A 31-year-old woman returned to her native Germany, after a two-year stay in Tanzania, suffering from cutaneous diphtheria which had spread to nose and throat. One of her three children, a four-year-old boy, also had Corynebacterium diphtheria in his throat, without any symptoms. Both mother and child had been immunized against diphtheria.

A low dose (1.5 Lf) diphtheria vaccine for use in adults was given to 310 university student volunteers whose diphtheria antitoxin titres were less than 0.1 IU per ml. The incidence of adverse reactions was low. Postimmunisation blood samples taken from 134 of the students showed that in susceptible vaccinees the vaccine induced titres of antitoxin were consistent with protection and that it boosted the titres of those whose immunity was low.

INTRODUCTION: A diphtheria outbreak occurred in 2017 in Jakarta, Indonesia, during which our hospital was appointed as a referral hospital where patients with upper respiratory tract symptoms were sent for confirmation of the diagnosis and medical intervention. Diphtheria is an acute, highly infectious, vaccine-preventable and previously endemic disease whose etiologic agent is Corynebacterium diphtheriae. Diphtheria may manifest as an upper respiratory tract infection, a cutaneous infection or as an asymptomatic carrier state.
