Prospective Assessment of Carboxyhemoglobin and Methemoglobin Levels in Operating Room Personnel During HIPEC/Peritonectomy Compared with Standard Colorectal Surgery.

Background/Objectives: In the operating room, surgical staff are exposed to inhalation of smoke from the combustion of tissues when using electrocautery. By introducing HIPEC/peritonectomy in the treatment of malignant neoplasms of the peritoneum, exposure to smoke is significantly increased. To adequately perform this procedure, high-power monopolar cautery is used. The aim of this study was to assess carbon monoxide exposure associated with surgical smoke generated during peritonectomy procedures and to compare it with exposure during standard colorectal resections. Methods: In this prospect

Feasibility of Enhanced Recovery in Emergency Colorectal Operation.

BACKGROUND: Emergency colorectal operations account for considerable surgical morbidity, leading to increased recognition of the importance of standardized care. Enhanced recovery pathways (ERPs) have successfully provided a framework to standardize elective surgical care, with some ERP elements spreading to emergency procedures. This study aims to characterize the degree of spread and demonstrate feasibility of ERP extension to emergency colorectal operations. STUDY DESIGN: Patients undergoing colorectal operations were identified from a national ERP collaborative. Adherence to ERP process m

Anesthesiological considerations and complications in robotic surgery: a retrospective analysis.

Robotic-assisted surgery has become increasingly prevalent across various surgical disciplines. Understanding anesthesiological factors influencing perioperative outcome is essential for optimizing patient care. This retrospective analysis evaluated 200 patients who underwent robotic surgery across three surgical categories: gastrointestinal (GI, n=60), hepatopancreticobiliary (HPB, n=28), and colorectal (n=112) procedures. We conducted an exploratory analysis of associations between anesthesiological parameters-including fluid management, surgical duration, and blood loss-and clinical outcome

Robotic versus laparoscopic intracorporeal anastomosis learning curve: a systematic review.

The learning curve for robotic colorectal surgery is increasingly characterised. However, the specific learning curve for intracorporeal anastomosis (IA) remains poorly defined. This review compares robotic and laparoscopic IA with respect to operative time, cases required for proficiency, complication rates, and conversion to extracorporeal anastomosis (EA). A systematic review was conducted in accordance with PRISMA guidelines. The protocol was registered on Open science framework ( https://doi.org/10.17605/OSF.IO/K4MYZ ). On March 18, 2025, OVID and PubMed were searched using the search ter

Persistent descending mesocolon and its impact on the short-term outcomes of minimally invasive colorectal surgery: a systematic review and meta-analysis.

PURPOSE: Persistent descending mesocolon (PDM) is a rare congenital anomaly that may complicate minimally invasive surgery (MIS) for colorectal cancer (CRC). This systematic review and meta-analysis evaluated the perioperative impact of PDM on MIS for CRC. METHODS: This study followed the PRISMA guidelines and was registered in PROSPERO (CRD420251055757). A systematic search was performed in PubMed, Cochrane, and Scopus (January, 2000 to April, 2025). Observational studies (OBSs) compared MIS for CRC in patients with and patients without PDM. The primary outcome was operative time and the sec

Simultaneous curative resection of double colorectal carcinoma with synchronous bilobar liver metastases.

Synchronous colorectal carcinoma (SCRC) indicates more than one primary colorectal carcinoma (CRC) discovered at the time of initial presentation, accounts for 3.1%-3.9% of CRC, and may occur either in the same or in different colorectal segments. The accurate preoperative diagnosis of SCRC is difficult and diagnostic failures may lead to inappropriate treatment and poorer prognosis. SCRC requires colorectal resections tailored to individual patients, based on the number, location, and stage of the tumours, from conventional or extended hemicolectomies to total colectomy or proctocolectomy, wh

Minimally Invasive/Robotic Surgery for Colorectal Liver Metastasis.

