The many faces of phytophotodermatitis.

Phytophotodermatitis is a common cutaneous phototoxic reaction produced by contact with a variety of plant substances, followed by sunlight exposure. Phytophotodermatits can simulate child abuse, impetigo, superficial lymphangitis, and larva migrans. The most frequent presentation is hyperpigmentation of the dorsum of the hands, but there are other less common findings such as blisters or lesions mimicking solar lentigines and melasma.

Ultraviolet recall dermatitis reaction with sorafenib.

PURPOSE: Recall dermatitis is a rare and poorly understood drug-related event. Activated by exposure to sunlight or Ultraviolet (UV), drug-related phototoxic reactions have been reported with conventional chemotherapy agents or antibiotics. METHODS: Here, we report the first case of acute dermatologic photo-induced recall reaction secondary to sorafenib in a patient with renal cell carcinoma. RESULTS: Four weeks after stopping sorafenib, a patient with renal cell carcinoma developed an acute erythematous and papulomatous eruption restricted to the hands after prolonged solar exposure. The er

Photoallergy.

Many--topically applied or systemically administered--substances may trigger photoallergic skin reactions following exposure to ultraviolet (UV) radiation. While most cases represent photoallergic contact dermatitis due to topically applied agents, systemically triggered photoallergy is considerably less common. The photopatch test--a UV-exposed variant of the conventional patch test--is the mainstay in the diagnosis of photoallergic or phototoxic reactions.

Photodermatitis with minimal inflammatory infiltrate: clinical inflammatory conditions with discordant histologic findings.

Dermatoses associated with cutaneous photosensitivity are a group of photodistributed skin eruptions caused or exacerbated by light. Multiple clinical variants of photosensitive dermatoses have been characterized including polymorphous light eruption, chronic actinic dermatitis, solar urticaria, phototoxic and photoallergic dermatitis, reticular erythematous mucinosis, acute cutaneous lupus erythematosus, and dermatomyositis. As there may be significant overlap among the clinical presentation of these conditions, the specific diagnosis of individual photodermatosis relies heavily on characteri

Investigation of an outbreak of photodermatitis confined to one shop in a large factory.

Out of 106 men employed in a foundry attached to a large factory, 29 men developed a photosensitization dermatitis over the period of three months. Three men out of 1,700 employed in the remainder of the works developed a similar eruption, but no common factor was apparent. Owing to the very high incidence in the foundry, a sensitizer associated with the process was suspected and it seemed likely that it would prove to be an atmospheric contaminant. Many chemical investigations and skin tests were carried out without positive result, though much interesting information regarding combustion pro

Contact dermatitis II. Clinical aspects and diagnosis.

Contact dermatitis (CD) is an altered state of skin reactivity induced by exposure to an external agent. "Eczema" and "dermatitis" are often used synonymously to denote a polymorphic pattern of inflammation of the skin characterized, at least in its acute phase, by erythema, vesiculation and pruritus. Substances that induce CD after single or multiple exposures may be irritant or allergic in nature. The clinical presentation may vary depending on the identity of the triggering agent and the reactivity of the subject, but in all cases the lesions are primarily confined to the site of contact. A

Parthenium dermatitis.

Allergic contact dermatitis (ACD) to Parthenium hysterophorus is the most common cause of plant dermatitis in India. Parthenium dermatitis is caused by dry powder of leaves and flowers and hair-like structures (trichomes). Sesquiterpene lactones (SQLs) are the most important allergens responsible for ACD to parthenium. The different patterns include classical airborne contact dermatitis, chronic actinic dermatitis (CAD), exfoliative and widespread dermatitis. There is a definite trend towards a change from an airborne pattern to a CAD pattern in the natural history of parthenium dermatitis. In

Feeling the Burn: Phototoxicity and Photoallergy.

An interaction between light's radiation and certain exogenous and endogenous substances can lead to the development of photoallergic and/or phototoxic dermatoses. Clinically, reactions may range from acute and self-limited to chronic and recurrent. Delays in diagnosis are not uncommon due to complex clinical presentations, broad differentials, and limited number of specialists who perform phototesting. Therefore, a critical understanding of these dermatoses is essential for accurate diagnosis and appropriate management. The epidemiology, light sources, mechanisms, clinical presentations, eval

Photosensitivity to exogenous agents.

OBJECTIVE: To better understand cutaneous photosensitivity reactions, a review of its etiologic factors, clinical characteristics, pathogenesis, and treatment modalities was undertaken. METHODS: Articles discussing the above aspects of phototoxic and photoallergic reactions were used to demonstrate what is currently known about photoinduced reactions and how to treat them. RESULTS: Upon interaction of solar UV radiation with the chemical that is present in significant levels on the skin, one of two known reactions may occur in susceptible patients: a phototoxicity and/or photoallergy. Photot

Phototoxicity and photoallergy.

Photosensitivity may be phototoxic or photoallergic. Phototoxicity is much more common. There are 2 types of phototoxicity: photodynamic, which requires oxygen, and nonphotodynamic, which does not. Reactions induced by porphyrin molecules, coal tar derivatives, and many drugs are photodynamic. The reaction induced by psoralens, for the most part, is nonphotodynamic. Acute phototoxic reactions are characterized by erythema and edema followed by hyperpigmentation. Long-term ultraviolet phototoxicity results in chronic sun damage and skin cancer formation. Also, certain chemicals such as psoralen
