BACKGROUND: Studies of tin spectral filtration have demonstrated potential in reducing radiation dose while maintaining image quality for unenhanced computed tomography (CT) scans. The extent of dose reduction, however, was commonly measured using the change in the scanner's reported CTDIvol . This method does not account for how tin filtration affects patient organ and effective dose.
PURPOSE: To investigate the effect of tin filtration on patient organ and effective dose for CT Lung Cancer Screening (LCS) and CT Colonography (CTC).
METHODS: A previously-developed Monte Carlo program was ad

BACKGROUND AND AIMS: The natural history of small polyps is not well established and rests on limited evidence from barium enema studies decades ago. Patients with one or two small polyps (6-9 mm) at screening CT colonography (CTC) are offered CTC surveillance at 3 years but may elect immediate colonoscopy. This practice allows direct observation of the growth of subcentimetre polyps, with histopathological correlation in patients undergoing subsequent polypectomy.
DESIGN: Of 11 165 asymptomatic patients screened by CTC over a period of 16.4 years, 1067 had one or two 6-9 mm polyps detected (

CLINICAL PROBLEM: Colorectal cancer (CRC) is a major cause of cancer-related morbidity and mortality. Most colorectal cancers derive from benign precursor lesions, so-called adenomatous polyps, over a long period of time. Colorectal cancer screening is based on the detection of precancerous polyps and early stage CRC in asymptomatic individuals to reduce CRC incidence and mortality. The protective effect of screening programs can be improved by increasing the screening rates.
PRACTICAL RECOMMENDATIONS: Apart from the established examinations, CT colonography (CTC) has been proposed as an opti

Computed tomography colonography (CTC) was performed in 5 patients with pneumatosis cystoides intestinalis (PCI). The virtual colonoscopy view of CTC as well as total colonoscopy (TCS) findings showed polypoid lesions in the colon, and multiplanar reconstruction images of the colon revealed in the polypoid lesions of the colon. We confirmed the diagnosis of PCI in all cases. CTC also detected the PCI lesions in the subserosa of the colonic wall which were not detected by TCS. Accurate evaluation of the extent of PCI involvement was obtained by CT air-contrast enema images. CTC is useful for de

AIM: Adequate colonic imaging is generally an invasive procedure with attendant risks, particularly perforation. Endoscopy, barium enema and computed tomographic colonography (CTC) are the main techniques for investigating patients with symptoms of, or screening for, colorectal cancer. The potential complications of these investigations have to be weighed against the benefits. This article reviews the literature on the incidence, presentation and management of iatrogenic colonic perforation at colonic imaging.
METHOD: A literature review of relevant studies was undertaken using PubMed, Cochra

PURPOSE: To prospectively evaluate short- and midterm patient preference of computed tomographic (CT) colonography relative to colonoscopy in patients at increased risk for colorectal cancer and to elucidate determinants of preference.
MATERIALS AND METHODS: Consecutive patients at increased risk for colorectal cancer underwent CT colonography prior to scheduled colonoscopy. Patient experience and preference were assessed both directly after the examinations and 5 weeks after the examinations. Differences in pain, embarrassment, discomfort, and preference were assessed with the Wilcoxon signe

Despite being readily preventable, colorectal cancer ranks second behind only lung cancer in overall mortality. However, this situation could be reversed if screening tests that effectively detect advanced adenomas and early cancers were broadly applied. Computed tomographic colonography (CTC) reflects an ideal balance of minimal invasiveness with high-level performance, assuming all facets of the examination are appropriately addressed. Unfortunately, this promising screening test remains grossly underused. This article details the technical and interpretive approaches used by one successful

CT colonography (CTC) is a validated screening test for detection and prevention of colorectal cancer (CRC). In the United States, screening CTC has been covered for Medicare beneficiaries since 2025, elevating it to equal status with other reimbursed CRC screening tests and ending a 15-year policy stalemate. Screening CTC is a highly accurate examination with an immaculate safety record and equivalent sensitivity to colonoscopy for clinically significant polyps. Long-term observational CTC cohorts confirm safety for established polyp size thresholds in terms of test positivity and screening a

