Recognition and management of acute noninfectious dacryocystic retention.

Eleven female and 4 male patients presented with 20 documented episodes of acute noninfectious dacryocystic retention. This syndrome complex consists of severe pain and tearing with minimal but tender distention of the lacrimal sac, without signs of inflammation. It is caused by impaction of a dacryolith in the nasolacrimal duct. In our group of patients, the mean age when seen was 39 +/- 14 years, and the mean onset of symptoms occurred at age 35 +/- 13 years. Six episodes resolved spontaneously, and the rest responded to interventional therapy. In six, irrigation relieved the symptoms. One p

Dacryocystectomy for the treatment of dacryocystitis patients with Wegener's granulomatosis.

We evaluated a woman with Wegener's granulomatosis who was experiencing recurrent dacryocystitis. Nasolacrimal duct obstruction had been present for many years and epiphora was not a significant concern. Our experience with the development of nasalcutaneous fistulas following dacryocystorhinostomy in several patients with Wegner's granulomatosis led us to perform a dacryocystectomy. The functional result was good, and there were no problems with wound healing. Dacryocystectomy should be considered as an alternative to dacryocystorhinostomy for dacryocystitis in patients with Wegener's granulom…

Research on animal models of chronic dacryocystitis in dogs.

OBJECTIVE: To explore an ideal animal model of chronic dacryocystitis. METHODS: (1) The anatomical structure of the lacrimal drainage system in dogs was observed; (2) The canine lacrimal drainage system was obstructed by cauterization of the nasolacrimal duct; (3) A canine model of dacryocystitis was established by combining nasolacrimal duct cauterization with inoculation of Staphylococcus aureus. RESULTS: Compared with inoculation of Staphylococcus aureus one week after nasolacrimal duct cauterization, the combined method of nasolacrimal duct cauterization and immediate inoculation of Stap…

Inverted Papilloma Presenting as Orbital Cellulitis and Dacryocystitis.

A 66-year-old female with a past medical history of psoriasis and chronic sinusitis presented with left orbital pain, proptosis, ptosis, and blurry vision, concerning for orbital cellulitis. Imaging revealed an enhancing soft tissue mass centered in the left anterior ethmoid cavity with extension into the nasolacrimal duct, with associated osseous erosion and orbital inflammatory changes consistent with ipsilateral orbital cellulitis. Endoscopic surgical resection was performed, and histopathology confirmed inverted papilloma without high-grade dysplasia or carcinoma. While orbital involvement

Pilomatrixoma in a Young Adult With Epiphora Mimicking Dacryocystitis.

Pilomatrixoma is a rare benign tumor originating from hair follicle matrix. It is commonly misdiagnosed clinically. The authors present an unusual case of a Pilomatrixoma in a young adult with epiphora mimicking dacryocystitis. An 18-year-old female presented with right eye tearing and nodular mass (12 mm × 10 mm) localized under the medial canthal tendon, resembling acute dacryocystitis for 2 months. Following 1-week oral antibiotic treatment, nasolacrimal duct obstruction was detected with lacrimal irrigation. On palpation, the mass was nontender, painless, reddish in color, and hard in cons…

Microbial spectrum and antibiotic sensitivity in infantile dacryocystitis.

PURPOSE: To identify the characteristics and the spectrum of microbial agents of infantile dacryocystitis and to assess the trends in both antibiotic sensitivities and pathogens over the past 10 years. METHODS: The microbial and medical records of 546 culture-proven patients (546 eyes) of infantile dacryocystitis diagnosed at Henan Eye Hospital between January 2009 and December 2018 were retrospectively reviewed. Patient demographics, microbial analysis, and susceptibility rates to various antibiotics were done. A chi-squared test for trends was applied to evaluate changes in antibiotic susce…

Surgical Treatment of Dacryocystitis by Using a Venous Catheter.

Aim of this work is to highlight the advantages of a new surgical technique performed over 15 patients affected dacryocystitis and treated by using a venous catheter. The arrangement of a 30 mm venous catheter used at the end of a dacryocystectomy (DCT) was performed in 15 patients with dacryocystitis. The venous catheter was removed at 30 days after the surgery. The use of the venous catheter allows washing the lacrimal drainage system, reducing the risk of postoperative infections and supports the recanalization of the lacrimal drainage system 30 days after surgery. The presented cases demonstr…

Panophthalmitis and Visual Loss as a Complication of Acute Dacryocystitis.

Acute dacryocystitis causing visual loss due to development of orbital complications like central retinal artery or ophthalmic artery occlusion, retinal vein occlusions, and optic neuritis is rare but a known entity. However, development of scleral abscess and panophthalmitis as a complication of acute dacryocystitis, leading to loss of vision, has not been described earlier. To the best of the authors' knowledge, this is the first report describing this mode of visual loss in acute dacryocystitis.

Dacryoendoscopy for dacryocystitis management in a dog: A case report.

A castrated, mixed-breed, 10-year-old male dog developed a skin fistula from a ruptured periorbital swelling on the right eye; the patient was successfully treated with dacryoendoscope-guided cannulation using a nasolacrimal tube. On presentation, computed tomography with concurrent dacryocystography (CT-DCG) revealed obstruction of the right nasolacrimal duct and abnormal flow of contrast agents towards the root of tooth 109 at the right maxillary bone; however, the origin of the fistula remained unclear on CT-DCG. Dacryoendoscopy was used to confirm the physical association between the fistu

IgG4-related dacryocystitis and aortitis: A new association.

A 73-year-old woman with a history of muscular weakness and dyspnoea of unknown etiology was referred to our Ophthalmology Department for dacryocystitis. Lacrimal sac biopsy revealed IgG4 plasma cell infiltration and systemic diagnosis was done based on this, allowing an appropriate treatment to be established. To our knowledge, this is the first reported case of IgG4-related dacryocystitis associated to aortitis.

The timing of antibiotic administration in the management of infant dacryocystitis.

PURPOSE: To report (1) the prevalence of bacteremia among infants with dacryocystitis and (2) the influence of timing of antibiotic administration on the need for repeat probing in the management of these patients. METHODS: A retrospective analysis of the hospital records of 25 infants < or =6 weeks of age treated for acute dacryocystitis was conducted, including analysis of laboratory data and outcomes. RESULTS: Of 22 infants who underwent blood cultures, 5 (22.7%) were bacteremic. Twenty-one of the 25 infants underwent nasolacrimal duct probing. Infants who received preoperative antibiotic…

[Diagnosis and treatment of chronic epiphora and recurrent dacryocystitis].

The goal of this article is to highlight relevant diagnostic tools to assess chronic epiphora and recurrent dacryocystitis in order to avoid unnecessary diagnostic explorations. Probing of the canaliculi is important for the differential diagnosis of chronic epiphora. Surgical steps are detailed, such as the inferiorly based mucosal flap on the AGGER NASI, which is repositioned at the end of the surgery improving wound healing on the lateral wall. Potential complications are presented, as well as measures to avoid and to treat them. Long-term follow-up of 750 endoscopic DCR confirms the good r…
