Situs inversus with dextrocardia is both a clinical and diagnostic challenge for emergency physicians to properly identify acute coronary syndrome. While dextrocardia itself does not independently increase the risk of coronary artery disease, mirrored symptoms, including right-sided and rightward radiating chest pain in any patient with cardiac risk factors should raise suspicion for acute coronary syndrome. In patients with a reversed cardiac silhouette on a chest radiograph, a reversed electrocardiogram, to include both the precordial and limb leads, is necessary to evaluate for cardiac isch…

A 49-year-old woman with dextrocardia and situs inversus underwent catheter ablation of paroxysmal atrial fibrillation (AF). During the electrophysiologic study, AF triggered by frequent premature atrial contractions (PACs) with a short coupling interval exhibiting a "P on T" pattern occurred. Pulmonary vein mapping revealed that those PACs originated from right-sided (anatomic left) or left-sided (anatomic right) pulmonary veins. In this case with mirror-image dextrocardia, the P-wave morphologies in leads I and aVL and the II/III ratio of the P-wave amplitude were helpful for predicting a ri…

OBJECTIVE: The educational objectives of this article are to describe an approach to analyzing imaging studies in adults with dextrocardia and to present the appearances of the most common underlying disorders. Topics reviewed include the morphology of the cardiac chambers, the concept of situs, and the relevant embryologic principles. The disorders discussed include situs inversus totalis (mirror-image dextrocardia), dextroversion, congenitally corrected transposition of the great arteries, and polysplenia syndrome.
CONCLUSION: In this article we describe an approach to dextrocardia in adult…

Dextrocardia is a rare anomaly affecting 0.01% of newborns. We report on a case of a 60-year-old female with mirror-image dextrocardia who presented with an acute myocardial infarction and underwent primary angioplasty of the left circumflex artery. A review of the literature and technical challenges of the case will be addressed.

Coronary arteriography is described in a patient with dextrocardia and situs inversus totalis. Six cases of coronary arteriography in dextrocardia were reviewed from the literature, and 3 other cases were reported from the authors' institutions. In 9 of the 10 patients, coronary arteriography was performed without difficulty using standard Judkins or Sones catheters. Angiographers encountering patients with dextrocardia may use standard coronary arteriographic techniques with a few subtle modifications.

This is a serial section examination of the conduction system of five hearts of various types of dextrocardia. In one case of dextroversion the conduction system was found to be normal. In one case of mirror-image dextrocardia the conduction system was normal but in the opposite spatial position. In one heart of mixed (discordant) dextrocardia, the sinoatrial node was in correct morphologic position. There were two atrioventricular nodes - a posterior node close to the coronary sinus and an anterior node in the roof of the morphologic right atrium. They formed two separate bundles which joined…
