The moral-behavioural logic of silence: gender, stigma, and disclosure among long-term HIV survivors.

Silence, the deliberate non-disclosure of HIV status - remains an overlooked yet central aspect of long-term antiretroviral therapy (ART) survivorship. While disclosure is promoted as empowerment, such framings may neglect the gendered, relational, and institutional contexts in which non-disclosure occurs. We conducted in-depth interviews with 21 long-term ART users (10-22 years on treatment) in Tamil Nadu, India, and analysed narratives using stigma, social identity, emotion work, spiritual reframing, and institutional betrayal lenses. Silence emerged not as absence, but as a moral-behavioura

Psychological variables potentially implicated in opioid-related mortality as observed in clinical practice.

Opioid-related deaths in the United States have become a public health problem, with accidental and unintended overdoses being especially troubling. Screening for psychological risk factors is an important first step in safeguarding against nonadherence practices and identifying patients who may be vulnerable to the risks associated with opioid therapy. Validated screening instruments can aid in this attempt as a complementary tool to clinicians' assessments. A structured screening is imperative as part of an assessment, as clinician judgment is not the most reliable method of identifying nona

Is every pancreatic cancer patient a palliative care patient?

Pancreatic cancer is an aggressive disease and often presents at an advanced stage with no curative options. The disease is often characterized by rapid progression, limited or short-lived responsiveness to standard therapies, and a profound impact on patients' quality of life. Despite advances in targeted therapies and immunotherapy, curative outcomes remain elusive for the majority of patients with advanced or high-grade disease with a 5-year survival rate of less than 10%. Treatment goals must evolve beyond prolonging survival to include alleviating suffering, optimizing function, and prese

Surgical management of gastrointestinal stromal tumors of the stomach.

BACKGROUND: Gastrointestinal stromal tumors (GISTs) are rare tumors of the GI tract with varying degree of dignity and prognosis. Intramural or extragastral growth of gastric GISTs is associated with diagnostic difficulties and uncertainty about the type and extent of surgical therapy. Based on our experience, we tried to formulate management guidelines for gastric GISTs. METHODS: Five patients with gastric GIST (36-85 years old) underwent subserosal excision with subsequent B-II resection (1x), full-thickness partial gastric resection (2x), gastrotomy with submucosal excision (1x), or gastre

Ethics of gene therapy.

CRISPR-Cas systems, base editing, and prime editing have made precise genetic interventions possible, and several approved therapies now treat monogenic disorders that were previously untreatable. Heritable genome editing remains ethically contested. We argue that heritable interventions should not be treated as a single category subject to uniform prohibition. We distinguish three targets: catastrophic monogenic disorders, polygenic risk reduction, and non-disease trait enhancement. For catastrophic monogenic conditions in which preimplantation selection cannot yield unaffected embryos, herit

Surgical therapy of juxtapapillary tumors.

INTRODUCTION: Tumors of the papilla of Vater represent a pathophysiological entity. We propose a precise definition of these tumors and give a summary of diagnostics and therapy as well as a review of the literature. DEFINITION, SYMPTOMS, THERAPY: The most accurate definition includes all tumors originating within one cm of the papilla, regardless of their histology. We therefore use the term of "juxtapapillary tumors". Early symptoms are itching, jaundice, sometimes pain and cholangitis/cholecystitis. The first diagnostic means are sonography, CT-scan, ERCP and angiography. The therapeutic o

Spontaneous Auto-Amputation of the Breast in a Patient With Advanced Malignant Fungating Wound and Distant Metastases Involving the Lungs and Left Upper Extremity: A Case Report.

Malignant fungating wounds in advanced breast cancer cause significant morbidity and psychological distress. Spontaneous auto-amputation of the breast is a rare clinical phenomenon, conventionally linked to neglected tumors rather than actively treated disease. We present a case of spontaneous auto-amputation in an extensively pretreated 60-year-old female with advanced human epidermal growth factor receptor 2-positive (HER2+) breast cancer. She presented to the palliative care unit with refractory left upper extremity pain, severe tactile and thermal allodynia due to malignant brachial plexop

Part 3: Ethics: 2025 American Heart Association Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care.

In this chapter, the American Heart Association provides guidance on ethical considerations relevant to cardiopulmonary resuscitation and emergency cardiovascular care. An overview is provided of ethical frameworks that provide a structure through which difficult decisions can be analyzed. These include principlism, currently the predominant medical ethical framework, which considers moral principles of beneficence, nonmaleficence, respect for autonomy, and justice. Additional consideration is given to the value of dignity, and other ethical frameworks such as narrative ethics, crisis standard

Selection of patients with hepatocellular cancer: a difficult balancing between equity, utility, and benefit.

Although liver transplantation (LT) represents the gold-standard strategy for hepatocellular cancer (HCC), its use is circumscribed by several factors like donor shortage, perioperative complications, or competition with other candidates without HCC. Moreover, different alternative approaches like resection or loco-regional therapies may be attempted in selected cases. The best option for the treatment of an HCC patient is a complex decision, involving several ethical principles including: equity (horizontal equity and vertical equity or urgency), and utility. These principles influence the di

Care for the patient with AIDS in the ICU: outcomes and ethics.

HIV infection and AIDS are common diagnoses in many intensive care units (ICUs) in the United States. Although Pneumocystis carinii currently represents only one quarter of all diagnoses for which HIV-infected persons are admitted to the ICU, it is the disease with the most clinically applicable outcome data and, therefore, is a model for ethical decision-making regarding patients with HIV infection in the ICU. Despite advances in diagnosis and treatment of HIV-related P. carinii, recent studies show that only 20% to 25% of the patients with acute respiratory failure survive to hospital discha

Culturally Meaningful End-of-Life Care for a Patient With Cancer of Unknown Primary, and Severe Motor and Intellectual Disabilities: A Case Report.