BACKGROUND: Computed tomography colonography (CTC) may be superior to colonoscopy and barium enema for detecting diverticula. However, few studies have used CTC to diagnose diverticula.
AIM: To evaluate the current prevalence and distribution of colonic diverticula in Japan using CTC.
METHODS: This study was conducted as part of the Japanese National Computed Tomographic Colonography Trial, which included 1181 participants from 14 hospitals in Japan. We analyzed the prevalence and distribution of colonic diverticula and their relationships with age and sex. The relationship between the diver

PURPOSE: To perform a radiologic-pathologic correlation analysis of sigmoid colon in patients undergoing pre-operative CT Colonography (CTC) after an episode of acute diverticulitis (AD).
METHODS: Fifty-nine consecutive patients (31/28 M/F; 58 ± 13 years) underwent CTC 55 ± 18 days after AD, 8 ± 4 weeks before surgery. Thirty-seven patients (63%) underwent conventional abdominal CT at time of AD. An experienced blinded radiologist retrospectively analyzed all images: disease severity was graded according to the Ambrosetti classification on conventional CT and according to the diverticular dis

For advanced right colon cancer, we perform lymph node dissection exposing the so-called surgical trunk. For the resection of advanced distal sigmoid/rectal cancer, we routinely perform lymph node dissection around the root of the inferior mesenteric artery, preserving the left colic artery. To perform either of these procedures safely, it is important to know the precise vascular anatomy with individual variations. However, there are major issues in laparoscopic surgery, such as a lack of tactile sensation and limited visual field. To overcome these issues and identify the vascular anatomy of

Each year in the UK, around 16,000 people die from colorectal cancer. At disease presentation, around 55% of people have advanced cancer that has spread to lymph nodes, metastasised to other organs or is so locally advanced that surgery is unlikely to be curative (Dukes' stage C or D). Overall 5-year survival for colorectal cancer in the UK is around 47-51% (compared to 64% in the USA), but only 7% at most in those presenting with metastatic disease. These facts underlie the current introduction of national bowel screening programmes in the UK. Here we assess the role of screening of the gener

With increasing emphasis among the medical community on the early diagnosis and staging of colorectal cancer, interest has grown in CT colonography as a developing technique to challenge existing methods such as the barium enema and conventional colonoscopy. First introduced in 1994, CT colonography has experienced dramatic improvements in both hardware and software capabilities, resulting in shorter scanning times, greater user-friendliness and promising performance statistics. The recent development in multi-slice CT scanners has meant the ability to scan patients in a single breath hold, wh

Early detection of colorectal cancer or advanced adenomas is a public health priority in many industrialized countries. There are various methods of screening average risk individuals for colorectal cancer, and their effectiveness may depend on subjective parameters like local expertise and patient's preferences. This paper reviews these tests with special emphasis regarding imaging techniques that aim to provide less-invasive alternatives to optical colonoscopy (OC) which is the standard of reference. Both Double-Contrast Barium Enema (DCBE) and Virtual Colonoscopy (VC) have >90% sensitivity

Screening for early-onset colorectal cancer is a growing public health concern, driven by a rising incidence in younger adults. As trends shift, screening guidelines continue to evolve. In this article, we examine recent epidemiologic patterns and potential causes of this shift. We also explore recent updates on current screening modalities, both structural and nonstructural, highlighting their efficancy, advantages, limitations, and cost-effectiveness to inform future clinical and policy decisions, with a focus on CT colonogrpahy. Additionally, we discuss the evolving complementary role of ar

PURPOSE: To compare examination quality and acceptability of three different low-volume bowel preparation regimens differing in scheduling of the oral administration of a Macrogol-based solution, in patients undergoing computed tomographic colonography (CTC). The secondary aim was to compare CTC quality according to anatomical and patient variables (dolichocolon, colonic diverticulosis, functional and secondary constipation).
METHODS: One-hundred-eighty patients were randomized into one of three regimens where PEG was administered, respectively: in a single dose the day prior to (A), or in a
