A 56-year-old man presented with progressive neurologic symptoms that developed more than 14 years ago. His symptoms started with numbness in the toes, followed by slowly progressive lower extremity weakness, increase in tone, and imbalance. He also developed bladder and bowel urgency, constipation, and erectile dysfunction. He did not have any symptoms in his arms.

A 39-year-old lawyer presented with intermittent spasms and pain in his abdominal muscles, particularly the right upper quadrant. These had occurred since his mid-20s and there had been long asymptomatic periods, including 8 years prior to the most recent 4-month exacerbation. Trivial movement triggered a spasm of the abdominal muscles, leading to severe pain, which made breathing uncomfortable and interfered with sleep. The symptoms subsided spontaneously after 4 to 5 days, leaving him with a sore abdomen for several weeks. Past attacks had also been precipitated by specific forms of repetiti

BACKGROUND: Clinical reasoning skills are essential for medical practice. Problem-based collaborative learning via the internet might prove useful in imparting these skills.
AIM: This randomized study assessed whether web-based learning (WBL) is superior to face-to-face problem-based learning (PBL) in the setting of a 6-week cardio-respiratory course.
METHODS: During winter term 2007/08, all 148 fourth-year medical students enrolled in the 6-week course consented to be randomized in small groups to diagnose a patient complaining of dyspnoea either using a virtual collaborative online module …

BACKGROUND: The quality gap and clinical inertia are stubborn problems that are prevalent, but difficult to explain. We puzzle why conscientious practitioners, who are keenly aware of appropriate evidence based treatments, avoid initiating or intensifying treatments. This has caused some to question that there may be hidden causes for these errors in the planning of chronic care.
METHODOLOGY: A timely clue enabled the author to recognize his flawed reasoning when planning the care of two elderly women sustaining hip fracture. The sudden awareness of planning error motivated a study of the und

INTRODUCTION: Clinical reasoning is a complex skill that requires a healthcare professional to apply clinical judgment, expertise, and reasoning, along with foundational knowledge, to solve complex problems in uncertain situations. This study evaluated the effectiveness of teaching Clinical Reasoning Skills (CRS) through an online Team-Based Learning (TBL) approach, using scores from a Script Concordance Test (SCT) as the main outcome measure.
MATERIALS AND METHODS: This quasi-experimental study was carried out at the Orthopedic Department of Khyber Teaching Hospital, Peshawar, Pakistan, from…

Clinical studies indicate that the progression of Type 2 Diabetes Mellitus (T2DM) is associated with characteristic alterations in tongue features, which may facilitate non-invasive early detection. However, current deep learning-based tongue imaging approaches for diabetes diagnosis remain constrained by limited datasets, subtle feature variations, dependence on clinical expertise, and the lack of quantitative evaluation. To address these issues, we developed an open-source dataset for T2DM tongue diagnosis (DMT) and benchmarked it using multiple baseline models. Building on DMT, we propose T

OBJECTIVE: To assess the impact of a Learning-by-Concordance (LbC) activity within second professional year (P2) and third professional year (P3) capstone courses on student perceptions of learning, and to compare student group clinical reasoning responses to the responses of a faculty expert.
METHODS: An LbC module with 3 clinical vignettes focused on type 2 diabetes mellitus pharmacotherapy was created and implemented in P2 and P3 capstone courses. Student groups provided Likert-based responses to the clinical vignettes and completed a perceptions survey following completion of the LbC modu…

The framework draws from addiction science, conditioning theory, attachment research, and tr …

INTRODUCTION: Place-Based Medical Education (PBME) aims for more physicians who are better prepared to serve medically underserved communities. However, discussions about the role of 'place' in PBME remain largely conceptual, with limited empirical evidence. In a previous scoping review, we identified a lack of research examining how distinct features of place influence the development of Clinical Reasoning (CR) and proposed the 'HARD' framework to guide future studies. The current study builds on this work to explain how place impacts the development of clinical reasoning in medical students.…

Artificial intelligence (AI)-enabled clinical decision support (CDS) tools are increasingly embedded in clinical workflows, generating and prioritizing clinical information, including diagnostic suggestions, evidence summaries, and management recommendations. Yet guidance on how clinicians should reason with AI at the point of care remains limited, creating risks of over-reliance, inconsistent verification, and diffusion of responsibility. We propose Ask-Audit-Apply (AAA) as a clinician-facing framework for AI-supported clinical reasoning that specifies decision-level behavioral practices. AAA…

This is a case report of an established genetic etiology with a unique clinical presentation and novel imaging findings that add to the clinical spectrum of this known condition. A 2-year-old male toddler presented with acute-onset weakness and ataxia after a respiratory viral illness. A previous admission was marked by similar weakness with areflexia, ataxia, and metabolic acidosis. Initial examination revealed weakness with areflexia, loss of tone, and ataxia, followed by intermittent encephalopathy. These physical findings localized to both central and peripheral etiologies. Differential di

A 41-year-old, left-handed man was admitted to the hospital for workup of a 3-week history of headache, constant vertigo, diplopia, and left hemiparesis. An initial diagnosis was made, and the patient was treated with symptomatic improvement. He was discharged and then presented a few weeks later with recurrence of his symptoms which required additional diagnostic workup. This ultimately led to a diagnosis of a rare disorder. In this case report, we outline his clinical course and our reasoning at each step which led to our eventual definitive diagnosis.
