Optimizing respiratory integrated care models in Ireland: insights from PCP feedback on the early days of integrated chronic disease management pathways.

BACKGROUND: In 2020 the Irish Health Service Executive (HSE) shifted chronic disease management (CDM), including COPD and asthma, into primary care via a funded CDM programme. An Enhanced Community Care (ECC) initiative established local clinical specialist teams (CSTs) providing spirometry, nurse-led clinics and pulmonary rehabilitation to support primary care practitioners (PCPs). We explored early PCP experience, diagnostic access, and confidence, to identify improvement priorities. AIM: We sought to evaluate PCPs experiences with the implementation of Ireland's CDM and ECC programmes for

Healthcare models, quality indicators and quality of care in the management of patients with chronic kidney disease in Spain: Results from the CARABELA-CKD initiative.

BACKGROUND AND OBJECTIVES: Chronic kidney disease (CKD) poses a significant public health challenge worldwide and its prevalence is rising due to aging populations and increasing risk factors. The CARABELA-CKD initiative aims to address CKD management inefficiencies. MATERIALS AND METHODS: The CARABELA-CKD initiative was structured into four phases. The methodology involved five pilot center studies, conducted to characterize CKD healthcare models (Phase 1), followed by the validation of disease care models, improvement areas, potential solutions, and strategic healthcare quality indicators i

Protocol for an evaluation of the initiation of an integrated longitudinal outpatient care model for severe chronic non-communicable diseases (PEN-Plus) at secondary care facilities (district hospitals) in 10 lower-income countries.

INTRODUCTION: The Package of Essential Noncommunicable Disease Interventions-Plus (PEN-Plus) is a strategy decentralising care for severe non-communicable diseases (NCDs) including type 1 diabetes, rheumatic heart disease and sickle cell disease, to increase access to care. In the PEN-Plus model, mid-level clinicians in intermediary facilities in low and lower middle income countries are trained to provide integrated care for conditions where services traditionally were only available at tertiary referral facilities. For the upcoming phase of activities, 18 first-level hospitals in 9 countries

Implementation Effect of a Pilot Urban Primary Health Care Model on the Usage of Health Care From Medically Trained Providers for Chronic Diseases Among Slum Populations in Bangladesh: Findings From an Implementation Research Study.

BACKGROUND: Ensuring affordable and quality primary health care for urban populations remains a persistent challenge in resource-constrained countries such as Bangladesh. The country's fragmented and pluralistic urban health system exacerbates these challenges, making it particularly difficult to deliver primary health care to urban residents. In Bangladesh, the Aalo Clinic has been piloted to provide essential health services to low-income urban residents, including slum dwellers. OBJECTIVE: This study aimed to assess how this model affects health care-seeking behavior from medically trained

Clinical prediction models for progression of chronic kidney disease to end-stage kidney failure under pre-dialysis nephrology care: results from the Chronic Kidney Disease Japan Cohort Study.

BACKGROUND: Reliable prediction tools are needed to identify patients with chronic kidney disease (CKD) at greater risk of developing end-stage kidney failure (ESKF). We developed and validated clinical prediction models (CPMs) for CKD progression to ESKF under pre-dialysis nephrology care using CKD-Japan Cohort (CKD-JAC) data. METHODS: We prospectively followed up 2034 participants with CKD, defined as an estimated glomerular filtration rate (eGFR) less than 60 mL/min/1.73 m2, aged 20-75 years for a mean of 3.15 years. We randomly divided the overall analysis set into development and validat

Effectiveness of Biopsychosocial Model-Based Care versus Routine Physical Therapy on Clinical, Sensorimotor, and Psychosocial Outcomes in Patients with Chronic Low Back Pain: A Randomized Clinical Trial.

BACKGROUND: Chronic low back pain (CLBP) is a leading cause of disability around the world and needs a treatment plan that should include the treatment of the physical, psychological and social aspects of the condition. Routine physical therapy (RPT) primarily targets physical impairments, with comparatively limited emphasis on the psychosocial and cognitive dimensions of recovery. The proposed study examines the efficacy of biopsychosocial (BPS) based treatment on pain intensity as the primary outcome, alongside secondary clinical, sensorimotor, and psychological outcomes. METHODOLOGY: This

Randomized controlled trial of a coordinated care intervention to improve risk factor control after stroke or transient ischemic attack in the safety net: Secondary stroke prevention by Uniting Community and Chronic care model teams Early to End Disparities (SUCCEED).

BACKGROUND: Recurrent strokes are preventable through awareness and control of risk factors such as hypertension, and through lifestyle changes such as healthier diets, greater physical activity, and smoking cessation. However, vascular risk factor control is frequently poor among stroke survivors, particularly among socio-economically disadvantaged blacks, Latinos and other people of color. The Chronic Care Model (CCM) is an effective framework for multi-component interventions aimed at improving care processes and outcomes for individuals with chronic disease. In addition, community health w…

Coordinating across correctional, community, and VA systems: applying the Collaborative Chronic Care Model to post-incarceration healthcare and reentry support for veterans with mental health and substance use disorders.

BACKGROUND: Between 12,000 and 16,000 veterans leave incarceration annually. As is known to be the case for justice-involved populations in general, mental health disorders (MHDs) and substance use disorders (SUDs) are highly prevalent among incarcerated veterans, and individuals with MHDs and SUDs reentering the community are at increased risk of deteriorating health and recidivism. We sought to identify opportunities to better coordinate care/services across correctional, community, and VA systems for reentry veterans with MHDs and SUDs. METHODS: We interviewed 16 veterans post-incarceratio
