BACKGROUND: Duchenne muscular dystrophy (DMD) is the most common muscular dystrophy in the pediatric population. The manifestations of this disease include progressive muscle weakness, gait dysfunction, and motor impairment, leading to a loss of ambulation by the age of 13 years. Molecular diagnosis is the standard diagnostic tool for DMD. This study aimed to investigate disease progression and genetic patterns in Iranian ambulant boys and to find the correlation between genotypes and motor function phenotypes.
METHODS: This study was performed on 152 DMD patients. Clinical history, including

INTRODUCTION: Many low-income and middle-income countries lack an organised emergency transportation system, leaving people to arrange informal transport to hospital in the case of a medical emergency. Estimating the effect of implementing an emergency transport system is impractical and expensive, so there is a lack of evidence to support policy and investment decisions. Alternative modelling strategies may be able to fill this gap.
METHODS: We have developed a spatial-epidemiological model of emergency transport for life-threatening conditions. The model incorporates components to both pred

Domestic Violence in Old Age: Prevention and Intervention Abstract. Abuse of the eldery occurs frequently. It is experienced most often as psychological abuse (20 %), less often as financial exploitation (4 %) or physical abuse (3 %). The most important risk factor for the elderly is dependence on regular help, especially because of dementia. Caregivers are at a high risk for abuse if they are psychologically conspicuous, live with the dependent or are financially dependent from them. Additional risks are social isolation, unsolved conflicts and a history of domestic violence. The most importa

INTRODUCTION: The incidence of hip dislocation (HD) in arthrogryposis multiplex congenital ranges from 15 to 30 %. Besides a stable hip, the ambulation potential of an AMC child is also dependent on severity of associated knee and foot deformations. The primary objective of this review is to determine the proportion of ambulators in AMC children treated by open reduction for HD.
METHODS: We searched major electronic bibliographic databases for reports on the treatment of HD among AMC children. Based on the surgical approach for open reduction of HD in AMC children, we divided the included stu

Thirty-three children (10 to 15 years of age) with myelomeningocele were studied to determine isometric muscle strength of hip and knee extension, range of hip and knee extension, usual and maximal ambulatory velocities, maximal aerobic capacity, and the energy cost (oxygen uptake) of ambulation versus wheelchair usage in subjects who both walked and wheeled. Subjects were placed into one of four groups depending on their level of motor function (those with motor levels at L2 and above, L3-4, L5 to sacral, and without motor deficit). Maximal ambulatory velocity correlated with strength of the

BACKGROUND: Duchenne muscular dystrophy (DMD) is a fatal disease for which no cure is available. Clinical trials have shown to be largely underpowered due to inter-individual variability and noisy outcome measures. The availability of biomarkers able to anticipate clinical benefit is highly needed to improve clinical trial design and facilitate drug development.
METHODS: In this study, we aimed to appraise the value of protein biomarkers to predict prognosis and monitor disease progression or treatment outcome in patients affected by DMD. We collected clinical data and 303 blood samples from

Patients with complete paraplegia after spinal cord injury (SCI) are unable to stand or walk on their own. Standing exercise decreases the risk of decubitus ulcers, osteoporosis, and joint deformities in patients with SCI. Conventional gait training for complete paraplegia requires excessive upper limb usage for weight bearing and is difficult in cases of complete quadriplegia. The purpose of this study was to describe voluntary ambulation triggered by upper limb activity using the Hybrid Assistive Limb® (HAL) in patients with complete quadri/paraplegia after chronic SCI. Four patients (3 men,

In a series of experiments, prenatal administration of phenytoin to rats at non-embryotoxic and non-teratogenic doses produced offspring exhibiting large and enduring abnormalities in behavior. Plasma drug concentrations on the last day of exposure in the dams were within the human therapeutic range for anticonvulsant efficacy. The behavioral abnormalities in the offspring were hyperactivity on tests of locomotion (increased pivoting behavior and figure-8 ambulation), but decreased figure-8 rearing. As adults, the phenytoin rats exhibited no impairment in swimming ability in a straight channel

Better understanding of older patients' perspective on physical, motivational and environmental barriers and enablers in physical activity promotion during hospitalization contributes to the development of strategies to increase in hospital physical activity. Using a mixed method approach, barriers and enablers from patients (in)dependent in physical activity (Functional Ambulation Categories ≥4 versus <4) were identified. Forty-nine patients (median age 77 years IQR:75-83, female: n = 25) were interviewed. Barriers reported as most important were feeling weak (n = 10), having lines/drains (n

Pregnant rats on the eighth day of gestation (GD 8) received 2 intraperitoneal injections (0.015 ml/g body weight) spaced by an interval of 4 hours, of either 0, 6, 12 18, or 24% (v/v) alcohol solution. Brains of pups sacrificed 24 hours after delivery revealed a dose-related impairment in hemispheric and cerebellar length, width and weight. Statistically significant differences were also observed in body weights of culled pups during Post-partum Day 1. An open field test performed when offspring were 40-45 days old demonstrated a dose-dependent increase in ambulation and rearing scores for bo

BACKGROUND: Parkinson's disease (PD) is associated with gait and visuomotor abnormalities, but it is not clear where PD patients look during ambulation.
OBJECTIVE: We sought to characterize the visual areas of interest explored by PD patients, with and without freezing of gait (FOG), compared to healthy volunteers (HVs).
METHODS: Using an eye-tracking device, we compared visual fixation patterns in 17 HVs and 18 PD patients, with and without FOG, during an ambulatory and a nonambulatory, computer-based task.
RESULTS: During ambulation, PD patients with FOG fixated more on proximal areas of

The objective of this study was to determine the prevalence, circumstances, and outcome of fractures in males with Duchenne muscular dystrophy (DMD) attending neuromuscular clinics. Three hundred and seventy-eight males (median age 12 years, range 1 to 25 years) attending four neuromuscular centres were studied by case-note review supplemented by GP letter or by interview at the time of clinic attendance. Seventy-nine (20.9%) of these patients had experienced fractures. Forty-one percent of fractures were in patients aged 8 to 11 years and 48% in independently ambulant patients. Falling was th

BACKGROUND: The world is facing many socio-demographic changes, such as an increased average life expectancy and the presence of chronic and non-communicable diseases, which in turn, leads to an enhanced dependency on others. Consequently, the demand for informal caregivers has significantly increased during the past few years. Caring for a dependent person is linked to a series of burdens that often leads to physical, psychological and emotional difficulties. Taking into consideration the difficulties faced by informal caregivers, knowing in which areas of functioning they need more guidance

Distal humerus fractures in the elderly are a difficult problem to treat. Open reduction internal fixation has a high rate of complications, particularly because osteopenia compromises what is already tenuous fixation in a metaphyseal fracture. Total elbow arthroplasty is a more predictable outcome and easier recovery for these patients. However, most surgeons perform a low volume of total elbow arthroplasty. In addition, traditional exposure requires detachment of the triceps tendon. The lateral paraolecranon approach maintains the central tendon attachment to the olecranon while still facili

Five hundred lower limb amputees were evaluated by personal interview at the Artificial Limb Centre, Pune, India. Information thus obtained on activities of daily living and functional capabilities with their prostheses was analysed. The analysis showed that 55% of the amputees were totally independent, 40% had to use a crutch or cane and only 5% were solely dependent on a wheelchair for ambulation. Further, it was also confirmed that as age increased, functional independence decreased, and that below-knee amputees were more independent than above-knee and bilateral amputees. When compared wit
