Community Collaboration to Advance Racial/Ethnic Equity in Colorectal Cancer Screening: Protocol for a Multilevel Intervention to Improve Screening and Follow-up in Community Health Centers.

INTRODUCTION: Colorectal cancer (CRC) screening utilization is low among low-income, uninsured, and minority populations that receive care in community health centers (CHCs). There is a need for evidence-based interventions to increase screening and follow-up care in these settings. METHODS: A multilevel, multi-component pragmatic cluster randomized controlled trial is being conducted at 8 CHCs in two metropolitan areas (Boston and Los Angeles), with two arms: (1) Mailed FIT outreach with text reminders, and (2) Mailed FIT-DNA with patient support. We also include an additional CHC in Rapid C…

The Impact of the COVID-19 Pandemic on Community Health Centers in Canada and the United States: A Scoping Review.

Community health centers (CHCs) play a critical role in addressing the health needs of diverse and underserved communities. However, there remains a significant gap in the literature examining their resilience, particularly in crises such as COVID-19. This scoping review analyzed 68 articles to identify which performance indicators have been used to assess the impact of the pandemic on CHCs, and to understand what these indicators reveal about the CHCs' performance and resilience. The findings revealed that most performance indicators focused on access, availability, and quality of care, highl…

Social risk factors and cardiovascular risk in obstructive sleep apnea: a systematic assessment of clinical predictors in community health centers.

We leveraged electronic health record (EHR) data from the Accelerating Data Value Across a National Community Health Center Network (ADVANCE) Clinical Research Network (CRN) to identify social risk factor clusters, assess their association with obstructive sleep apnea (OSA), and determine relevant clinical predictors of cardiovascular (CV) outcomes among those experiencing OSA. Geographically informed social indicators were used to define social risk factor clusters via latent class analysis. EHR-wide diagnoses were used as predictors of 5-year incidence of major adverse CV events (MACE) using

Identifying Avoidable Emergency Department Visits and the Potential Role of Community Health Centers in Northern Italy: A Retrospective Observational Study.

BACKGROUND: Emergency departments (EDs) are experiencing a growing burden of non-urgent presentations, which frequently signal deficiencies in access to primary and community-based healthcare. Evidence suggests that a substantial proportion of ED visits could be safely and effectively managed within community care settings. The general aim of the study was to identify which ED visits could have been managed within primary and community care services in a scenario of full implementation of community health centers. METHODS: A retrospective observational study was conducted. This study examined…

"An Act of Complete Care": Provider Perspectives on Linking Maternal Contraceptive Care With Well-Baby Visits in Community Health Centers.

BACKGROUND: Short inter-pregnancy interval (IPI) is associated with adverse health outcomes for women and infants, and low-income women experience disproportionate rates of short IPI. An essential solution is providing postpartum (PP) women with timely contraceptive care. However, patient-centered approaches for facilitating care access are needed. OBJECTIVE: To explore Community Health Center (CHC) staff and provider perspectives on the implementation of a clinical trial offering co-scheduled well-infant/maternal contraceptive care for women with infants 0 to 6 months at the Well-Baby Visit

A Nurse-Run Public Health Clinic for Individuals Utilizing a Community Outreach Center.

This article describes a nurse-run clinic for low-income individuals or individuals experiencing homelessness. We conducted a quality improvement project using data from 111 individuals over 194 visits. Thirty-seven percent of patients had difficulty managing their health and 22% reported being in an active health crisis. The visits resulted in patients leaving with their visit goal being met (86%), and 96% of nonemergent cases being diverted from the emergency room. The nurse-run clinic provided access to care and served as a safety net for a vulnerable population, providing support for this

Preventive counseling for chronic disease: missed opportunities in a community mental health center.

BACKGROUND: The tremendous burden of cardiovascular risk among persons with serious mental illness underscores a critical need for prevention. Counseling by primary care clinicians increases patient smoking cessation, physical activity, and the consumption of fruits and vegetables. The extent to which community mental health clinicians counsel about cardiovascular risk factors has not been reported. METHODS: This cross-sectional study examines the rates of counseling about cardiovascular risk factors by mental health providers at an urban community mental health center (n = 154). Logistic reg

Identification of patients with coexisting alcohol and cocaine disorders at a community mental health center.

To measure the prevalence of cocaine abuse at a community mental health center's inpatient alcohol unit, a 20 item document was used to obtain information from the medical records of 162 patients admitted from January 1988 through July 1988. The data collected were analyzed to evaluate the assessment, diagnosis, treatment and referral of patients with coexisting alcohol and cocaine disorders. Of the 162 patients in the sample, 154 (95%) had a diagnosis of alcohol abuse/withdrawal and five (3%) had a diagnosis of cocaine abuse. Fifty-five patients had used cocaine for at least one year and 39 (

Academic Health Center-Community Justice Program Partnerships: Linking Men in the Justice System to Health Care.

BACKGROUND: Linkage to community health and social services after incarceration is often inadequate. This is troubling, because risk taking and poor health are common immediately after release, and access to health providers and insurance helps individuals to transition home. OBJECTIVES: This article describes how faculty and clinicians at a large academic health center in New York City, established partnerships with justice programs to jointly develop services that aimed to increase health care access for men in the justice system. METHODS: We describe how faculty and students from Columbia

Commentary: Sustaining the Community Health Center Model: Urgency of Primary Care Investment.

Community Health Centers (CHCs) provide care to 32.5 million low-income patients across high need areas, forming the largest primary care network in the US. CHCs consistently improve outcomes and lower costs, yet face growing financial stress. Rising demand, workforce shortages, and uncompensated care costs are stretching limited budgets at CHCs, while Medicaid payments and federal funding do not reflect the true cost of care. Many CHCs lack financial reserves to modernize or join new payment models. This article argues for long-term investments to maintain access to affordable and comprehensi…
