to perform MICS, and good venous drainage and a bloodless field are crucial for the success of this procedure. We assessed the benefits of using a virtually wall-less cannula in comparison with the standard thin-wall cannula in clinical practice. Materials and Methods: Between January 2021 and December 2022, we evaluated 65 elective patients, who underwent isolated minimally invasive mitral valve surgery. Both the virtually wall-less and the thin-wall cannulas

We created a novel ring-type knot pusher with a closed triangle tip, wherein the thread is never detached from the knot pusher head during the ligation process. This knot pusher has a small head and is suitable for complex fields, such as the subvalvular apparatus of the mitral valve, allowing the surgeons to observe the knot itself. Considering that the thread passes through the inner corner of the triangle during the tying-down process without swinging inside the triangle, this knot pusher allows for a stable and secure ligation.

Over the past several decades, alcohol septal ablation has become an established therapy for selected patients, in whom there is clinical improvement in symptoms as well as objective functional capacity. Patient selection is essential to success, with continued emphasis on the procedure being performed by experienced operators as part of a multidisciplinary team. In many patients, the outcomes of alcohol septal ablation are comparable to the standard of surgical myectomy. The optimization of the outcomes of alcohol septal ablation is essential for the longitudinal care of patients with hypertr
