Although predominantly a disease in older adults diverticulitis does affect younger patients. The disease has been described as not only rare but virulent by some authors, and a young patient age is considered to be a relative indication for early sugery. The goal of this study was to evaluate the experience of the Louisiana State University Health Sciences Center-Shreveport and affiliated hospitals with diverticulitis in young patients. This study was a retrospective chart review of 22 patients with diverticulitis age 40 years and younger over the past 20 years. Inclusion criteria were either…

Three men aged 52, 60, and 38 years, complained of dysuria and recurrent cystitis. One patient mentioned pneumaturia, a feature which later appeared to be present in all three. Ultrasound examination showed air in the bladder, and demonstrated the fistulous tract in two cases. Computed tomography identified the third fistula. In all 3 the enterovesical fistula was a complication of sigmoid diverticulitis. After sigmoid resection they recovered well. The key to the diagnosis of enterovesical fistula is to think of it. Pneumaturia and faecaluria are pathognomonic symptoms. Both ultrasound and co…

Diverticulosis of the colon is a disease that mainly affects the elderly, presenting in 10 to 20% of this age group. The most common complication--painful diverticular disease, is usually treated conservatively with a high-fiber diet, intermittent use of antispasmodics and possibly regulation of the stools. Diagnostic differentiation is best performed using ultrasonography of the bowel wall, supplemented where necessary by computer tomography and colonoscopy (sigmoidoscopy) or, where this latter is not possible, barium enema using Gastrografin. Conservative treatment of diverticulitis with ant…

The aim of this work is a critical analysis of our results with primary laparoscopic resection and anastomosis of the colon for diverticulitis. From October 1993 to October 1997, 171 patients with a mean age of 60 years (97 males, 74 females) have been operated laparoscopically. 95 patients were operated electively after many episodes of acute diverticulitis and 76 patients in the acute phase, of whom 11 patients presented a colovesical fistula. 6 left hemicolectomies and 165 sigmoid resections were performed. Among the sigmoid resections 11 patients with simultaneous resection of a colovesica…

Sonographic findings in 16 patients with diverticulitis of the colon are described. The wall of the inflamed segment of the colon appeared hypoechoic and thickened. Maximum thickness of the wall ranged from 5 to 17 mm. Length of the most severely inflamed segment of the colon ranged from 6 to 9 cm. In addition, adjacent contiguous segments of the colon, less involved with inflammation, ranging in length from 7 to 15 cm, were demonstrated on sonographic examination of five patients. Long segments of the inflamed colon could be demonstrated by oblique scanning. The appearance of the mucosal refl…

On the basis of three case reports some aspects of diverticulitis of the caecum and ascending colon are discussed. The surgical treatment of congenital diverticula (mostly situated in the caecum) with inflammation should be as conservative as possible to minimize the risk of postoperative complications. Our two patients I and 2 with diverticulitis of the caecum were both treated by ileocaecal resection; possibly in case I diverticulectomy alone would have been sufficient. Patient 3 was treated by right hemicolectomy for a condition residual to (peri)diverticulitis of the ascending colon. In al…

About half of the patients in whom diverticulitis develops have subsequent attacks. Recurrence increases the likelihood both of complications, such as perforation, abscess, fistula, obstruction, and hemorrhage, and of the necessity for surgical intervention. For these reasons, patients who have had two or more attacks of acute diverticulitis should be considered candidates for prophylactic resection.

Two cases of inflammation of a solitary diverticulum of the caecum are reported. These probably represent the first cases to be reported in Chinese. The pathogenesis of solitary diverticulum of the caecum is different from that of multiple diverticulosis of the colon. A study of these cases was made to understand better this uncommon entity. Before operation, the symptoms, signs and physical findings of caecal diverticulitis are those found in appendicitis. The diagnosis at operation in some cases is difficult because the inflammatory reaction may simulate a malignant process. Local excision i…

In four cases of sigmoidovesical fistula secondary to sigmoid diverticulitis, the diagnosis was made by the findings of pneumaturia and fecaluria. The fistula was visualized in the bladder in two cases. A one-stage operative procedure was used for treating the condition. The patients recovered promptly with no morbidity.

History A previously healthy 28-year-old man developed right lower quadrant pain while traveling. The pain progressed over the course of 2-3 days, and his family took him to a local emergency department. He was found to have an elevated white blood cell count of 12.2 × 109/L (reference range, [3.9-10.3] × 109/L), with a predominance of neutrophils. Contrast material-enhanced computed tomography (CT) of the abdomen and pelvis was performed, and findings were abnormal. The patient elected to leave the emergency department without undergoing treatment, and he returned home via airplane. He presen
