[Toxoplasma gondii seroprevalence in a university hospital in Chile].

Toxoplasmosis is a zoonotic disease caused by Toxoplasma gondii. It is estimated to affect a third of the world's population. In Chile, in 1996, a seroprevalence of IgG antibodies against T. gondii of 36.9% was reported, which progressively increased from north to south of the country. There are no updated reports of the seroprevalence of T. gondii in the Metropolitan Region. In the present study, we determined the seroprevalence of anti T. gondii IgG in the 2013-2018 period at the Clinical Hospital University of Chile. Of a total of 1,666 results, 386 (23.2%) were positive, without gender dif…

[Scrub typhus, an emerging infectious disease in Chile].

Scrub typhus, caused by Orientia tsutsugamushi and transmitted by larvae of trombiculid mites, is an endemic rickettsiosis in the Asia Pacific region. After the first identification of a case in Chile in 2006, more than 30 cases have been diagnosed by our group since 2015. Cases were detected predominantly during the Chilean summer months. Patients presented with fever, rash, and a typical eschar at the inoculation site; other frequent findings were intense headache, night sweats, increased laboratory markers of inflammation and transaminases. The vast majority of cases have been diagnosed in …

[Cutaneous anthrax, the last outbreak diagnosed in Chile].

Anthrax is a zoonosis caused by a spore-forming bacterium, called Bacillus anthracis. Naturally it is of global distribution, with a predilection in agricultural zones with few norms of public veterinary health. Human contagion occurs through the consumption of diseased animal's meat or through a doorway into the skin or through the spores inhalation of products derived from the affected animal (wool, leather, bones). The most frequent infection in humans occurs in the skin, followed by the gastrointestinal tract and lungs. We present the last outbreak of cutaneous anthrax diagnosed in Chile w…

Typing of the rabies virus in Chile, 2002-2008.

In Chile, dog rabies has been controlled and insectivorous bats have been identified as the main rabies reservoir. This study aimed to determine the rabies virus (RABV) variants circulating in the country between 2002 and 2008. A total of 612 RABV isolates were tested using a panel with eight monoclonal antibodies against the viral nucleoprotein (N-mAbs) for antigenic typing, and a product of 320-bp of the nucleoprotein gene was sequenced from 99 isolates. Typing of the isolates revealed six different antigenic variants but phylogenetic analysis identified four clusters associated with four di…

Past, present and future of newborn screening in Chile.

The history of the Newborn Screening Program in Chile begins in 1984, when a pilot plan was developed that demonstrated the feasibility of its implementation. In 1992, the Ministry of Health started a national newborn screening program for phenylketonuria (PKU) and congenital hypothyroidism (CH), and in 1998, this was extended to the entire country. Throughout this period, a total of 2,478,123 newborns (NB) have been analyzed, obtaining initial coverage of 48.8%, which was later increased to 87.7%, and at present it is at 98.7% of all NB of our country. During this period, 131 cases with PKU h…

[Imipenem-cilastatin use in a university hospital in Chile].

Imipenem is an expensive broad-spectrum antimicrobial, reserved for infections caused by multi-resistant nosocomial pathogens. Since 2001 our university hospital applies a restriction policy that allows rejecting or authorizing its use after a supervising evaluation with pre-specified criteria for appropriate or inappropriate use. An audit was performed for all the supervisions made during the periods of March-April and September-October, 2004, totalling 136 treatments. In global terms, 58.1% of treatments were considered appropriate and 11.8% inappropriate; other 20.6% had been discontinued b

Molecular characterization of Cryptosporidium species and genotypes in Chile.

Knowledge of species and genotypes on Cryptosporidium involved in human transmission is fundamental in studying the epidemiology of cryptosporidiosis. This is the first study in molecular epidemiology of cryptosporidiosis that has been made in Chile. Cryptosporidium parvum species and genotypes involved in human and bovine cryptosporidiosis were determined by PCR-RFLP. Low prevalence of Cryptosporidium was found in humans and cattle. Of the 127 cattle studied, only in one was C. parvum detected . Of the 68 human fecal samples studied, four contained Cryptosporidium: two patients presented Cryp…

[Polydactyly: a genetic epidemiological study in Santiago, Chile].

The aim of this work was to study the prevalence at birth and family aggregation of polydactyly in Chile. We studied 125,652 newborns between 1969 and 1991. The prevalence was 1,329 for each 1,000 live newborns and higher in males than in females. Familial recurrence was 22.5% and gene penetrance was estimated as 0.5 for the postaxial-A type and 0.3 for the postaxial-B type. Gene penetrance for postaxial-B type was higher in males. Estimation of gene frequencies and mutation rates gave the highest values for postaxial-B polydactyly.

Genetic epidemiology of single gene defects in Chile.

We have studied the correlation between the ethnic structure and the prevalence of single gene defects in Chile. At present the Chilean population is approximately 64% white and 35% Amerindian with traces of other admixture. Fewer than 4% of the Chilean population are foreign born. Investigations indicate that all severe diseases and many others without impaired reproduction have mutation rates within the range of the white population. Classical ethnic diseases are very rare. Autosomal recessive disorders have a wide range of variability: cystic fibrosis has a low incidence and PKU has a simil…

[Medical liability lawsuits according to specialties in Chile].

One-hundred forty-five cases of medical liability lawsuits originating in Chile from January 1987 to June 1991 and studied at the Medico Legal Service, were reviewed. A persistent increase in the number of accusations is observed in these five years. The principal medical specialties involved are Obstetrics and Gynecology (32%), General Surgery (18%), Internal Medicine (15%) and Pediatrics (9%). Seventy seven percent of implicated physicians were accredited specialists. Fifty percent of cases originated from Santiago and 60% from public hospitals. Eighty three percent of analyzed medical recor…

[Epidemiological and diagnostic aspects of human Cryptosporidiosis in Chile].

Cryptosporidium sp. infections have been reported in different Chilean localities, both in healthy individuals and immunocompromised patients. Among the first, hospitalized and ambulatory children less than 3 years old are the age group more affected reaching frequencies from 3.1 up to 19.3%. In pre-school children, school children and adults the prevalence rates have been 1.1, 0.2 and 0.3% respectively. According to the epidemiological studies carried out, the routine laboratory diagnosis of this parasitosis should be emphasized in the parasitological stool examinations in children under 3 ye…