Colorectal liver metastases (CRLM) are common and potentially curable with complete resection. This article reviews contemporary minimally invasive (laparoscopic and robotic) strategies for CRLM, emphasizing multidisciplinary decision-making, patient selection, and peri-operative planning. It outlines key imaging and functional assessments and operative approaches ranging from parenchymal-sparing resections to complex posterosuperior segmentectomy and two-stage hepatectomy. The article also discusses adjunctive ablation and hepatic arterial infusion pump chemotherapy as integrated liver-direct

Deep learning approach for critical exposure during division of the inferior mesenteric artery in colorectal surgery.

With growth of minimally invasive surgery, there is an opportunity to analyze and learn from intraoperative videos. These videos contain a wealth of data which can inform surgical techniques. In colorectal surgery, a common critical step is inferior mesenteric artery (IMA) division. However, optimal retraction and relative tissue-relationship metrics for critical exposure during IMA division are not defined. We leverage intraoperative robotic surgery videos and artificial intelligence (AI) models to define and quantify key angles and surgical approaches during IMA division. Videos of robotic-a

Robotic surgery for rectal cancer in 2026: a hierarchical appraisal of the evidence.

Robotic total mesorectal excision (TME) now rests on an evidence base that has accumulated layer by layer: randomized trials, meta-analyses, registry cohorts, and dedicated functional-outcomes studies. The publication of the long-term results of the REAL trial (2025) marked a turning point. We propose a hierarchical evidence framework in which the REAL trial functions as confirmatory apex rather than foundational basis. It provided the first randomized evidence of better locoregional control and disease-free survival with robotic surgery compared to laparoscopy (3-year locoregional recurrence

Learning curves in robotic and laparoscopic colorectal surgery: a systematic review.

OBJECTIVE: To evaluate and compare learning-curve assessment in laparoscopic and robotic colorectal surgery and to develop a standardized framework for future learning-curve reporting. BACKGROUND: Learning curves in minimally invasive colorectal surgery are widely reported, but inconsistent definitions limit interpretability and comparability. This systematic review analyzed learning-curve assessment in laparoscopic and robotic colorectal surgery and proposes a standardized framework. METHODS: MEDLINE, Web of Science, and CENTRAL were systematically searched according to PRISMA guidelines (r

[Minimally invasive liver resection-New techniques].

Minimally invasive procedures have been slowly gaining ground in liver surgery since the 1990s but have recently become increasingly more established. In Germany they are now used in around a quarter of all resections for colorectal metastases and much more often in specialized centers. In addition to less blood loss, the advantages include faster postoperative recovery with a correspondingly shorter hospital stay and all this with an oncological safety comparable to open procedures. Robotics provide additional advantages over laparoscopy, especially in complex resections such as posterosuperi

[Guidelines for enhanced recovery after elective colorectal surgery].

Early recovery after surgery provides patients with all means to counteract or minimize the deleterious effects of surgery. This concept is suitable for a surgical procedure (e.g., colorectal surgery) and comes in the form of a clinical pathway that covers three periods (pre-, intra- and postoperative). The purpose of this Expert panel guideline is firstly to assess the impact of each parameter usually included in the rehabilitation programs on 6 foreseeable consequences of colorectal surgery: surgical stress, postoperative ileus, water and energy imbalance, postoperative immobility, sleep alt

Laparoscopic versus robotic surgery for colorectal cancer: an updated meta-analysis with comparison against published systematic reviews.

The comparative effectiveness of robotic versus laparoscopic surgery for colorectal cancer remains debated. We performed an updated meta-analysis through June 2026, with cross-validation against published systematic reviews. PubMed was searched for comparative studies of laparoscopic versus robotic colorectal surgery. After deduplication, 51 primary studies met inclusion criteria; three verified published meta-analyses (2023-2025) were cross-referenced. Meta-analysis used Mantel-Haenszel odds ratios (OR) with DerSimonian-Laird random-effects models, computed as robotic/laparoscopic (OR > 1 ind
