PURPOSE: Opioids and benzodiazepines (BZDs) are frequently implicated as contributing to falls in older adults. Deprescribing of these medications continues to be challenging. This study evaluated primary-care prescribers' confidence in and perceptions of deprescribing opioids and BZDs for older adults.
METHODS: For this study, we conducted a quantitative analysis of survey data combined with an analysis of qualitative data from a focus group. A survey evaluating prescriber confidence in deprescribing opioids and BZDs was distributed to providers at 15 primary-care clinics in North Carolina b

Proton pump inhibitors (PPIs) are potent inhibitors of gastric acid secretion that have changed treatment practice for gastric acid-related disorders. The major adequate indications for their use are treatment of gastro-esophageal reflux disease, peptic ulcers, eradication of Helicobacter pylori infection in combination with antibiotics and prophylaxis for patients on non-steroidal anti-inflammatory or antiplatelet drugs. Since their introduction, clinical success has been accompanied by widespread use of PPIs, which has steadily increased over the last decades without concomitant increase in

INTRODUCTION: Older people face higher risks of medicine-related harm due to polypharmacy and the use of potentially inappropriate medicines. Current treatment guidelines rarely specify when to stop medicines, leading to medicines often being continued indefinitely without a clear deprescribing plan. While deprescribing guidelines exist for some medicine classes, limited guidance is a major barrier to deprescribing. These new guidelines address this gap by providing structured recommendations that complement more detailed drug-specific deprescribing guidance, disease-specific therapeutic guide

BACKGROUND: Deprescribing decisions in palliative care involve complex interactions among multiple stakeholders, yet the experiences and needs of those involved remain poorly understood and lack systematic integration. OBJECTIVES: This review aims to synthesize qualitative evidence on the experience, needs and influencing factors of multi-stakeholder in deprescribing decisions within palliative care, and to identify the core challenges faced in clinical practice. DESIGN: A systematic review and meta-synthesis of qualitative studies. METHODS: Six electronic databases (PubMed, CINAHL, EMBASE, We

Multimorbidity and multicomplexity are increasingly becoming the norm in geriatric clinical practice, with a recent Indonesian multicentre study revealing that 80.7% of older outpatients present with multiple coexisting conditions. This editorial discusses the limitations of conventional single-disease guidelines when applied to complex older adults, where simultaneous treatments often lead to polypharmacy, cumulative anticholinergic burdens, and potential adverse outcomes. Highlighting findings from a recent study in Acta Medica Indonesiana, which found polypharmacy in 43.9% of Indonesian ger

OBJECTIVES: To evaluate the effect of deprescribing interventions that explicitly targeted psychotropic medications on fall outcomes in older adults across community, inpatient, long-term care, and postdischarge settings.
DESIGN: Systematic review and meta-analysis.
SETTING AND PARTICIPANTS: Older adults (≥65 years).
METHODS: We searched MEDLINE, Embase, Cochrane CENTRAL, Web of Science, and CINAHL from inception to June 2025. We included randomized controlled trials (RCTs) and high-quality nonrandomized studies of interventions (NRSIs) that compared structured deprescribing or medication-o

BACKGROUND: Central nervous system (CNS) active medications have been consistently linked to falls in older people. However, few randomized trials have evaluated whether CNS-active medication reduction reduces falls and fall-related injuries. The objective of the Reducing CNS-active Medications to Prevent Falls and Injuries in Older Adults (STOP-FALLS) trial is to test the effectiveness of a health-system-embedded deprescribing intervention focused on CNS-active medications on the incidence of medically treated falls among community-dwelling older adults.

AIMS: To provide posthoc analyses of a clinical trial that reported beneficial effects of medication reviews on health-related quality of life. Specifically, to describe the medication changes with a focus on deprescribing and to explore patient- and medication-related factors that may identify patients most likely to benefit from medication reviews.
METHODS: Posthoc analyses of data from a pragmatic, nonblinded, randomized clinical trial investigating a medication review intervention (NCT03911934) in 408 geriatric outpatients treated with ≥9 medicines.
RESULTS: In the medication review grou

OBJECTIVES: Prevention of falls and fall-related injuries is a priority due to the substantial health and financial burden of falls on patients and healthcare systems. Deprescribing medications known as 'fall-risk increasing drugs' (FRIDs) is a common strategy to prevent falls. We conducted a systematic review to determine its efficacy for the prevention of falls and fall-related complications.
DESIGN: Systematic review and meta-analysis.
DATA SOURCES: MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, CINAHL and grey literature from inception to 1 August 2020.

Deprescribing is defined as "the planned and supervised process of dose reduction or stopping of medication that might be causing harm, or no longer be of benefit". Barriers to deprescribing include healthcare professional fear and lack of guidance. These may stem from limited available evidence on benefits and harms of deprescribing medications commonly used among older persons. Advances in pharmacoepidemiology and causal inference methods to evaluate comparative effectiveness and safety of prescribing medications have yet to be considered for deprescribing medication. This paper discusses se

BACKGROUND: Polypharmacy is defined as the prescription of at least 5 different medicines for therapeutic or prophylactic effect and is a serious issue among elderly patients, who are frequently affected by multi-morbidity. Deprescribing is one of the proposed approaches to reduce the number of administered drugs, by eliminating those that are inappropriately prescribed. The aim of this systematic review is to provide an updated and systematic assessment of the benefit-risk profile of deprescribing of anti-hypertensive drugs, which are among the most commonly used drugs.

BACKGROUND: Long-term sedative use is prevalent and associated with significant morbidity, including adverse events such as falls, cognitive impairment, and sedation. The development of dependence can pose significant challenges when discontinuation is attempted as withdrawal symptoms often develop. We conducted a scoping review to map and characterize the literature and determine opportunities for future research regarding deprescribing strategies for long-term benzodiazepine and Z-drug (zopiclone, zolpidem, and zaleplon) use in community-dwelling adults.

BACKGROUND: Antidepressants are recommended for moderate-to-severe depression and anxiety, but concerns exist around overprescribing, long-term use, and paucity of evidence-based deprescribing strategies. We aimed to compare the effectiveness of different deprescribing approaches in individuals with clinically remitted depression or anxiety.
METHODS: For this systematic review and network meta-analysis, we searched PubMed, PsycINFO, Web of Science, CENTRAL, CINAHL, and online trial registries from inception to April 6, 2025, for randomised controlled trials comparing abrupt discontinuation, f

OBJECTIVES: Deprescribing is increasingly recognized as an effective strategy for reducing potentially inappropriate medication (PIM) use and polypharmacy in older adults. However, existing deprescribing frameworks often lack detailed operational guidance, which may hinder their routine implementation in clinical practice. To systematically map and characterize the reported deprescribing processes in older adults with PIM use or polypharmacy and identify gaps in their operationalization.

BACKGROUND: Deprescribing is beneficial in reducing medication burden and improving patient outcomes. Deprescribing success is influenced by both patient and health care professional barriers and enablers. Pharmacists can play an important role in deprescribing success, yet little is known about this profession's specific barriers and enablers to deprescribing.
OBJECTIVES: The primary objective of this study was to summarize the literature on pharmacists' barriers and enablers to deprescribing across multiple healthcare settings.
METHODS: A systematic review of the available literature was c