Geriatric and palliative care are becoming increasingly complex, particularly for patients with severe motor and intellectual disabilities (SMID). Cancer of unknown primary origin (CUP) is an aggressive malignancy that is often diagnosed at an advanced stage and requires individualized decision-making and careful integration of patient values into care planning. We report the case of a 68-year-old man with SMID who was diagnosed with peritoneal carcinomatosis of unknown primary origin, accompanied by pulmonary and splenic metastases and peritoneal implants. Because he was unable to express his

Psychedelic-assisted therapies for existential and spiritual suffering in palliative care.

Existential and spiritual suffering are frequently reported by individuals facing serious illnesses, particularly at the end of life, and are associated with diminished quality of life, increased psychological distress, and requests for hastened death. While Palliative Care (PC) aims to provide holistic support, existing therapeutic options often fail to adequately address the profound disruptions in meaning, connection, and dignity experienced by patients. Psychedelic-Assisted Therapies (PAT), notably those using psilocybin, have re-emerged as promising interventions capable of eliciting tran

Diseases of the salivary glands in infants and adolescents.

BACKGROUND: Diseases of the salivary glands are rare in infants and children (with the exception of diseases such as parotitis epidemica and cytomegaly) and the therapeutic regimen differs from that in adults. It is therefore all the more important to gain exact and extensive insight into general and special aspects of pathological changes of the salivary glands in these age groups. Etiology and pathogenesis of these entities is still not yet fully known for the age group in question so that general rules for treatment, based on clinical experience, cannot be given, particularly in view of the

Four clinical dilemmas in surgical decision making for low rectal cancer.

Advances in neoadjuvant therapy, imaging, and surgical techniques over the past three decades have significantly transformed the management of low rectal cancer. However, in this anatomically and functionally sensitive region, where decisions intersect with continence, identity, and quality of life, strict adherence to standard protocols is often inadequate. This article explores four recurring dilemmas that influence contemporary surgical decision-making: whether to preserve the sphincter or perform an abdominoperineal resection, whether to create a stoma or avoid it, whether to use minimally

Stigma, deforming metaphors and patients' moral experience of multibacillary leprosy in Sobral, Ceará State, Brazil.

In response to the call for a new Science of Stigma, this anthropological study investigates the moral experience of patients diagnosed with severe multibacillary leprosy. From 2003 to 2006, fieldwork was conducted in the so-called 'United-States-of-Sobral', in Ceará State, Northeast Brazil. Sobral is highly endemic for leprosy, despite intensified eradication efforts and a 30% increase in primary care coverage since 1999. Of 329 active leprosy cases at two public clinics, 279 multibacillary patients were identified and six information-rich cases selected for in-depth ethnographic analysis, ut

Silent images, growing minds.

BACKGROUND: This manuscript reflects on the role of visual art as a gentle and meaningful pathway for supporting the cognitive development of children on the autism spectrum. Rather than approaching autism through deficit based assumptions, it foregrounds the potential of visual engagement as a supportive mode of learning. PURPOSE: The manuscript aims to explore how images, patterns, shapes, and repeated visual interaction may create conditions for focus, memory, recognition, and understanding among autistic children. APPROACH: Through poetic reflection, the manuscript presents visual therap

Palliative Care Needs in Pediatric Hematologic Oncology: Parent-Child Perspectives Across Stages.

CONTEXT: Children with hematologic malignancies undergo prolonged, intensive therapy that imposes physical, psychological, social, and spiritual challenges on patients and families. Although early palliative care is recommended, little is known about how these needs evolve across the illness course, especially in resource-limited settings. OBJECTIVE: To describe the dynamic palliative care needs of children with hematologic cancers and their families and to identify stage-specific opportunities for family-centered support. METHODS: A descriptive qualitative study was conducted at a national

What does oral care mean to society?

Oral care is defined in a narrow sense as cleaning of the teeth, oral cavity, and dentures, and in a broad sense as the maintenance of oral functions (feeding, swallowing, chewing, speech, aesthetics, etc.), dental treatment, feeding and swallowing training, and articulation training. In the past, it was recognized as simply cleaning the mouth, but the concept of oral care has gradually expanded, and many studies and surveys have been conducted in cooperation with various other professions. As a result, oral health care is involved not only in the prevention of pneumonia, but also in the onset

[Updated WHO classification of tumors of the breast: the most important changes].

The development of the WHO classification of tumors of the breast is driven by new knowledge from research whose translation into daily practice is considered clinically relevant. The fifth edition represents an update of the fourth edition and essentially follows the previously known systematics. The histologic features of the lesions continue to form the basis of the classification in the update. This also applies to the definition of invasive tumor types. However, several new molecular classifications as well as additional prognostic and predictive factors are presented and discussed, which

Cybernetic Principles in Psychophysiology: Their Significance and Conclusions for Palliative Care.

Palliative care is dedicated to terminally ill patients with advanced disease, regardless of diagnosis, under the overarching premise of optimizing quality of life. This narrative review examines the extent to which principles of cybernetics and psychophysiology underlie this approach. Psychophysiology researches the physiological equivalents of psychological states and traits such as activation and individual reactivity, the interoception and the personal characteristics. Cybernetics specifies these principles, which are possible by understanding terms such as "psychophysiology" or "cyberneti
