BACKGROUND: Despite pervasive need for peripheral intravenous catheters, insertion is often difficult, and approximately two thirds fail prematurely. Midline catheters are an alternative long peripheral catheter, inserted in the upper arm, ideal for patients with difficult access.
AIM: The aim of this study is to test feasibility of the protocol and compare the efficacy and safety of midline catheters to peripheral intravenous catheters.
DESIGN: A parallel-group, pilot randomized controlled trial of adult medical/surgical hospitalized patients, from a single Australian referral hospital.

BACKGROUND AND PURPOSE: Urethral dilation in the setting of difficult urethral catheterization is sometimes necessary to avoid suprapubic catheterization. Anecdotally, we have observed that less dilation is needed when advancing a silicone catheter over a Glidewire compared with a latex catheter of the same size. Our aim was to quantify the difference in the resistance to buckling between silicone and latex catheters.
MATERIALS AND METHODS: A BOSE Electroforce load testing device was used to test 12F and 16F silicone and latex catheters under tensile and compressive forces. This information w…

BACKGROUND: Peripherally inserted central venous catheters (PICC) have been proposed as an alternative to central venous catheters (CVC). The aim of this study was to determine the thrombosis rate in relation to PICC placement in patients discharged from the intensive care unit (ICU).
METHODS: Data of patients admitted to the ICU (Careggi Teaching Hospital, Florence, Italy; January-August 2008) and discharged with a central venous device were sequentially studied. During the first 4 months, CVCs were used (CVC group), whereas during the last 4 months, PICCs were used (PICC group). Demographic…

Central venous catheters (CVCs) are traditionally used for central venous access in the intensive care unit setting. Use of peripherally inserted central catheters (PICCs) now often extends into the intensive care unit. The goal of this review is to compare the use and safety of PICCs vs CVCs in burn patients. This institutional review board-approved cohort review included all burn patients at a single center who received one or more PICCs during a 2-year period. Primary outcome was number of days each line remained in place. Secondary outcomes were catheter-related bloodstream infection (CR-B…

Percutaneous transluminal angioplasty revolutionized coronary and peripheral revascularization. However, it is always accompanied by major drawbacks such as elastic recoil and neointimal hyperplasia. Percutaneous transluminal angioplasty along with bare metal stents reduced elastic recoil, but in-stent restenosis (ISR) and peripheral artery stenting remain problems. Drug-eluting stents addressed the issue of ISR, but late stent thrombosis and delayed endothelialization, along with longer dual antiplatelet therapy, are of concern. Non-stent-based delivery such as drug-coated balloons (DCBs) tha

The insertion of non-tunneled temporary hemodialysis catheters (NTHCs) is a core procedure of nephrology practice. While urgent dialysis may be life-saving, mechanical and infectious complications related to the insertion of NTHCs can be fatal. In recent years, various techniques that reduce mechanical and infectious complications related to NTHCs have been described. Evidence now suggests that ultrasound guidance should be used for internal jugular and femoral vein NTHC insertions. The implementation of evidence-based infection-control 'bundles' for central venous catheter insertions has sign…

In dialysis patients, both central venous catheter (CVC) insertion and CVC use during the dialysis procedure pose important legal issues, because of potentially severe, even fatal, complications. The first issue is the decision of the kind of vascular access that should be proposed to patients: an arteriovenous (AV) fistula, a graft, or a CVC. The second issue, when choosing the CVC option, is the choice of CVC: nontunneled versus tunneled. Leaving a temporary nontunneled CVC for a prolonged time increases the risk of complications and could raise a liability issue. Even when choosing a long-t

Forty-four percent of hospital patients with indwelling catheters can develop significant bacteriuria within 72 hours of catheterisation. It is, therefore, critical that catheters are manufactured with perfectly smooth surfaces and apertures to reduce the potential for infection. An ultrasonic cutting technology is described here that produces smooth apertures and eliminates problems associated with debris.

A patient undergoing total knee replacement was fitted with a stimulating femoral catheter for postoperative analgesia. After the catheter was secured to the skin, the minimal stimulatory threshold was verified again and found to be extremely low (0.00-0.01 mA; pulse width, 0.1 ms). However, in spite of this situation, no paresthesia or pain was experienced by the patient. Use of the catheter resulted in satisfactory analgesia with no complications noted during the hospital stay or at follow-up three months later. When the nerve stimulator was sent to our biomedical engineering department to v

Urinary tract infections and intravascular infections cause pain or even death, delay healing, and raise costs. New technology makes catheters safer than ever, so you can reduce these infections in your departments.

BACKGROUND: This study compares central venous catheters (CVC) and peripherally inserted central catheters (PICC) for indications for insertion, complications, and economic impact.
METHODS: A retrospective review of 838 (283 CVC, 555 PICC) consecutively placed venous catheters reflected 49,365 CVC and 11,814 PICC days.
RESULTS: There were 57 (20%) complications in the CVC group, 197 (35%) complications in the PICC group. PICC were associated with a statistically significant increase in the incidence of catheter malfunction (P = 0.0005), arm vein phlebitis (P = 0.0004), and overall complicati…

One hundred patients undergoing urinary bladder catheterization for various reasons were studied. 42 patients received Sewoon catheters of various sizes. 38 received Eschmann catheters of various sizes and 20 received Bard catheters of various sizes. The duration of catheterization ranged from a few hours to 21 days. Various problems associated with catheterization were recorded whenever encountered. Stuck catheter was found to occur in 15 of the 40 patients who received Sewoon catheters. There were no stuck catheters recorded for Eschmann or Bard catheters. Infections occurred in 9 out of 100…

Between 1983 and 1985, 170 consecutive patients received doxorubicin-containing adjuvant chemotherapy through central venous catheters, and four via a long indwelling catheter in the antecubital fossa. The objective of this retrospective study is to determine the acute and chronic complications associated with indwelling catheters. Ninety-four (56%) patients did not experience any complications. The incidence of acute complications was 2%, which included three pneumothorax. Twenty-nine (17%) patients experienced chronic complications. Of those, 20 (12%) developed infectious complications, and …
